A clinicopathological study on valvular diseases in 3,000 consecutive autopsies of the aged.

The incidence and etiological classification of valvular diseases were examined on 358 cases from 3,000 consecutive autopsies of more than 60 years of age. The incidence of valvular disease was 11.9% (358 out of 3,000 cases). Mitral stenosis was found in 23 cases (6.4%), of which 21 cases were rheumatic and the remaining 2 were mitral ring calcification (MRC). Mitral regurgitation was observed in 126 cases (35.3%): 69 of papillary muscle dysfunction, 26 of mitral valve prolapse (MVP), 16 of MRC, 9 of ruptured chordae tendineae, 3 of rheumatic and 3 of congenital. Aortic stenosis was noted in 33 cases (9.2%): 27 of calcified, 5 of rheumatic and one of congenital. Aortic regurgitation was found in 169 cases (47.2%): 112 of degenerative, 47 of syphilitic, 7 of rheumatic and 2 of aortitis syndrome. There were 6 cases (1.7%) of tricuspid regurgitation. Etiological classification revealed 6 cases (1.7%) of congenital, 36 (10%) of rheumatic, 49 (13.7%) of syphilitic, 27 (7.5%) of MVP, 69 (19.3%) of ischemic and 166 (46.4%) of degenerative valvular disease. The results emphasize the high incidence of non-inflammatory valvular diseases including those of degenerative etiology in the cases studied.

Average urinary excretion of sodium in 24 hours can be estimated from a spot-urine specimen.

We assessed to what extent sodium intake can be estimated from the sodium content of a spot-urine specimen (spot-UNa) collected within 4 hours after the first voiding upon awakening but before breakfast. Subjects were asked to collect spot-urine and 24-hour urine specimens other than the spot-urine for a 3-day period, either successively or intermittently. The coefficient of correlation between spot-UNa and urinary excretion of sodium in 24 hours (24-h UNa) collected on the first day was 0.47. When the coefficient of the variation of creatinine excretion in the spot-urine was above 20%, the sample was discarded as the "outlier', and in this case the coefficient of correlation was 0.725. A marked fluctuation of creatinine excretion in spot-urine was considered to represent technical errors at the time of the spot-urine collection. The coefficient of variation of intra-individual 24-h UNa exceeded 20%, suggesting that a single determination of 24-h UNa does not represent the individual average of daily urinary excretion of sodium. It is concluded that the determination of a substantial number of spot-urine specimens to estimate daily salt ingestion of a given subject may be more reliable than a single determination of 24-h UNa, if the "outlier' of creatinine excretion in spot-urine specimen is excluded from the determination of spot-UNa. The average urinary excretion of sodium in 24 hours by the Japanese subjects investigated here was 218 +/- 67 mEq.

The incidence and pathogenesis of hyperlipidaemia in 16 consecutive acromegalic patients.

Serum lipids were analyzed in 16 patients with active acromegaly. Of these 62.5% had hyperlipidaemia defined as exceeding and 90% fiducial limits of normal controls. The mean serum cholesterol (5.50 mmol/l) and triglyceride (4.09 mmol/l) levels of the patients were significantly higher than those of age-matched normal controls. Type V hyperlipoproteinaemia was observed in two cases and type III hyperlipoproteinaemia in one. There was no difference in the incidence of diabetes between the normolipidaemic (n = 6) and hyperlipidaemic (n = 10) groups. Serum levels of growth hormone in hypercholestelaemic patients (n = 3) were significantly higher than those of normolipidaemic patients and combined hyperlipidaemic patients (n = 5 tended to have higher levels of growth hormone than normolipidaemic patients. In cases developing type III or type V hyperlipoproteinaemia, the activity of hepatic triglyceride lipase of lipoprotien lipase was decreased, but in increased when serum GH levels fell after therapy for acromegaly. It is suggested that 1) growth hormone may play some role on the pathogenesis of hyperlipidaemia associated with acromegaly, and 2) growth hormone has an inhibitory effect on H-TGL and LPL, and so hyperlipoproteinaemia in some cases of acromegaly might be caused by low H-TGL or LPL activity resulting from high growth hormone levels.

Phage inactivation and DNA strand scission activities of 7-N-(p-hydroxyphenyl)mitomycin C.

