Nuclear actin and transport of RNA.

The role of nuclear actin filaments in the RNA transport was investigated. Mouse lymphoma cells, L5178Y, were labeled for 20 min with 3H-uridine, and the isolated nuclei were incubated in a medium consisting of 0.25 M sucrose, 10 mM Tris-HCl (pH 7.5), 2 mM CaCl2, 1 mM ATP and 1mM PMSF. Release of the rapidly labeled RNA from the nuclei was temperature-dependent and was stimulated by ATP. Phalloidin, an inhibitor of actin filament depolymerization, had no effect on the system at 10 or 100 micrograms/ml. Therefore, actin filament depolymerization may not be involved in the transport of RNA.

Expression of a multidrug-resistance gene in human tumors and tissues.

The identification and cloning of a segment of a human multidrug resistance gene (mdr1) was reported recently. To examine the molecular basis of one type of multidrug resistance, we have prepared RNA from human tumors and normal tissues and measured their content of mdr1 RNA. We find that the mdr1 gene is expressed at a very high level in the adrenal gland; at a high level in the kidney; at intermediate levels in the lung, liver, lower jejunum, colon, and rectum; and at low levels in many other tissues. The mdr1 gene is also expressed in several human tumors, including many but not all tumors derived from the adrenal gland and the colon. In addition, increased expression was detected in a few tumors at the time of relapse following initial chemotherapy. Although controlled clinical studies will be required, our results suggest that measurement of mdr1 RNA may prove to be a valuable tool in the design of chemotherapy protocols.

Expression of a full-length cDNA for the human "MDR1" gene confers resistance to colchicine, doxorubicin, and vinblastine.

Intrinsic and acquired multidrug resistance (MDR) is an important problem in cancer therapy. MDR in human KB carcinoma cells selected for resistance to colchicine, vinblastine, or doxorubicin (former generic name adriamycin) is associated with overexpression of the "MDR1" gene, which encodes P-glycoprotein. We previously have isolated an overlapping set of cDNA clones for the human MDR1 gene from multidrug-resistant KB cells. Here we report the construction of a full-length cDNA for the human MDR1 gene and show that this reconstructed cDNA, when inserted into a retroviral expression vector containing the long terminal repeats of Moloney leukemia virus or Harvey sarcoma virus, functions in mouse NIH 3T3 and human KB cells to confer the complete multidrug-resistance phenotype. These results suggest that the human MDR1 gene may be used as a positive selectable marker to introduce genes into human cells and to transform human cells to multidrug resistance without introducing nonhuman antigens.

Thyroid disorders in the general population of Hisayama Japan, with special reference to prevalence and sex differences.

Serum triiodothyronine (T3), thyroxine (T4), thyroid stimulating hormone (TSH) and thyroid autoantibodies (TAA) to either microsome or thyroglobulin were measured in 1251 samples from the general population, 546 males and 705 females, aged 40 or over in Hisayama, a Japanese rural town. TAA was positive in 7.7% of men and 15.0% of women, and the male/female ratio was 1:2. This ratio was markedly different from those in hospital patients with thyroid disorders where ratios such as 1:8 for chronic thyroiditis and 1:4 for Graves' disease were found. Among the study subjects, definite hyperthyroidism was found in 0.2%, borderline hyperthyroidism in 0.7%, overt hypothyroidism in 0.3%, latent hypothyroidism in 4.2%, and non-thyroid illness in 2.4%. Overt thyroid dysfunction was more evident in women, while latent thyroid dysfunction or non-thyroidal illness was equally prevalent in men and women. These results indicate that the incidence of immunologically or functionally latent thyroid disorders is rather higher than expected, in both women and men. Some unknown factor(s) might act as a trigger to manifest thyroid dysfunction, mainly in females.

Treatment of recurrent subareolar abscess.

