[Spontaneous hyperstimulation of the ovaries with luteal cysts and ascites during a twin pregnancy-extreme case of the syndrome of painfull early pregnancy (author's transl)].

Laparotomy was performed in a para II gravida II of 28 years in an early twin pregnancy. The left ovary was enlarged to 18 X 8 cm, the right ovary to 1i X 8 cm by luteal cysts. The abdominal cavity contained an ample amount of peritoneal fluid. The history is understood as spontaneous ovarian hyperstimulation, and as an extreme case of the relatively common syndrome of painfull early pregnancy, occasionally leading to unnecessary surgical intervention.

The relative bioavailability and pharmacokinetics of carbamazepine.

Differences in the bioavailability after administration of two pure carbamazepine samples that were different in particle sizes have been found. Carbamazepine was given to each of 3 healthy male volunteers in a single oral dose of 400 mg in gelatin capsules. The other carbamazepine sample was administered after a three-week time interval. Plasma was collected during the following 96 hr. Quantitative determination of carbamazepine was performed by gas liquid chromatography. By comparing the areas under the various curves for each volunteer, the relative bioavailability of one of the carbamazepines in relation to the other one was found of approximately 70%. There were no differences in the half-life time.

Concentration behavior of carbamazepine in bile and plasma of man.

The concentration in plasma and bile of carbamazepine was determined in four humans with Kerr's T-tube drain after an oral administration of a single dose of 400 mg of Finlepsin (carbamazepine). The mean plasma half-life time was 25 (15-31) hr. The concentration time curves in bile and plasma were parallel with the bile to plasma concentration ratio of 0.62 (0.24-0.82). A negative linear correlation between the lithogenic index and the bile plasma concentration ratio was found. It is suggested that the concentration of carbamazepine in bile is influenced by the saturation of biliary micelles with cholesterol. The amount of carbamazepine eliminated by bile within 72 hr was 4.1 +/- 1.9 mg, i.e., 1% of the administered dose. It is concluded that there is no significant enterohepatic circulation of carbamazepine.

Dizepam metabolism in human foetal and adult liver.

Diazepam was metabolized by human foetal liver microsomes to N-desmethyldiazepam and N-methyloxazepam as early as the 13th week of gestation. The metabolic activity was lower than that of microsomes from adult human liver. Diazepam was shown mainly to be hydroxylated to N-methyloxazepam at substrate concentrations higher than 0.1 mM. Diazepam levels above 1.0 mM were inhibitory to the overall metabolic reaction. SKF 525-A inhibited diazepam metabolism by foetal liver microsomes at a concentration of 0.1 mM. The addition of diazepam to foetal and adult human liver microsomes resulted in a type II spectral change. Its inhibition by carbon monoxide indicated that biotransformation of diazepam was performed by the cytochrome P-450-linked mono-oxygenase system.

Long-term haemodialysis-an enhancing factor for cadaveric graft survival.

In 220 consecutive cadaveric kidney transplants, graft survival in patients dialysed for a period of two years or longer was significantly greater than in those dialysed for less than two years. More blood transfusions were received and the formation of HLA antibodies was greater in the group dialysed for more than two years. A positive cross match was the basis for excluding 21% of the HLA positive patients from transplantation during a one year period of observation. When these same HLA positive patients had more than two subsequent opportunities for donor-recipient selection the exclusion rate dropped to 2%. The longer period of dialysis with more blood transfusions had a favourable influence on cadaveric graft survival whereas the formation of HLA antibodies had a negative influence. Even in HLA positive patients graft survival was better in the long-term dialysis group when compared with the group dialysed for a shorter period. These results suggest that longer dialysis with more blood transfusions represents an enhancing factor for cadaveric kidney grafts.

[Endometrioid carcinoma of the spinal canal (author's transl)].

A woman, born in 1923, suffered since menarche of pain of the vertebra and has had a headache during menstrual periods. 1963 she developed a transverse lesion of the cord with an incomplete paraplegia because of an endometriosis externa of spinal canal. After a 12 years symptomless interval, she complained again of a transverse lesion. Histological examination disclosed an endometrioid carcinoma with infiltration of vertebral bodies. The rare occurrence of malignant transformation in endometriotic tissue is pointed out.

[Psychological aspects of hysterectomy (author's transl)].

The effects of hysterectomy on social and psychological behavioural changes were evaluated in 88 patients who were examined psychologically and somatically preoperatively and for a period of 5 to 6 months postoperatively. All patients were aware of the surgical consequences and were investigated in regard to social history and by means of psychological tests (HAWIE, MMPI, Rorschach). The personality profiles of these patients fell within the normal range, although showing a tendency towards psychosomatic alteration, which, however, decreased significantly after the operation. Hysterectomy does not appear to produce any clear-cut changes in the attitude toward sexuality according to the findings of this study. Neither the age at which sexual maturity was reached, nor the intelligence level, number of children, age nor social class appeared to show any significant influence on the capacity to achieve orgasm or the frequency of intercourse. Only three patients showed transient postoperative depression, although this is frequently described in the literature; its aetiology is discussed. The therapeutic approach, especially in the field of surgical gynaecology, must encompass every aspect of medicine and appropriate explanations of the surgical consequences should be given to the patients to prevent postoperative psychological complications.

[Representation of gynaecological fistulas by wick urethrocystocolporectography (author's transl)].

In connection with gynaecological diseases and/or their therapy, genital fistulas constitute a grave complication. Accurate representation of these fistulas in conjunction with the adjacent hollow organs is of curcial importance for their successful removal. A modified kind of wick urethrocystocolporectography, which is a method that has proved efficient in cases of a dislocation in the female genital tract and also in cases of insufficiency of locking of the urinary bladder, provides these preconditions for an efficient therapy. That is, through the difference in the course of motion during holding and pressing in comparison with rest there are detected not only any valve mechanisms present but also dislocations, if any. Moreover, the method, easy to apply as it is and involving only little strain, provides an advantage which cannot be overlooked.

[Choroid neurilemmoma. Considerations for clinical differential diagnostics].

A report is given on an extremely rare case of neurilemmoma of the choroid and observations are made on the difficult differential diagnosis of malignant melanoma. Clinical diagnosis and treatment apart from enucleation, was impossible. The recognition of the nature of this tumor could be made only on histological examination.
