[The treatment of superficial bladder carcinoma with intravesical HPC-adriamycin and verapamil].

A new form of hydroxypropylcellulose-adriamycin (HPC-ADM), which was supplemented with verapamil, a known calcium antagonist, in order to potentiate the antineoplastic effect of HPC-ADM, was synthesized and used for the treatment of superficial bladder carcinoma for clinical evaluation. The results are reported in this paper. A total of 23 patients who were treated for bladder carcinoma during a 2-year period (1987-1988) were subjected in this study. All patients were diagnosed cases of superficial bladder carcinoma as follows; Ta, 3 cases, T1, 18 cases and Tis, 2 cases. For the injection protocol, firstly HPC-ADM solution supplemented with verapamil was introduced into the urinary bladder via the urethra. After 2-3 weeks, the efficacy of the drug against the tumor was evaluated by cystoscope. Patients in whom the drug were found to be effective received increased administration of drug. In cases in whom the drug was found to be ineffective, different drugs for further treatment were adopted. The average frequency and amount of drug was 2.3 times and 45.2 mg, respectively. Complete responses achieved in 9 of 23 cases (39.1%). Of the 23 cases, 4 cases was with PR and 10 with NC. No cases were found to be with PD. Side effects were observed in 4 of the 23 cases (17.4%), however, severe side effects were not recognized. The important aspect in applying the method of intravesical therapy is that it makes enhanced antineoplastic potency possible with concomitant reduction of side effects and prevents the invasion of tumor.

The effects of metal ions on the DNA damage induced by hydrogen peroxide.

The effects of metal ions on DNA damage induced by hydrogen peroxide were investigated using two methods, agarose-gel electrophoretic analysis of supercoiled DNA and sequencing-gel analysis of single end-labeled DNA fragments of defined sequences. Hydrogen peroxide induced DNA damage when iron or copper ion was present. At least two classes of DNA damage were induced, one being direct DNA-strand cleavage, and the other being base modification labile to hot piperidine. The investigation of the damaged sites and the inhibitory effects of radical scavengers revealed that hydroxyl radical was the species which attacked DNA in the reaction of H2O2/Fe(II). On the other hand, two types of DNA damage were induced by H2O2/Cu(II). Type I damage was predominant and inhibited by potassium iodide, but type II was not. The sites of the base-modification induced by type I damage were similar to those by lipid peroxidation products and by ascorbate in the presence of Cu(II), suggesting the involvement of radical species other than free hydroxyl radical in the damaging reactions.

[The expression of ras p21 product in hepatocellular carcinoma].

To elucidate the cell biological significance of ras oncogene, the expression of ras-p21 was analyzed in 53 cases of liver tissues including 34 cases of hepatocellular carcinoma (HCC), by using immunohistochemical method. In result, 22 (65%) cases of 34 HCC and 34 (79%) cases of 43 liver cirrhosis were positive for p21, whereas all of chronic hepatitis and normal livers were negative. Especially, comparative study between the expression of p21 and clinicopathological background of HCC revealed that p21 was prominently expressed in well differentiated form, nodular type, small liver cancer, and the cases showing AFP levels below 400 ng/ml. From these results, it was indicated that ras oncogene might play an important role in malignant transformation of hepatocytes or differentiation of HCC.

[Clinical evaluation of recombinant human G-CSF in children with cancer].

Recombinant human granulocyte colony-stimulating factor (rG-CSF), produced by Chinese hamster ovary cells, was administered in 69 chemotherapy-induced neutropenic pediatric patients (pts) with malignant tumors. Each pt received two cycles of the same chemotherapy and had neutropenia with absolute neutrophil counts (ANC) less than 500/microliter in the first cycle. Initiating 72 hours after termination of chemotherapy in the second cycle, rG-CSF (2 micrograms/kg/day) was given subcutaneously or intravenously to each pt for 10 days. rG-CSF significantly increased ANC at nadir; 72 +/- 14 vs. 206 +/- 40/microliter (data in the first cycle vs. data in the second cycle, respectively), and reduced the period of neutropenia with ANC less than 500/microliter; 9.7 +/- 0.6 vs. 5.1 +/- 0.6 days, and the period for restoration to ANC greater than or equal to 1,000/microliter after initiation of chemotherapy; 25.5 +/- 0.6 vs 17.5 +/- 0.9 days. rG-CSF did not affect other components of peripheral blood. The number of days with fever greater than or equal to 38 degrees C was significantly reduced by rG-CSF treatment. Neck pain and lumbago were observed in one pt, pollakisuria in one pt, and elevation of the serum levels of LDH and uric acid in one pt, however these were mild to moderate, transient, and resolved without any specific treatment. We concluded that rG-CSF was effective in neutropenia induced by intensive chemotherapy for malignant tumors without any serious side effects.

