[Conditions suitable for the adjuvant chemotherapy of gastric cancer].

From a retrospective analysis of 1,169 cases of gastric cancer treated with curative gastrectomy and randomized controlled adjuvant chemotherapy of six different protocols from 1961 to 1981, good candidates for adjuvant chemotherapy were found to be those who had lymphatic spread of n1 to n2 or serosal involvement of S1 to S2 or stage II or III diseases. Multidrug combination therapy produced better survival than single drug therapy. Long term oral maintenance therapy was not yet concluded to be essential for the adjuvant chemotherapy of gastric cancer. Patients with relatively late stage disease should be treated earlier than two weeks after surgery.

[Clinical application of a right ventricular pressure-volume loop determined by gated blood-pool imaging and simultaneously measured right ventricular pressure].

The data obtained by ECG-gated radionuclide angiography were collected simultaneously with right ventricular pressure and thermal cardiac output (CO) obtained by a Swan-Ganz catheter in Scintipac 1200 ( Shimazu Co) in order to create a right ventricular pressure-volume (RV P-V) loop. Subjects consisted of 15 patients with old myocardial infarction (MI group), seven with angina pectoris (AP group), six with congestive cardiomyopathy (CCM group) and five with neurocirculatory asthenia (NCA group). Right ventricular end-diastolic volume ( RVEDV ) was calculated as RVEDV = CO/(EF X HR) (CO = cardiac output; HR = heart rate). Systolic work (Ws), diastolic work (WD) and net work (WN) were calculated from a RV P-V loop by Simpson's method. The measurements were performed before and 5 min after sublingual administration of nitroglycerin (NG) (0.3 mg). The results were as follows: RV P-V loops shifted towards the left lower part of the P-V plane after sublingual administration of nitroglycerin, indicating the reduction of pressure and volume of the right ventricle. Right ventricular ejection fraction (RVEF) in the MI, AP and CCM groups showed smaller values than that of the NCA group. The CCM group presented a significantly smaller value than the NCA group (p less than 0.005). RVEF of each group increased after NG. In the AP and CCM groups, it increased significantly (p less than 0.005). Right ventricular end-diastolic volume index ( RVEDVI ) showed a converse relation with RVEF. The MI and CCM groups demonstrated significantly higher values (p less than 0.05). After NG, RVEDVI of each group decreased significantly (p less than 0.001 in the MI and NCA groups, and p less than 0.005 in the AP and CCM groups). Cardiac index in all groups decreased after NG and a statistical significance was seen in the MI, AP and NCA groups (p less than 0.05). RV Ws, RV WD and RV WN showed no difference among each group in the control state, and significantly decreased after NG. This was due to the reduction of RV pressure and volume. It was indicated that the principal cause was the systemic volume reduction. We conclude that the present method using RV P-V loop might be useful as a noninvasive bedside monitoring and permits the evaluation of RV function in a clinical setting.

[Early diagnosis of pancreatic cancer and its clinical significance from prognostic point of view].

Clinical significance of early diagnosis of pancreatic cancer was studied from the prognostic point of view, based on 18 surgically resected cases which had been obtained and pathologically proven in a period of 3 years from 1978 to 1980 at the Cancer Institute Hospital. The resectability was 45.0% of all laparotomized cases (18/40). Of these, 4 cases with the greatest diameter larger than 4 cm and 7 cases with the largest diameter smaller than 4 cm invading the pancreatic capsule died within 2 years after surgery, while 6 cases without lymph node metastasis and without the capsular invasion have survived until present. It is considered that detection of a cancer limited to pancreas without lymph node metastasis may leads to the improvement of the prognosis.

[Clinical evaluation of cefroxadine in bacterial infections of the eye].

