Causes of death in the elderly and their changing pattern in Hisayama, a Japanese community. Results from a long-term and autopsy-based study.

The causes of death for the elderly were prospectively studied in Hisayama, Japan, a rural community. We compared 1,621 subjects, aged 40 years or over, recruited in 1961, and 2,053 subjects recruited in 1974. Each cohort was studied in a follow-up that lasted 10 years; they had autopsy rates of 82.1% and 86.1% during each 10-year period, respectively. The most common causes of death for those aged 70 years or over were cerebrovascular disease, malignant neoplasms, and pneumonia. Deaths due to cerebrovascular disease tended to decrease in the recent cohort, but the proportion of decline was more prominent in cases aged 40 to 69 years. There was a sex difference in the changing pattern of mortality from heart diseases including ischemic heart disease. Deaths by both heart diseases and ischemic heart disease increased in the more recent cohort of aged women, whereas they decreased in the aged men. Pneumonia was an important cause of death for the elderly in both cohorts. Deaths due to "senility" were rare, being only 1% of the deceased aged 70 or over. With prolonged lifespan, especially for women, the impact of atherosclerosis and its related disorders on the recent Japanese aged population appears to have increased.

[Echovirus type 18 infection: clinical features of 15 cases in Kitakyushu in 1988].

An epidemic disease with maculopapular exanthem, especially on the face and the extremities was observed form June to July of 1988 in Kitakyushu-shi. The clinical findings of 15 patients (male 12, female 3) with exanthem were described. Symptoms included fever (10 patients), diarrhea (5), cough and nasal discharge (2), headache and vomiting (1), and hyperreflexia (1). Echovirus type 18 was isolated from throat swabs and stool samples of 9 patients.

Sharpness and amplitude envelopes of broadband noise.

This paper discusses how amplitude envelope shapes, sound-pressure level, and duration of broadband noise affect sharpness and brightness. In the first experiment, sharpness, brightness, and similarity were judged for paired stimuli by 13 subjects. The stimuli consisted of broadband noise with different types of amplitude envelopes, sound-pressure levels, and duration. Experimental results were analyzed with a multidimensional scaling technique. In addition, the second experiment measuring the point of subjective equality (PSE) of sharpness was carried out for four subjects. The results of these two experiments showed large individual differences in sharpness judgment, difficulty in brightness judgment for the stimuli used in the experiments, and influence of the amplitude envelope shapes on sharpness. The individual differences observed in sharpness judgment were explained by the differences between weights given to two psychological dimensions: loudness and subjective duration. This study forms a basis for understanding sharpness of nonsteady sounds.

A case of congenital factor V deficiency combined with multiple congenital anomalies: successful management of palatoplasty.

A patient with congenital factor V deficiency combined with mental retardation and several congenital anomalies including cleft palate, dwarfism, microcephaly and right hydrocele testis is described. The levels of factor V activity and factor V antigen of plasma were significantly decreased. The platelet lysate obtained from him also showed a significantly low level of factor V activity. Palatoplasty and tooth extraction were successfully performed under transfusion therapy with fresh-frozen plasma.

Seasonal variation in stroke incidence in Hisayama, Japan.

We investigated seasonal variation in the incidence of cerebral stroke among the general population aged greater than or equal to 40 years in November of 1961 in Hisayama, Japan. During the 24-year follow-up period, 311 cases of cerebrovascular diseases occurred. The date or month of onset was determined in 308 cases, of which 51 were classified as intracerebral hemorrhage, 223 as cerebral infarction, and 27 as subarachnoid hemorrhage. We observed a significant seasonality in the incidence of all stroke (p less than 0.01), of intracerebral hemorrhage (p less than 0.05), and of cerebral infarction (p less than 0.01), whereas subarachnoid hemorrhage had no significant seasonal pattern. Subjects less than 64 years of age showed a significant seasonal variation in the incidence of both intracerebral hemorrhage (p less than 0.05) and cerebral infarction (p less than 0.01). A significant seasonal pattern for the incidence of intracerebral hemorrhage was also noted among persons with hypertension (p less than 0.05) or a high serum cholesterol level (p less than 0.05), whereas such a pattern for cerebral infarction was documented among normotensive persons (p less than 0.05) and those with a low serum cholesterol level (p less than 0.01). In addition, the incidences of intracerebral hemorrhage and cerebral infarction were negatively correlated with mean ambient temperature (p less than 0.01 and p less than 0.05, respectively), and all stroke and intracerebral hemorrhage in men were significantly related to intradiurnal temperature change (p less than 0.05 and p less than 0.01, respectively). The significance of the seasonal occurrence of stroke is discussed in relation to relevant risk factors.