A new derivative of mitomycin C (MMC), 7-N-(p-hydroxyphenyl)mitomycin C (M-83), had higher phage inactivation activity against phages phiX174 and PM2 than MMC, and also higher DNA strand scission activity against their single- and double-stranded DNAs. M-83, at one third to one sixth concentration of MMC, showed the same level of phage inactivation and DNA strand scission activities. The mechanism of phage inactivation and DNA strand scission by M-83 were similar to those of MMC: (1) Reduction of M-83 was required for its action. (2) Oxygen radicals were involved in DNA strand scission, and metal ions possibly participated in the generation of oxygen radicals. (3) DNA strand scission was single strand scission, and dependent on temperature. The high DNA strand scission activity of M-83 is considered to reflect the rapid conversion to the active form.

[The development of demographic surveys and censuses in Indonesia].

"The development of statistical activities in Indonesia since the 1960s is outlined with special reference to demographic surveys and population censuses." The author notes the substantial improvements that have been made in the design, organization, and execution of censuses and surveys and expresses the hope that similar improvements can be made in the vital statistics system. [Experimental and clinical studies on ceftizoxime in the field of oral surgery (author's transl)].

Experimental and clinical studies on ceftizoxime (CZX), a new cephalosporin derivative, were carried out and the following results were obtained. 1. The minimal inhibitory concentrations of CZX, cefazolin (CEZ), cephalothin (CET), cefotiam (CTM) and cefmetazole (CMZ) against Gram-positive cocci (31 strains) and Gram-negative rods (169 strains) isolated from the patients with oral infections were determined. CZX, though somewhat less active against Gram-positive cocci than the other cephalosporins, was the most active of the antibiotics tested against Gram-negative rods. 2. The mean serum levels in 19 patients who received 1 g of CZX by intravenous drip infusion for 1 hour were as follows. 19.6 micrograms/ml 30 minutes after the start of intravenous drip infusion, 52.2 micrograms/ml after 1 hour, 25.0 micrograms/ml after 1.5 hours, 20.3 micrograms/ml after 2 hours, 7.9 micrograms/ml after 4 hours, 3.5 micrograms/ml after 6 hours. The mean peak tissue levels of CZX after 1 g dose by intravenous drip infusion for 1 hour were 14.3 micrograms/g (n = 5) in gingiva and 8.5 micrograms/g (n = 2) in bone marrow at the end of intravenous drip infusion. 3. CZX was administered to 17 patients with various infections in the oral surgical field at daily dose of 1 approximately 2 g for 3 approximately 6 days. The therapeutic effect was as follows: 'excellent' in 6 cases, 'good' in 9, 'fair' in 1 and 'poor' in 1. The final rate of effectiveness was 88.2%. No adverse reaction was observed except for 2 cases of slight elevation of S-GPT.

[Fundamental and clinical studies of cefotiam in the field of oral surgery].

Cefotiam (CTM) is a new synthetic cephem antibiotic developed in Japan. The results of the fundamental and clinical studies are as follows. 1. CTM showed antibacterial activity, in terms of MIC, as strong as those of cephalothin (CET) and cefazolin (CEZ) for Gram-positive cocci, and several times superior to for Gram-negative bacilli. 2. CTM serum levels approximately reached the peaks on completion of 60 minutes intravenous drip infusion of 1 g of this preparation dissolved in 5% glucose solution of 250 ml; the mean value was 65.00 micrograms/ml. Then the levels dropped rather quickly up to 180 minutes after the start of drip infusion. After that, the levels dropped gradually up to 360 minutes. 3. As for the passage of CTM in the oral tissues, satisfactory passage was observed in both maxillomandibular marrow and gingiva, which adequately exceeded MICs of the clinically isolated strains of oral infections. 4. This preparation was administered 1 g of 2 g daily by intravenous drip infusion in 18 cases of moderate or more serious infections in the field of oral surgery; the clinical efficacy rate obtained was 94.4%. 5. No manifestations of side effect were observed clinically. As for laboratory findings, 1 case of large increases in GOT and GTP (a hepatitis B antigen positive patient) and 2 cases of slight increase in GTP were observed. On the basis of these results of the fundamental and clinical studies, it was concluded that CTM is an excellent antibiotic for the treatment of oral infections.