Cefroxadine (CXD) capsules and dry syrup, an oral cephem antibiotic, were administered into 120 cases with ocular infections and the following results were obtained: The daily dose of CXD capsule was ranged from 500 to 1,500 mg and that of CXD dry syrup from 17.9 to 85.7 mg/kg, and the duration of CXD administration was from 2 days to 14 days. Clinical response rates classified by diagnosis The clinical response rates were 77.8% (14/18) in blepharitis, 86.7% (26/30) in hordeolum, 62.5% (5/8) in meibomianitis, 74.6% (44/59) in conjunctivitis, 100% (2/2) in corneal infiltration, 100% (1/1) in cellulitis of the lid, in dacryocystitis and in corneal ulcer, respectively. Clinical response classified by isolated organisms The response rates on S. aureus were 80.0% (20/25), on S. epidermidis 75.8% (47/62) and on S. pneumoniae 66.7% (2/3), respectively. The overall clinical response rate on Gram-positive bacteria was 78.3% (94/120). The response rates on H. influenzae, Acinetobacter spp., P. mirabilis, E. coli and Moraxella spp. were ranged from 42.9 to 100%. The sensitivity distributions of clinically isolated S. aureus and S. epidermidis to CXD were ranged from 1.56 to greater than 100 micrograms/ml and from 0.39 to 12.5 micrograms/ml, respectively. The former showed a peak at 3.13 micrograms/ml and the latter in 1.56 micrograms/ml. Side effects in 3 cases (2.3%) out of 129 were observed. That is; glossitis, thirst feeling and palpitation in each case, respectively.

[Metachronous triple malignant lymphoma of the stomach].

A 72-year-old female who had undergone partial gastrectomy (Billroth I method) 8 years earlier because of a diagnosis of malignant carcinoma of the stomach (histopathological diagnosis: ML. Histiocytic, Diffuse--Rappaport--) visited our hospital because of dysphagia for the past three months. X-ray and fiberscopic examination revealed the same malignancy in the remnant stomach. Curative extirpation of the remnant stomach, resection of the lower esophagus and pancreatosplenectomy were performed. There were two lesions in the resected specimen. One was a large tumor of Borrmann 3 type (8 cm in diameter) at the EGJ; the other was a small IIc-like tumor (2 cm in diameter) at the surgical stump on the lessercurvature. These three metachronous malignant lymphomas returned the same pathological findings. Compared to carcinoma, malignant lymphoma of the stomach occurs more frequently in the fornix and the tendency for multiple occurrence is strong. When partial gastrectomy is performed for malignant lymphoma of the stomach, follow-up of the remnant stomach must be carried out carefully.

[Ba-induced contraction of the guinea pig ileal longitudinal muscle and its inhibition by D-600, benactyzine, and papaverine].

In the guinea-pig ileal longitudinal smooth muscle, Ba caused an initial phasic contraction, followed by a gradual decrease of the contraction (tonic contraction). In the Ca-free and the Ca-free, K-rich physiological solution, Ba could also contract the longitudinal muscle, but 30 min- and 60 min-immersion of the tissue in the Ca-free solution decreased lower than 20% of control of the Ba contraction. Although the K-induced depolarization intensified the Ba contraction after the 30 min-immersion, within 10 min after the immersion in the two solutions there is no difference in the height of Ba contractions. In the normal physiological solution, Ba seems to serve the contractile protein with free Ca ions rather than itself for the Ba contraction. D-600 induced an equal inhibition on the Ba contractions in the normal physiological solution and in the Ca-free solution, and it inhibited the tonic contraction more than the phasic one. The phasic component may be due to the cellular Ca mobilized by Ba which passes through D-600-sensitive pathway, and at least part of the tonic component may utilize the extracellular Ca. Benactyzine perferentially inhibited the Ba contraction in the Ca-free solution, and it depressed the phasic component more than the tonic one. Treatment with papaverine induced an equal inhibition on the phasic and tonic components of Ba contractions and the Ba contraction in the Ca-free solution. Benactyzine may exert the main influence on the mobilization of the cellular Ca through the competition with Ca. Papaverine seems to inhibit the Ba contractions by diverse actions.

[Histamine-induced contraction of the guinea pig taenia coli and its inhibition by D-600, papaverine, and benactyzine].

