Antiepileptic drug levels and side effects in man.

It was found in 22 hospitalized epileptic patients taking DPH, primidone, and phenobarbital as a combined therapy that the number of side effects per patient could be correlated only with the serum levels of DPH. In the case of clinial manifestation of side effects it might be concluded that the dosage of DPH should be reduced. One of our patients was treated in this manner resulting in a reduction of the side effects, whereas the frequency of seizures increased. But we succeeded in reducing this frequency by phenobarbital and side effects completely disappeared. If there are any side effects, then, according to our results, it seems to be better to add phenobarbital in order to decrease the DPH serum level instead of reducing DPH dosage. It is, however, known that by phenobarbital the DPH level in patients need not necessarily decrease.

[Comparison of the Limulus test with the pyrogen test in rabbits].

The Limulus test was compared with the official pyrogen test in rabbits for detecting pyrogens in medical devices for single use. 46 samples were examined. Two of them were positive in the rabbit test and also in the Limulus test; 5 samples were only positive in the Limulus test. This is in agreement with published papers. Against endotoxins of gram-negative bacteria the Limulus test is more sensitive than the common test in rabbits, but not sensitive for other pyrogens. Therefore, the Limulus test cannot replace the official rabbit test in the quality control of medical devices. We recommend the Limulus test as a quick, simple and highly sensitive screening method for gram-negative bacterial endotoxins, the most common pyrogen contaminants in drugs.

[Possibilities and limitations of endoscopic retrograde cholangiopancreaticography (ERCP)].

Important experiences are reported from an analysis of 500 examinations. The ERCP gives eminent possibilities for the improvement of the pre- and postoperative diagnostics of the bile ducts, particularly in unclear cholestasis and in conditions after operations of the bile ducts. The ERCP is little suited for the early diagnosis of the carcinoma of the pancreas and for the ascertainment of the diagnosis in easy and moderate forms of chronic pancreatitis. Of particular value is the possibility to clear the causes of recidivations of pancreatitis as well as of complications of the severe chronic pancreatitis with regard to operative consequences.

Ultrastructural aspects of bubble formation in human fatal accidents after exposure to compressed air.

Electron microscopic investigations were performed on samples of human tissue obtained from subjects following fatal decompression sickness, associated with hyperbaric air-therapy. Intra- and extracellular gas bubbles of varying size were identified throughout the entire body. Each bubble was covered by an osmiophilic non-homogeneous coat of cloudy and flocculent material, native to its specific locality. This envelope measured from 30 to 560 Angstroem-units in thickness. Association of this covering with an electrokinetic zonal activity, detected biophysically by Lee and Hairston (1971) is assumed. We consider this surface coat prevents nitrogen from being eliminated via the blood-lung barrier.

[Fatal decompression sickness following therapeutic hyperbaric environment (author's transl)].

In Hannover/Germany in 1976 a so called "Society for Regenerative Hyperbaric Therapy" (Gesellschaft für Regenerative Uberdruck-Therapie) subjected 20 patients within two coupled multi-place chambers to a simulated hyperbaric environment, equivalent to a maximum of 4 ata, followed usually by gradual reductions of the pressure. The patients were of an average age of 67.2 years and were afflicted by various disorders. During one of the "dives" a patient developed air embolism of the central nervous system. During ensuing confusion of the chamber was opened suddenly, with resultant explosive reduction of the high pressure. This resulted in five letal accidents of decompression sickness. Post mortem examination revealed diffuse distribution of gas bubbles throughout the entire body. By elecronmicroscopy each bubble was covered by an osmiophilic coat. Post mortem findings are discussed and correlated with well-known and new clinical symptomes. These observations present for the first time fatal accidents of decompression sickness in humans, associated with hyperbaric air-therapy.

[Clinical significance of serum phenytoin level].

Phenytoin serum concentrations in 115 epileptics treated on an inpatient basis showed, in two groups, only relatively weak correlation with the dosage used. The range of variation was assigned a factor of 16. For individual patients there was observed a correlation between the dosage in mg/kg and the corresponding phenytoin serum concentration. The level of phenytoin serum concentration was not found to be related to the frequency of attacks. The effective phenytoin serum level showed interindividual differences and can be low as well as relatively high. Resulting from this is the need for determining, from the determination of serum concentrations, the effective serum concentration for each individual patient.
