Bleeding secondary to vitamin K deficiency in patients receiving parenteral cephem antibiotics.

Thirteen patients who were treated with cephem antibiotics developed blood coagulation disorders due to vitamin K deficiency. At the time of detection of prolongation of the prothrombin time, cefazolin was being administered in four cases, cefmetazole and latamoxef (moxalactam) in three cases each, and cefoperazone, cefpiramide and ceftazidime in one case each. Bleeding occurred in ten patients, but administration of vitamin K rapidly eliminated the prothrombin time prolongation and the haemorrhagic tendency. To date, we have not detected prothrombin time prolongation or a haemorrhagic tendency with cefotaxime, ceftizoxime, cefotiam, cefoxitin or cefsulodin. Thus, different cephem antibiotics show different effects on the prothrombin time.

Follicular and uterine prostaglandin levels in relation to uterine contraction and the first ovulation of a sequence in the hen.

Changes in the concentration of progesterone, estrone, estradiol, prostaglandins (PG) E2, 6-keto F1 alpha and 13,14-dihydro-15-keto F2 alpha (PGFM) were measured in peripheral plasma, and in venous effluent from the shell gland and the largest (F1) and the second largest (F2) preovulatory follicles. Tissue concentrations in the F1, F2 and the most recently ruptured follicle and the shell gland also were determined. Changes in these criteria were compared to changes in uterine contraction before the first ovulation of a sequence. Significant increases of PGF2 alpha and PGFM in the peripheral plasma were observed when the frequency of uterine contraction reached a maximum, about 1 h before ovulation. Relative to peripheral plasma, the concentrations in F1 plasma of progesterone, PGF2 alpha and PGFM were increased 20-fold, 150-fold and 15-fold, respectively, at the time of the maximum frequency of uterine contraction. The highest tissue concentrations of PGs were also observed in the F1 follicle. These results suggest that the largest preovulatory follicle is the major source of PG synthesis and release. These PGs may stimulate uterine contraction and may also play a role in follicular rupture and release of the ovum.

[Histological statistics of malignant urogenital tumors].

A statistic survey was carried out on the histology of the 465 malignant tumors at our urological department during the last 10 years. The most frequent tumors were bladder tumor (50%), prostatic tumor (17%) and renal tumor (15%). Histological typing and grading were tabulated for each tumors.

[The Pediatric Urology Conference].

This meeting was first established in 1972 for the purpose to make free discussion between the urologists and pediatric surgeons on pediatric urology. Several themes, such as undescended testis, hydronephrosis, intersex and stone disease, have been talked over. Ano-rectal malformation was one of the main themes, and many different opinions from both urological and surgical sides have been presented for the prevention of the complications of pelvic surgery. Pediatric urology has undergone a remarkable progress in the past 30 years. The main contribution has been brought about through the better understanding of the diseases in this field. Newer diagnostic methods and operative techniques have also influenced the development. What is left for the future in pediatric urology is how to preserve the renal function. It is not rare that the diseased child goes on to the end-stage renal failure, in spite of the successful intervention of urinary tracts. Hypodysplasia of the kidney is still incurable in the modern medicine. The best alternative may be renal transplantation. Pediatric urology is one member of a team of specialists to whom cases are referred by pediatricians and obstetricians. Earlier diagnosis, for example by the echogram of the fetus, and higher operative techniques are the themes in the future.

[A case report of infarction of the bilateral thalamus and the midbrain].

A 63 years old man was admitted because of coma of 24 hours duration. He had a history of infarction of the medulla oblongata 7 months earlier. On admission he was unconscious and had right oculomotor nerve palsy. To a painful stimulus, he responded with only a slight body movement. On the CT scan, low density areas were seen in the bilateral thalamus and the midbrain. In the next day he recovered from coma and abnormal behaviors appeared such as wondering in the ward, touching other patient's face and manipulating his stool. Verbal expression was extremely poor. Chlorpromazine was needed to suppress abnormal behaviors. Thereafter, abnormal behaviors gradually subsided and by the 23 hospital day he became bedridden even when chlorpromazine was withdrawn. At that time, he said only "yes" or "no" in response to questions and only few spontaneous movements were observed. From the 40 hospital day he suffered from pneumonia and gradually sank into the state of akinetic mutism. He died on the 50 hospital day. Pathologically, infarctions destroyed the n. parafascicularis, n. centromedianus and the intralaminar nuclei in the left thalamus, n. parafascicularis and a part of n. centromedianus in the right. In the medulla oblongata, right n. ambiguous and reticular formation were destroyed. Both thalamic lesions extended posteriorly to unite with the midbrain lesion.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of fracture of the penis].

A case of fracture of the penis was presented. A 44-year-old man in the state of morning erection, suddenly heard an abnormal cracking sound while he rolled over in bed. He immediately felt pain of the swelling penis. Surgery was carried out soon after he was admitted. Postoperative course was uneventful, except for slight pain on erection. The Japanese literature was reviewed, and the clinical statistics on 208 patients were summarized.

[Localization of hypoplastic kidney by enhanced computed tomography: experiences with five ectopic ureters with hypoplastic kidneys].

Five children with ectopic ureters and hypoplastic kidneys are reported. Hypoplastic kidneys in these cases could not be detected by excretory urography, renal scintigraphy or other routine examinations. Finally, in three cases we could diagnose the localization of hypoplastic kidney by enhanced computed tomography. Enhanced computed tomography is very valuable for diagnosis of localization of hypoplastic kidney.

[A case of urinary schistosomiasis in a Japanese male].

Urinary schistosomiasis is an endemic disease mainly distributed in most of Africa and Southwest Asia, and caused by infestation with Schistosoma haematobium. The case presented here was a 33-year-old Japanese male who had suffered from schistosomal infestation in the endemic area, and diagnosed as urinary schistosomiasis by detection of Schistosoma haematobium ova in urine and specimens obtained by mucosal biopsy of the bladder. Radiographic and endoscopic examinations revealed neither obstructive uropathy nor urothelial malignancy. The patient underwent treatment with antimonyl sodium tartrate (Stibnal), and the disease is under good control.