The histamine-induced contractions and effects of some smooth muscle relaxants on their contractions were examined in the guinea-pig taenia coli. Application of Ca to the Ca-free physiological solution caused a contraction of the taenia coli (Ca contraction), and histamine accelerated and increased the Ca contraction (H . Ca contraction). Histamine also contracted the taenia coli in the Ca-free solution (Ca(-) . H contraction). D-600 which preferentially reduces the Ca influx into muscle cells, more effectively depressed the H . Ca contraction and more feebly the Ca(-) . H contraction than histamine-induced contraction in the normal physiological solution. Papaverine inhibited the H . Ca contraction and the Ca(-) . H contraction more than the histamine-induced contraction. Benactyzine was a specific agent against the Ca(-) . H contraction. When the Ca contraction reached the peak, application of histamine induced a further contraction. D-600 effectively inhibited the Ca contraction and the further contraction induced by histamine. Although papaverine blocked the Ca contraction similarly to D-600, the application of histamine produced a large contraction. Benactyzine induced only a feeble inhibition on the Ca contraction, but it was effective against the following contraction caused by histamine. Histamine probably mobilizes the extracellular and cellular Ca and contracts the smooth muscle. Papaverine seems to inhibit smooth muscle contractions by diverse actions. Benactyzine may exert the main inhibition on the mobilization of the cellular Ca through the competition with Ca.(ABSTRACT TRUNCATED AT 250 WORDS)

Perchloric acid treatment of human blood for quantitative endotoxin assay using synthetic chromogenic substrate for horseshoe crab clotting enzyme.

A new reliable human blood treatment was established for quantitative endotoxin assay using synthetic chromogenic substrate [Boc-Leu-Gly-Arg-rho-nitroanilide]. Addition of perchloric acid in a final concentration of 1.25% to platelet-rich plasma or serum in a 2:1 volume ratio completely eliminated nonspecific amidase activities as well as inhibitors. By this method, the recovery of added endotoxin was nearly 100%, and was almost independent of sample dilutions and anticoagulants.

The reaction of the dura to bone morphogenetic protein (BMP) in repair of skull defects.

Trephine defects in the adult rat skull 0.8 cm in diameter, which do not spontaneously heal, were filled with a bovine bone morphogenetic protein (BMP) fraction. The defects healed not only by bony ingrowth from the trephine rim, but also by proliferation of pervascular mesenchymal-type cells (pericytes) of the dura mater. Under the influence of BMP, dural pericytes differentiated into chondroid and woven bone. Between three and four weeks postimplantation, sinusoids formed and the woven bone remodelled into lamellar bone. Concurrently, blood-borne bone marrow cells colonized the bone deposits, and the diploe were restored. Demonstrating that it is soluble in interstitial fluid, and diffusible across a nucleopore membrane (which isolated the bony margins of the skull), BMP induced new bone formation in the underlying dura and complete repair of the defect. The response of the dura to the BMP fraction produced more new bone than the response to allogeneic bone matrix. The BMP-induced repair was dose dependent; the quantity of new bone was proportional to the dose of the implanted BMP.

Relationship between the Ba-induced contraction and cyclic AMP levels in the isolated longitudinal smooth muscle from guinea pig ileum.

Relationship between contraction and cyclic AMP levels induced by BaCl2 was examined in the longitudinal smooth muscle isolated from guinea pig ileum. BaCl2 3 X 10(-3)M caused a fast initial contraction, often followed by a gradual decrease of the contractile state. There was an increase in the tissue cyclic AMP 7 min or 14 min after the application of Ba. A phosphodiesterase activator imidazole reinforced the later phase of contraction by Ba and inhibited the increase in cyclic AMP. These results indicate that there is still a positive correlation between relaxation and increase in cyclic AMP and that an inhibitory action mediated by cyclic AMP is veiled behind the Ba contraction. Furthermore, these results may be interpreted by assuming that strong Ba contraction operates a feedback mechanism and that the feedback mechanism is associated with cyclic AMP increase. Indomethacin, an inhibitor of prostaglandins synthesis, little influenced the Ba-induced increase in cyclic AMP and rather inhibited the Ba contraction. Propranolol, a beta-adrenergic blocking agent, failed to exert influence on the Ba contraction. Based on these facts, it is suggested that the increase in cyclic AMP is not mediated by prostaglandins or catecholamines.