[Coronary arterial rupture during percutaneous transluminal coronary angioplasty: a case report].

A case who developed rupture in a diagonal branch of the left anterior descending coronary artery (LAD) during percutaneous transluminal coronary angioplasty (PTCA) is reported here. The present case was 80-year-old man with severe focal stenosis of the LAD at its junction with a diagonal branch. PTCA for the LAD lesion was successfully performed, but occlusion of the diagonal branch developed later. A subsequent ECG showed elevation of an ST segment in a VL, and PTCA for the diagonal branch was attempted. A 018 Hi-torque floppy guide wire was introduced into the occluded diagonal branch, and its dilatation was attempted using a 2 mm Simpson-Robert catheter. During a maximal pressure of 120 psi, a deformity was found at the distal end of the balloon. Post-PTCA angiograms showed rupture of the diagonal arterial branch, and mild to moderate pericardial effusion was observed by echocardiography. The patient experienced transient hypotension (60 mmHg at systolic), but his condition gradually stabilized after the administration of only a pressor medication. Neither pericardiocentesis nor emergency surgery was performed. The next day, follow-up angiograms showed diagonal branch occlusion at the proximal portion of the rupture site. His clinical course was satisfactory with spontaneous resolution of pericardial effusion and mild elevation of his cardiac enzymes (CPK = 243IU). In this case, it was concluded that the cause of coronary arterial rupture was the difference in diameters of the coronary artery (1 mm) and the balloon catheter (2 mm). This was the first rupture case experienced among 750 PTCA sites (0.13%).(ABSTRACT TRUNCATED AT 250 WORDS)

[Susceptibility of bacteria isolated from patients with lower respiratory tract infections to antibiotics (1987)].

Since 1981, in cooperation with research institutions across the nation, Ikemoto, et al. have been collecting clinical isolates from patients with respiratory tract infections and conducting an annual retrospective survey of patients' background factors and of isolated strains and their sensitivities to various antibacterial agents and antibiotics. In the period from October, 1987 to September, 1988, 17 institutions participated in the survey and a total of 706 strains which were demonstrated to be causative organisms were isolated from 562 patients with respiratory tract infections. Strains were mostly isolated from the sputum. The taxonomic breakdown of these strains was: Staphylococcus aureus (69 strains), Streptococcus pneumoniae (120), Haemophilus influenzae (170), Mucoid-producing Pseudomonas aeruginosa (42), Non-mucoid-producing P. aeruginosa (87), Escherichia coli (11), Klebsiella pneumoniae (35), Brahamella catarrharis (72), etc. Of these strains, 629 were used to determine MICs of various antibacterial agents and antibiotics for susceptibility analyses. Relationships between patient backgrounds and diagnoses and between infections diseases and causative organisms were also investigated. Most of the major causative organisms, such as H. influenzae and P. aeruginosa, showed no substantial changes from previous years, with regard to their sensitivities to antibiotic agent, but S. aureus, particularly methicillin/cephem-resistant strains of S. aureus (MCRSA) showed somewhat lower sensitivity to beta-lactams, and as in recent years, to ofloxacin, a new quinolone drug, as well. Regarding background factors of patients, the age distribution was heavily concentrated in age brackets of 50 years and older, thus patients in these age group accounted for 75.2% of all the patients, which was comparable to 73.5% in 1985 and 77.9% in 1986. Among infections encountered, bacterial pneumonia was most frequent at 28.3%, followed by chronic bronchitis (27.2%) and bronchiectasis (16.0%). Bacterial pneumonia was actually the most frequent, throughout the entire age groups accounting for 34.3% of patients up to 29 years, 26.6% in the group of 30-69 years and 30.7% in patients aged 70 years and older. Chronic bronchitis was next most frequent and accounted for 20.0%, 26.4% and 30.7% among the three age groups, respectively. Breaking down clinical isolates by diagnosis, H. influenzae, S. pneumoniae and P. aeruginosa were isolated frequently from most of the infectious diseases.(ABSTRACT TRUNCATED AT 400 WORDS)

[Expression of amyloid beta-protein gene in Alzheimer's disease].

Deposition of amyloid beta-protein in senile plaque cores and cerebral vessels is a major neuropathological finding of Alzheimer's disease (AD). Three species of cDNA clones encoding amyloid beta-protein precursors (APP 695, APP 751 and APP 770) were isolated and sequenced. We examined quantitatively the expression of these APP mRNAs in autopsied brains (frontal cortex) of AD patients and control subjects, using Northern blot analysis and the ribonuclease protection assay. Northern blot analysis revealed the production of three types of APP mRNA in the human brain and that AD/control ratios were 2.04 for APP 770 mRNA, 1.11 for APP 751 mRNA and 1.12 for APP 695 mRNA. In the protection assay the ratio of APP 770/APP 751/APP 695 mRNA was approximately 1:10:20 in the brain of control and the AD/control ratios were 2.38, 1.30 and 0.81 for APP 770, APP 751 and APP 695 mRNAs, respectively. These results suggested that an increase in APP 770 and APP 751, harboring a protease inhibitor domain, may disturb the balance between biosynthesis and degradation of APPs, which might lead to amyloid deposition.