Recurrent subareolar abscess is an infrequent but troublesome disease of the breast among young women. Recent experience with 5 patients indicated that successful treatment of recurrent subareolar abscess requires excision of the diseased ducts and necrotic cavity, including the inverted section of the nipple. The operative procedure described herein allows us to radically manage recurrent subareolar abscess and at the same time leave a nipple with a good cosmetic appearance.

Chediak-Higashi syndrome with intestinal complication. Report of a case.

A 21-year-old woman with Chediak-Higashi syndrome developed intestinal lesions similar to those of Crohn's disease. Radiographic findings showed a stenotic lesion with multiple nodular changes in the ascending colon and cecum, and a longitudinal ulceration in the terminal ileum. In addition, multiple oval-shaped ulcerations were seen between the terminal ileum and descending colon. However, serial histological examinations revealed only an infiltration of lymphoid cells, which was a nonspecific finding. Though there are few reports of gastrointestinal involvement in Chediak-Higashi syndrome, such an intestinal complication may develop in patients long surviving with this disease.

Severe disability related to cerebral stroke: incidence and risk factors observed in a Japanese community, Hisayama.

To elucidate the incidence of severe disability due to cerebral stroke and its related factors, prospective data of 1,621 Hisayama residents aged 40 and over were examined. Severe disability resulting from stroke was defined as patients who were unable to dress, take care of their toilet needs, and feed themselves without assistance, or who required a wheel chair for ambulation three months after the most recent episode. During 20 years of follow-up 255 stroke patients were observed among the sample population. The annual incidence of stroke per thousand was 9.8, and rate of severe disability was 2.8 for men and 6.4 and 2.0 for women, respectively. Of the 74 cases with severe disability, approximately 92% were attributed to cerebral infarction. Related factors to severe disability due to cerebral infarction were recurrent attacks, hypertension, changes in ocular fundi and diabetes mellitus among predispositions and quadriplegia or muscular contraction, and intelligent or mental disorders among inhibiting factors for functional recovery. Furthermore, in 59 autopsy cases with multiple cerebral infarctions, the frequency of disability increased as the number of infarcts increased. Hypertension and diabetes mellitus, as risk factors for cerebral infarction and factors inhibiting post-ictal functional recovery were discussed.

Familial erythrophagocytic lymphohistiocytosis: surface marker analysis using monoclonal antibodies.

Surface marker analysis of atypical cells, present at elevated levels in the peripheral blood of a 2-yr-old patient with familial erythrophagocytic lymphohistiocytosis (FEL), was performed with monoclonal antibodies. The atypical cells expressed T11, T8, Ia and LeuM2 antigens, markers of suppressor/cytotoxic T lymphocyte and monocyte. The monoclonal antibody-defined surface markers were thus distinguishable from those of other lymphoproliferative disorders clinically resembling FEL.

Perceptual components of pitch: spatial representation using a multidimensional scaling technique.

Paired comparison experiments were carried out on the pitch of 18 computer-generated complex tones synthesized by slight modification of the frequency structure of Shepard's endless scale sounds. The results were analyzed by a multidimensional scaling technique, and simple helixes were obtained in which two components of pitch (tone height and tone chroma) were represented. Some individual differences in perception of pitch were observed according to the weight that the subject gives to each component.

To what extent does the size of a ventricular septal defect correlate with haemodynamic data derived from cardiac catheterisation?

A simplified model based on Gorlin's formula was used to derive an index of the cross sectional area of ventricular septal defects from commonly used cardiac catheterisation data. This index was compared with the area of the defect measured during operation and expressed as a ratio to the cross sectional area of the aorta. The highly significant linear relation (r = 0.94) between this index of the defect area and the size of the defect measured at operation means that the severity of a ventricular septal defect can be assessed from haemodynamic data obtained at cardiac catheterisation.

Use of bromocriptine in the treatment of normoprolactinemic infertility.