Expression of the multidrug resistance, MDR1, gene in neuroblastomas.

Metastatic neuroblastoma is a childhood malignancy that is frequently responsive to chemotherapy with doxorubicin, vincristine, and teniposide (VM26), among other drugs, but in the majority of treated patients, the tumor recurs during or after chemotherapy. In this work, we have examined the hypothesis that the development of resistance to chemotherapy in neuroblastoma might be related to the expression of the human MDR1 gene, which encodes a multidrug transporter that functions as an energy-dependent drug efflux pump. RNA samples from 49 neuroblastomas were analyzed, including 31 from untreated and 18 from treated patients. MDR1 RNA was detectable in the majority of treated and untreated tumors using a sensitive, semiquantitative slot blot assay. Of the samples from treated patients, five of 18 were found to have high MDR1 RNA levels, whereas only three of 31 from untreated patients had high MDR1 levels, a statistically significant difference (P less than .01). These results show that high levels of MDR1 RNA are often associated with resistance to chemotherapy in neuroblastoma and suggest that they may contribute to this resistance. Many of the neuroblastoma samples were also evaluated for N-myc amplification but there was no correlation between N-myc copy number and the level of MDR1 mRNA expression.

Structure of pollen grains of Tradescantia reflexa with special reference to the generative cell and the ER around it.

A pollen grain in Tradescantia reflexa consists of two cells, the generative and the vegetative cells, the generative cell being surrounded completely by the vegetative cell. The generative cell has many lobes or surface invaginations. A complicated network of rER extends throughout the entire vegetative cytoplasm, forming a system of channels made up by the cisternae of rER. Lipid granules are surrounded by ER. Branches of the rER enter all the concavities of the invaginations and attach to the plasma membranes at the bottoms of the invaginations. In the generative cell, no reserve substances, such as lipids, are seen. There is little ER, mitochondria are few in number, and Golgi bodies seem to be less active within this type of cell. Bundles of microtubules run parallel to the long axis of the generative cell. No microtubules or microfilaments can be detected at or near the bottoms of concavities, either on the generative or the vegetative side. ER is the sole cell element that bears a positional relationship to the invaginations. It appears, therefore, that rER is intimately involved in the shaping of the invaginations. This is the first report that a cell element other than microtubules and microfilaments can be involved in the formation of the outer shape of a cell. The possibility that materials from decomposed lipid droplets are transported through the rER to the generative cell is also discussed.

[Properties of N-acetyl-beta-glucosaminidase in the masseter muscle of the mouse].

The present study was undertaken to clarify various characteristics of N-acetyl-beta-glucosaminidase (NAG) in the masseter muscle of the mouse. The pH optimum of NAG in tissue homogenate was 4.3, and Km value for p-nitrophenyl-N-acetyl-beta-glucosaminide was 0.98 mM. By means of isoelectric focusing. NAG in the masseter muscle was separated into four enzymes (NAG I-IV). The isoelectric points were: I, 4.9; II, 6.4; III, 7.2 and IV, 8.7. The pH optima of NAG I-IV ranged from 4.2 to 4.4, and Km values from 0.61 to 0.83 mM. The molecular weights of I, II, III and IV were 135,000, 120,000, 135,000 and 110,000, respectively. Studies on heat stability showed that NAG I and IV were slightly labile as compared with II and III. The activities of NAG I-IV were dose dependently inhibited by both N-acetyl-glucosamine and N-acetyl-galactosamine. Both Ag and Hg2+ ions caused a marked inhibition on activities of NAG I-IV.

Antinucleolar autoantibody induced in mice by mercuric chloride.