[Correlation between changes in obesity from adolescence to young adulthood and family obesity--the results of cross sectional and longitudinal studies].

We conducted a survey of 356 married couples and their 552 children living in Hisayama in Fukuoka prefecture in order to investigate the correlation between changes in obesity from adolescence to young adulthood. 1. A positive correlation between couples in weight and height could be found, but its coefficient was weak (r = 0.12, 0.10). There was no correlation between couples in BMI. 2. The correlation between parents and their children in height, weight and BMI was significantly positive (r = 0.18, 0.45), having a coefficient greater than that of the correlation between married couples. 3. The correlation coefficient between mother and child was greater than that of the correlation coefficient between father and child. 4. The BMI of a child with either parent obese was significantly greater than that of a child with neither parent obese. Obesity appeared more frequently in children whose BMI of parent was higher. 5. A positive correlation between the BMI of young adults and that of adolescents could be found, and the average BMI of the obesity group was higher than that of the non-obesity group even in the adolescent subjects. 6. Even after considering BMI during adolescence, the familial factor had a significant relationship to the BMI of young adulthood. These results suggest that obesity in adolescence will influence obesity in young adulthood, and that the appearance of obesity strongly correlates with the familial obesity factor. In conclusion, it is very important to take preventative measures, in cooperation with the family, early in a child's adolescent years in order to avoid obesity in adulthood.

[Correlation between changes in blood pressure from adolescence to young adulthood and history of parental hypertension and obesity].

Between 1980 and 1987, medical examinations and measurement of height, weight, and blood pressure were performed on 613 persons from among a cohort of 740 persons living in Hisayama town in Fukuoka prefecture who had attained the age of 19 to 20 by the end of the study. Of this group, 439 persons (215 males and 224 females) were further studied to determine the correlation between changes in blood pressure, height, weight and obesity that had occurred since age 14-15 years old, and their family history of hypertension and obesity. 1. A positive correlation was found between blood pressure levels and BMI. 2. There was a significant positive correlation between blood pressure levels at the age of 14-15 and those at the age of 19-20. 3. Those who maintained high blood pressure levels from the age of 14-15 through the age of 19-20 had high BMI. Many of these subjects had family history hypertension, but family history of obesity did not appear to have influenced the continuity of their high blood pressure levels during this stage of life. These results indicate that there was a weak tracking phenomenon for blood pressure from adolescence to young adulthood, and that family history of hypertension and continuity of obesity were detrimental factors contributing to continued high blood pressure levels. In conclusion, it is crucially important to take measures against obesity in early adolescence, especially in the presence of family history of hypertension, in order to prevent adulthood hypertension.

[A sensitization effect of hematoporphyrin oligomer (HpO) and caffeine for X-ray radiation of skin cancer].

Human malignant melanoma cells (G-361) were irradiated with X-ray for different irradiation times. The amount of DNA damage was much increased by the treatment of the cells with HpO before X-ray irradiation. Furthermore, it was found that the caffeine inhibited effectively the repair of the damaged DNA. Furthermore, HpO was administered before X-ray irradiation of transplantable skin tumor in C3H mice and/or caffeine+ was to be given the mouse after X-ray irradiation. The growth of the tumor irradiated with HpO and/or caffeine was more inhibited and the necrotic area was more expanded than that of the tumor irradiated X-ray only. Labeling Index utilizing bromodeoxyuridine (BrdU) in the viable area of the treated tumor decreased when HpO and/or caffeine were administered. The amount of DNA damage more increased in the tumor cells irradiated with HpO than that irradiated only. And, the repair of the damaged DNA was more delayed in the tumor cells irradiated with HpO and caffeine than that irradiated with the other treatments. In conclusion, it was considered that HpO might be a radioactive sensitizer and caffeine enhanced the inhibition of the repair of DNA damaged. We hope that the treatment of X-ray radiation with HpO and caffeine is useful for clinical treatment of human malignant tumor.

Smoking and cardiovascular disease in the general population in Japan.

The effects of cigarette smoking on cardiovascular disease were investigated in the Japanese community of Hisayama. This long-term prospective study assessed the occurrence of coronary heart disease and non-embolic cerebral infarction in relation to smoking habits at entry. The study population comprised 1603 subjects aged greater than or equal to 40 years. Seventy-six per cent of 699 men and 17% of 904 women were smokers. During 23 years of follow-up, 73 cases of coronary heart disease and 175 cases of non-embolic cerebral infarction developed. In both sexes, coronary heart disease occurred more frequently in those who smoked greater than 10 cigarettes per day than in those who never smoked. There was no relationship between smoking status and the incidence of non-embolic cerebral infarction. When more than one of the major risk factors (smoking, hypertension and elevated cholesterol) was present in the same person, the risk of coronary heart disease increased 2.4-7.7 times. Cox's proportional hazards model showed that cigarette smoking was an independent and significant risk factor for coronary heart disease, but not for non-embolic cerebral infarction.

