Long-term results following fixation of the vagina on the sacrospinal ligament by the vaginal route (vaginaefixatio sacrospinalis vaginalis).

Vaginaefixatio sacrospinalis vaginalis is an operative procedure whereby the vaginal stump is affixed to the sacrospinal ligament of one side of the vaginal route. Intercourse is not inhibited by this operative method. This technique was performed on 81 patients, starting in 1959, with a follow-up period of up to 10 years. In 78 cases the indication for operation was a true vaginal vault prolapse following hysterectomy; in three cases it was a prolapse of the uterus and the vagina because of complete incompetence of the visceral fascia of the pelvis. The vaginal vault prolapse was alleviated by the colpopexy technique in all patients. However, coexisting cystocele, rectocele, and enterocele and related incontinence remained in a few instances.

[Explanations for the standard in roentgen diagnosis: basic tests of the thoracic organs--adults].

Explanations are given concerning the standard of radiodiagnostics--basis examination of the organs of the thorax/TGL-No. 36661, which shall facilitate the understanding of the parameters given. It results from the fact that the roentgenological examination of the organs of the thorax begins with a posterior-anterior ray-path radiograph of the thorax. In these cases the large format technique with film-foil-combination and the fluorography are classified as equivalent. With regard to the high speed of movement to be expected is striven for the achievement of possibly short times of exposure. The authors briefly enter the particularities of the radiograph of the thorax in lateral as well as in the full size radiograph sitting and lying positions and at the patients bed. Due to the law solution and the higher radiation dose the fluoroscopy is to be used only with regard to the findings and other information concerning the patient.

[Purpose and usefulness of the present day risk catalog (author's transl)].

Risk catalogs have proved to be an important instrument of preventive medicine for the identification of pregnancies at risk. Their purpose and usefulness is beyond all doubt. Nevertheless problems of principle are attached to them since they deflect the attention from the living reality of the individual case to statistically formed collectives. The importance of the problems for risk catalogs changes according to the social change and the constantly improved perinatal results. Age, parity and socioeconomic risks reflect such trends in development most clearly. Future tasks lie in the quantification of risks by differentiating the degrees of severity. A certain intrinsic dynamism of the present risk catalogs leads, e. g. to a constantly increasing incidence of sections. For this reason a constant feedback must exist between risk catalog and statistics.

[Transsexualism and the H-Y antigen].

First reports about incongruous H-Y antigen status in male-to-female and female-to-male transsexuals have been published by us in 1979. Meanwhile H-Y antigen expression was analyzed with the cytotoxicity assay of Goldberg et al. in 61 transsexuals. In 55 cases H-Y antigen status was found discordant with the anatomical, chromosomal and hormonal sex and corresponded to the gender identity of transsexuals. The relative frequency was 0,91. In 33 male-to-female transsexuals 29 were H-Y negative, one was intermediate, three were H-Y positive. In 28 female-to-male transsexuals 25 were H-Y positive, one was intermediate, two were H-Y negative. The new findings of an incongruous H-Y antigen status in genuine transsexuals may lead to new considerations about the pathogenesis of the disease and about the function of H-Y antigen.

Serum-induced chronic pancreatitis.

Clinical research into patients with idiopathic chronic pancreatitis points to a possible immunopathogenetic process in a number of cases. In order to examine the behaviour between the exocrine pancreas under the influence of anti-rat-pancreas immune serum produced in rabbits, a 1.00 ml immune serum is administered once a week over a maximum 26 week period into Wistar-rats by intraperitoneal injection. By electrone-microscopy a much reduced production of enzymes apparently takes place, though to differing extent. There is also destruction of the basal membrane of acinocytes: the production of interstitial oedema, the new formation of collagen fibres and the proliferation of connective tissue cells. Under a conventional light microscope the first changes become noticeable after 8-12 weeks of study. These take the form of localised cell decay, deterioration and lysis of acinocytes; and an increasing non-specific inflammation. There is also the new formation of connective tissue. After 20-26 weeks the exocrine pancreas is characterised by reduction of parenchyma, acino-ductal metaplasia, chronic inflammatory infiltrates of differing density, fibrous and irregular calibres of the smaller and larger ducts. The findings are almost identical to the structural changes of chronic idiopathic pancreatitis in human beings. The results support the view of an immuno-pathologic aetiology for human chronic idiopathic pancreatitis.