Mercuric chloride (MC) was administered to mice subcutaneously 3 times a week for 4 weeks. When inbred BALB/c, CBA/JN, C3H/He, C57BL/6 and DBA/2 and outbred ICR female mice were given 1 mg/kg MC, 8 of 10 ICR mice developed antinucleolar autoantibodies (AnuA) of the IgG class but none of the inbred mice produced AnuA. So the ICR mice were used in the following experiments. In a kinetic study, in which mice were given 1.5 mg/kg MC, the positivities and titers of AnuA increased time-dependently, and were kept at high levels until 8 weeks after the final treatment. The AnuA titers of mice receiving 0.5-2.0 mg/kg increased dose-dependently, while the positivity was similar at all dosages. The antibody titers were higher in mice pretreated with an intramuscular injection of either complete or incomplete Freund's adjuvant. The titer increase was more prominent in mice administered doses lower than 0.5 mg/kg. Neither nude mice nor mice treated with a monoclonal antibody to L3T4 (GK1.5) developed AnuA. These results indicate that the induction of AnuA by MC depend primarily on L3T4-positive T cells.

Tumor necrosis factor and fever at diagnosis in children with solid tumors.

In order to determine the pathogenesis of fever in solid tumors, we studied the association of fever at diagnosis in children with solid tumors (malignant lymphoma, rhabdomyosarcoma, and neuroblastoma), serum levels of interleukin 1 (IL-1), and tumor necrosis factor. Thirteen of 20 patients (65%) with solid tumors were complicated with fever at diagnosis. There was no difference in C-reactive protein or IL-1 levels between the patients with and without fever, while the erythrocyte sedimentation rate and TNF levels were higher in the former than in the latter by Wilcoxon's rank sum test (p less than 0.01). These findings suggest that most febrile episodes at diagnosis in children with solid tumors are associated with the release of tumor necrosis factor.

Serum granulocyte colony-stimulating factor levels in chronic neutropenia of infancy.

Eight infants with chronic neutropenia ranging in age from 1 to 13 months were studied for serum granulocyte colony-stimulating factor (G-CSF) levels. Serum G-CSF levels were elevated, especially when peripheral blood absolute neutrophil counts (ANC) were under 500/microliter in seven patients with autoimmune neutropenia. On the other hand, in a patient with congenital agranulocytosis (Kostmann type), G-CSF levels were below the sensitivity of the assay (less than 50 pg/ml) despite severe neutropenia (less than 100/microliter).

[A clinical study of hypertrophic obstructive cardiomyopathy in the elderly].

Seven elderly patients with hypertrophic obstructive cardiomyopathy (HOCM), who had the three following characteristics on echocardiograms 1) extremely thickened septum, 2) systolic anterior motion of the mitral valve, 3) mid systolic semi-closure of the aortic valve, were clinically evaluated. Ages ranged from 73 to 86 years old (average 78.9% yr.) and all were women. None had not a family history of hypertrophic cardiomyopathy but they had mild hypertension. Six patients showed a significant high voltage on the ST-segment and T-wave abnormalities ("strain" pattern). The left ventricular posterior wall as well as the septum was thickened in 5 and the remaining 2 showed asymmetrical septal hypertrophy (ASH) on echocardiograms. The left ventricular cavity was narrowed due to left ventricular hypertrophy and the shape of the left ventricular cavity was ovoid in all patients. The aorto-septal angles in these 7 patients were 80 degrees to 120 degrees. In addition, proximal septal bulge in all and anterior displacement of the mitral posterior leaflet due to the mitral ring calcification (MRC) in some patients contributed to the narrowing of the left ventricular outflow tract, and the mitral valve was pulled up toward the septum because of the good left ventricular systolic function (ejection fraction: 70 to 94% by echocardiography) and blood was ejected at a high velocity through a narrowed outflow tract (Venturi effect). Pressure gradients in the left ventricular outflow tract was 38 to 146 mmHg in 5 examined by cardiac catheterization. Biopsy specimens were obtained from 2 patients, showing hypertrophic myocytes (diameter: 20 to 30 micron) in 2 and mild disarray in 1.(ABSTRACT TRUNCATED AT 250 WORDS)

Increase of 63 kDa protein kinase in the nuclear matrix of HL-60 cells during differentiation by retinoic acid.

The changes of protein kinases (PKs) in the nuclear matrix of HL-60 cells during the differentiation by retinoic acid (RA) were examined by in situ assay after sodium dodecyl sulfate-polyacrylamide gel electrophoresis. In the nuclear matrix of HL-60, at least seven species of PKs (83, 63, 58, 46, 42, 38, and 31 kDa) were always detected. Among these PKs, 63 kDa PK was increased dramatically during differentiation by RA. Two-dimensional electrophoresis revealed that two species of 63 kDa PKs (pI 5.0 and 5.4) were presented in the nuclear matrix and increased similarly during the differentiation. This increase of 63 kDa PKs may be related to the lobulation of the nucleus and the nuclear matrix accompanying the differentiation by RA.