Ciliated hepatic foregut cyst. A mucus histochemical, immunohistochemical, and ultrastructural study in three cases in comparison with normal bronchi and intrahepatic bile ducts.

We describe herein the morphologies of three cases of ciliated hepatic cysts and compare them with those of normal bronchi and intrahepatic bile ducts. Grossly, the hepatic cysts were located in the subcapsular region. They were solitary, unilocular, and rather small (less than 4 cm in diameter). Histologically, the cyst wall consisted of four layers: pseudostratified ciliated columnar epithelia with mucous cells, subepithelial connective tissue, smooth-muscle bundles, and an outermost fibrous capsule. The epithelial cytoplasm contained neutral, carboxylated, and sulfated mucus. We noted moderate immunoreactivity to keratin, epithelial membrane antigen, carcinoembryonic antigen, DU-PAN-2 and secretory component; weak immunoreactivity to cytokeratin CAM 5.2, cytokeratin AE1 + 3, and carbohydrate antigen 19-9; and faint or negative immunoreactivity to IgA and IgM. Cilia were immunoreactive to actin and tubulin; smooth muscles were immunoreactive to actin and desmin. Ultrastructural observations revealed definite cilia arranged in a 9 + 2 pattern as well as mucous cells. These morphologic features of the hepatic cysts were similar to those of normal bronchi but different from those of normal bile ducts. Our findings suggest that ciliated hepatic cysts arise from the embryonic foregut and differentiated toward bronchial structures in the liver.

[Cellular kinetics and the nuclear ploidy pattern in relation to the gross pathological classification and location of advanced colorectal cancers as analyzed by DNA-RNA cytofluorometry].

Correlated studies on the cellular kinetics in association with the nuclear ploidy pattern and the pathological morphology of advanced colorectal cancers have been carried out using DNA-RNA cytofluorometry with an AO stain. The results showed that advanced colorectal cancers could be separated into the 3 distinct groups that follow: group I, involving a diploid cell population; group II, a polyploid cell population; and, group II', an aneuploid cell population. The group I type was located in the entire colorectal region, where as groups II and II', were in the sigmoid and rectum. Three groups were mostly Borrmann type 3 cancers, and the gross pathological classification was not found to be related to the ploidy pattern.

[A primary malignant melanoma of the small intestine].

In October 14, 1982, a 30-year-old male was admitted to the hospital because of a left flank pain and melena. On undergoing testing, an upper gastrointestinal series revealed barium retention in the small bowel, in which an abdomen X-p showed an abnormal gas shadow. Endoscopic examinations revealed depressive lesions, and the histological diagnosis was malignant neoplasm with a duodenal wall infiltration. A superior mesentric arteriogram showed the dilatation of the media colic artery and neovascular lesions. Thus a leiomyosarcoma of the small intestine was suspected, and the patient underwent surgery on November 15. The tumor was sack-like and looked black; histologically it was diagnosed as a small intestine, because the primary origin was only detected in the small intestine. Four months after the operation, the tumor recurred and the patient died in June, 1983.

[Comparison of intraocular penetration level of cefuzonam sodium and cefotiam after intravenous and subconjunctival injection].

We investigated the intraocular penetration of a new Cephem type antibiotic: Cefuzonam sodium (CZON) following intravenous and subconjunctival injection. CZON was administered to 28 eyes in 26 patients by 1 g intravenous injection (IV) and to 11 eyes in 10 patients by 20 mg/0.2 ml subconjunctival injection (SCI). The penetration level of CZON by the IV method was from less than 0.0025-3.35 micrograms/ml into aqueous humor, from less than 0.0025-0.315 microgram/mg into iris tissue and 2.7-157.5 micrograms/ml into serum. The penetration level of CZON by the SCI method was from less than 0.0025-12.8 micrograms/ml into aqueous humor, from less than 0.0025-0.0426 microgram/mg into iris tissue and 1.03-18.5 micrograms/ml into serum. The penetration level of CZON into aqueous humor by SCI was higher than by IV administration. In comparison with the penetration level of CZON and Cefotiam (CTM), both aqueous humor and serum levels of CTM were higher than those of CZON. These results suggested that therapeutic levels in aqueous humor effective against gram-positive and gram-negative pathogens were achieved with the 1 g IV and 20 mg SCI of CZON. However levels effective against Staphylococcus epidermidis were only erratically attained.