Spleen cell transfer induces T cell-dependent granulomas in tuberculous nude mice.

After IV infection with an attenuated strain of Mycobacterium bovis BCG, athymic nude mice produced a few hepatic granulomas; most of the other foci of bacterial parasitization lacked obvious cellular reactions. Nude mice BCG-infected four weeks earlier received spleen cells from normal euthymic littermates. After transient splenomegaly which peaked at about two weeks, numerous hepatic granulomas appeared by four weeks and the number of viable BCG decreased. The development of hepatic granulomas required viable spleen cells and depended on the spleen cell dose and corresponded with positive footpad reactions. The effective population seemed to be T cells, since they were sensitive to treatment with anti-brain associated theta antigen serum (antiBAT) plus complement, and were found in nylon-wool passed fractions. After transfer with sensitized spleen cells, adoptive immunity was observed, provided that the recipient BCG-infected nude mice had been splenectomized before transfer. AntiBAT plus complement treatment of the sensitized spleen cells decreased the granuloma-producing as well as antimycobacterial capacity. When BCG-infected nude mice were irradiated with 500 r of gamma ray, granuloma production was decreased after immune spleen cell transfer. Co-transfer of immune spleen cells with normal nude mouse bone marrow cells to BCG-infected irradiated nude recipients restored the granuloma-forming ability.

[The double contrast hysterography using CO2 insufflation (author's transl)].

The double contrast hysterography, which was performed by the continuous CO2 insufflation with the ordinary HSG, was applied to 162 infertile patients. Forty-nine of 162 patients showed the abnormal findings by this method. Hysteroscopic or histological examinations revealed that 26 of these patients had hyperplasia, 15 had polyp, 2 had myoma and 1 had intrauterine foreign body. When these results were compared with those obtained by ordinary HSG, it was found that the double contrast hysterography would be more useful to investigate the detailed intrauterine changes such as hyperplasia or polyp than the ordinary HSG, and, therefore, would be a useful aid in the clinical care.

[Experimental and clinical studies of cefmetazole in the field of oral surgery].

Experimental and clinical studies were carried out on cefmetazole (CMZ), a synthetic antibiotic of cephamycin group, and the following results were obtained. 1) The minimal inhibitory concentrations (MICs) of CMZ against Gram positive cocci (30 strains) isolated from the patients with oral infections showed slightly inferior to those of ABPC, CEZ, while MICs against Gram negative bacilli (166 strains) of CMZ exhibited stronger activity than ABPC, CEZ. 2) The mean serum concentration of CMZ at 60 minutes after drip infusion showed 65.6 micrograms/ml. The concentration of CMZ in oral tissues varied in 4--60% of serum level at 60 minutes after drip infusion. The concentration in pus showed 15.2 micrograms/g at 60 minutes after drip infusion. 3) CMZ was administered by the drip infusion to 22 patients with various infections in the oral surgical field. The efficacy rate was 90.9%. 4) No side effect was observed.

[A case of malignant melanoma responded to chemotherapy including DTIC and local injection of OK-432].

A 33-year-old woman with malignant melanoma well responded to a chemotherapy including DTIC, ACNU, and VCR, and intralegional injection of OK-432. Four years prior to admission, a lentigo of 5 mm diameter at her left frontal chest was found, but, histological examination resulted in no distinctly malignant findings. In November 1979, multiple subcutaneous tumors appeared over posterior surface of the chest and left axillar region, which were gradually increasing in number and size, then she was admitted to our hospital in June, 1980. Biopsy of subcutaneous tumors revealed a malignant melanoma and its metastasis to skin. Immunochemotherapy was started immediately based upon this diagnoses. The patient received 100mg DTIC i.v. for 4 days, 100mg ACNU i.v. for one day and 1 mg VCR i.v. for one day. OK-432 was locally injected into some tumors as an immunotherapy. On completion of the third course of chemotherapy, all subcutaneous tumors were decreased in size, especially the tumors injected with OK-432. However, patient didn't respond to a repeated chemotherapy, thereafter malignant melanoma was metasized to uterus and peritoneum in May, 1981, and she died in September, 1981. Major side effects of this chemotherapy were nausea and mild transient leukocytopenia. The combination of chemotherapy including DTIC and local injection of OK-432 appeared to be effective for malignant melanoma.