[Carcinoma of the renal pelvis and ureter. An investigation of 384 registered cases in the Tokai Urological Cancer Registry].

Registration of renal pelvic and ureteral tumor was done from 1980 through 1986, in the Tokai Urological Tumor Registry. Among the 404 cases of the carcinoma, 384 cases (210 renal pelvis, 174 ureter) were subjected to the present study. A total of 319 cases (83.1%) showed a complete cure, while 20 cases (5.2%) were considered as partial cure, and 45 cases (11.7%) were classified to progressive disease group. Chemotherapy was performed on 151 patients including systemic administration to 141 patients (33.9%) and 11 patients received combined intravesical instillation therapy. Irradiation was performed on 49 patients (12.8%). Degree of histological and morphological differentiation of renal pelvis and ureter tumor were studied in 341 cases of transitional cell carcinoma (88.8%); G0, 0.3%, G1, 12.9%, G2, 49.3%, Gx, 5.2%. Stage of the tumor were T1, 33.6%; T2, 12.5%; T3, 14.6%; T4, 13.3%; Tis, 0.3%; Tx, 24.7%. Five-year relative survival for carcinoma of the renal pelvis was 46.6%, and for the ureter it was 53.1%. Five-year survival according to histological differentiation of transitional cell carcinoma and according to the stage of the tumor were as follows; G1, 91.6%; G2, 58.5%; G3, 27.2%; Ta, 84.6%; T1. 74.2%; T2, 48.5%; T3, 24.1%, T4, 7.3%. The rate of survival was correlated with histological differentiation of the tumor cells. The survival was poor in cases who had the components of squamous cell carcinoma and/or adenocarcinoma. Overall survived case were 177, 122 patients died of cancer, 20 patients died of other causes than cancer, and 1 patient died from immediate surgical effect.(ABSTRACT TRUNCATED AT 250 WORDS)

[A combined treatment with 8 MHz radiofrequency hyperthermia, HPC-adriamycin, immunotherapy, radiation, systemic chemotherapy and irradiation for invasive bladder carcinoma].

Hyperthermia was induced for the treatment of invasive bladder carcinoma in order to study its usefulness. The subjects were 12 cases of invasive bladder cancer; including 5 cases of T2, 3 cases of T3, 2 cases of T4, and 2 cases of recurrence after total cystectomy. As previous treatment, 4 patients received radiotherapy and the other received TUR, systemic chemotherapy, and intravesical injection of anticancer drugs. For hyperthermia treatment, a Thermotron RF-8 was used for heating a deep seated tumor. Each case received hyperthermia 2 to 10 times. Combined therapy included injection of HPC-adriamycin into the urinary bladder in 5 cases, immunotherapy in 3 cases, M-VAC therapy in one case, radiotherapy in one case, radiotherapy and intra-arterial injection in one case, and Peplomycin and OK-432 local injection in one case. The treatment results showed a 75% effectiveness; with CR in 4 cases, PR in 5 cases, MR in 2 cases and PD in one case. Three patients died and 9 survived. Of four patients who had received radiotherapy as a previous treatment 3 cases obtained CR and one case MR. Therefore, it was considered that a favorable treatment effect with hyperthermia could be obtained after radiotherapy.

[Mass screening for prostatic cancer in Hisayama town, Fukuoka].

A mass screening for prostatic cancer was conducted in Hisayama town, Fukuoka Prefecture, Japan from 1988 to 1989. As the primary screening questionnaire survey about urination and measurement of serum prostate specific antigen (PSA) were performed in 789 of the 1025 male residents over 50 years old (77.0%). For the second screening with digital rectal examination and transrectal ultrasonography, 184 subjects (23.3%) were picked up. Prostatic cancer was suspected in 33 (4.2%) of the 134 subjects who actually underwent the second screening examination. Prostatic biopsy and other examinations such as measurement of prostatic acid phosphatase and gamma-seminoprotein, urinalysis and radiographic examination were performed in 26 as the third screening, and prostatic cancer was detected in 5 (0.63%); 3 in stage B and 2 in stage C. Four of the 5 subjects with prostatic cancer had complained no urinary symptoms on questionnaire. Serum PSA was useful for detecting early stage prostatic cancer.