The efficacy of bromocriptine therapy was studied in 84 normoprolactinemic infertile patients. Bromocriptine (2.5-5.0 mg/day) was given for at least 1 month. Bromocriptine therapy was effective in 54 of 84 cases (64%), including 6 of 15 (40%) cases with amenorrhea, 16 of 24 cases (67%) with anovulatory cycle, 9 of 15 cases (60%) with delayed ovulation and 23 of 30 cases (77%) with luteal phase defect. Sixteen cases were able to conceive with bromocriptine alone. Twenty-five patients who did not respond to bromocriptine were treated with a combination therapy consisting of bromocriptine and clomiphene. Of the 25 cases, 14 responded to the therapy, and 6 of them were able to conceive. The response of prolactin to domperidone (10 mg, i.v.) was significantly (p less than 0.01) higher in the group responding to bromocriptine than in the nonresponding group. These results indicate that the administration of bromocriptine is an effective therapy for patients with normoprolactinemic endocrine disorders, and that domperidone may be useful in selecting the candidates for bromocriptine therapy.

Characterization of acidic actin in mouse sarcoma 180 cells.

Mouse sarcoma 180 cells have a polypeptide that has the same molecular weight as actin but it is more acidic than alpha-actin. Its tryptic peptide pattern on reversed-phase HPLC was very similar to that of beta + gamma-actin, an actin sample prepared by affinity chromatography on DNase I-Sepharose contained the acidic polypeptide, and monoclonal anti-actin antibody reacted with it; therefore, the polypeptide is considered an actin isoform. The mRNA for this variant actin was identified by analyzing the polypeptides translated in vitro, which indicated that the variant actin is not a post-translationally modified form of any known actin. The variant actin was not stained by polyclonal anti-gizzard actin antibody which reacts with gamma-cytoplasmic, alpha-smooth and gamma-smooth muscle actins, nor by polyclonal anti-skeletal muscle actin antibody which reacts with skeletal, cardiac and alpha-smooth muscle actins. These results suggest that this variant actin is related to beta-cytoplasmic actin or, is a novel species whose N-terminal amino acid sequence is not Glu-Glu-Glu.

A clinicopathologic correlation study of thallium-201 myocardial scintigraphy in diagnosis of myocardial infarction.

In a series of 1,000 consecutive autopsy cases, we evaluated the clinical utility of thallium-201 (Tl-201) myocardial scintigraphy and electrocardiography (ECG) in 101 patients who had been studied while alive. Fifty-five cases had myocardial infarctions (MI) at autopsy. The Tl-201 scintigram and ECG in diagnosis of MI showed sensitivities of 68% and 60%, specificities of 87% and 83%, and diagnostic accuracies of 76% and 70%, respectively. The sensitivity of the Tl-201 scintigram was 70% in anterior MI, 80% in postero-inferior MI, 25% in lateral and subendocardial infarction. The sensitivity was 88% for large massive MI, but was low in scattered (50%) or middle-sized MI (17%). The diagnostic limit of the resolution of Tl-201 scintigrams was 4.5 cm in long diameter. All 8 cases with MI of less than 4 cm could not be diagnosed with the technique. There were 48 cases of large MI (more than 5 cm), but 8 cases could not be diagnosed by scintigraphy because of non-transmural or scattered MI. A comparison of the Tl-201 scintigram and ECG showed that 27 cases out of 60 cases were diagnosed by both methods, 14 only by the Tl-201 scintigram, 9 only by ECG and 10 by neither method.

A histopathologic study of the conduction system in an elderly patient with partial atrial standstill.

Electrophysiologic and histopathologic studies were performed on an 83-year-old man with partial atrial standstill. In the electrophysiologic study, no atrial electrical activity was recorded in the right atrium, but f waves were seen in the coronary sinus electrogram. Although the high right atrium and the atrial septum could not be activated by electrical stimuli of up to 10 V, the low atrium could be stimulated. The pathologic examination showed that there was extensive fatty infiltration into the sinus node and fibrosis in the atrial muscle. In the low right atrium, the musculature was partially preserved.