Synthesis of Saccharomyces cerevisiae catalase A in vitro.

mRNA isolated from cells of the yeast Saccharomyces cerevisiae was translated in the cell-free protein synthesis system from wheat germ. Catalase proteins synthesized were isolated from incubation mixtures by immunoadsorption followed by polyacrylamide gel electrophoresis in the presence of sodium dodecyl sulfate. On dodecyl sulfate gels catalase A synthesized by the wheat germ system migrates like catalase A protein synthesized in vivo. Evidence is presented that yeast catalase T and A synthesized in vivo are no glycoproteins. Synthesis of the two catalase proteins in the wheat germ system and dissimilarity of proteolytic fingerprints of the two proteins demonstrate conclusively that catalase T and A are biogenetically unrelated.

The "Internal Leak" as a possible cause in the pathogenesis of peptic ulcer.

In cat gastric mucosa stimulated with histamine and damaged by direct application of acetylsalicylic acid, the fluorescent dye acridine orange may be found outside the parietal cell. Normally it is distributed throughout, or bound to the limiting membrane of, the vesicles in the parietal cell. The special properties of this cationic dye in an acid environment, support the hypothesis that a so-called "internal leak" may possibly play an important role in the pathogenesis of peptic ulcer.

The effect of delta-aminolevulinate on catalase T-messenger RNA levels in delta-aminolevulinate synthase-defective mutants of Saccharomyces cerevisiae.

Total RNA was isolated from mutants of Saccharomyces cerevisiae that lack active delta-aminolevulinate synthase and are therefore defective in heme biosynthesis. The RNAs were translated in the cell-free protein synthesis system from wheat germ, and the catalase T synthesized was isolated by immunoadsorption and polyacrylamide gel electrophoresis in the presence of dodecyl sulfate. Little or no catalase T product was detected with RNA from mutant cells grown in the absence of a heme precursor. As judged from in vitro translational capacity, RNA fractions from mutant cells grown in the presence of delta-aminolevulinate contained at least 10 times more catalase T mRNA than RNA from unsupplemented cells.

[Acute disease with fever with rapidly developing shock and ecchymoses].

The history, the clinical and laboratory findings of a 13 year old boy, who has initially shown the signs and symptoms of febrile infectious disease are presented. The petechial and purpuric skin lesions have been suspected to be a rash. Accurate diagnosis therefore has been delayed up to 20 hours after onset of specific skin lesions. A very rapid deterioration made adequate treatment unsuccessful. It is pointed out that prompt accurate diagnosis and medical treatment in this serious infectious disease may improve the overall poor prognosis.

[The gynaecologist's view of lower back pain (author's transl)].

Pain is a sensation, "lower back" a vague anatomical term, and lower back pain no disease, but a multilayered symptom. Women with this condition usually first see the gynaecologist, but in only approximately 10% of these cases is the pain referable to disease of the genital organs. 90% are due to inflammatory or degenerative skeletal disease. The differential diagnosis of lower back pain requires knowledge of innervation. Head's or Mackenzie's zones, blood supply, and anatomy of the pelvic area. The effects of pregnancy on the sacroiliac joints, anomalies of positive adnexitis, endometriosis, weakness of the pelvic muscles and connective tissue as a result of trauma during childbirth or on a genetic basis, as well as psychosexual vasomotor alterations may play a causative role. Differentiating pointers and therapeutic methods are presented.

Diagnostic problems in differentiation of uterine sarcoma.

A 35 year old patient underwent radical abdominal hysterectomy for uterine malignancy, which was confined on curettage material. Histological examination revealed a wide range of atypical mesenchymal components. For further specification ultrastructural examination was performed. Intracytoplasmic microfilaments, sometimes arranged in bundles, were a feature common to all tumor cells despite their shape and arrangement. The tumor thus was classified as a polymorphcellular fibrosarcoma. The patient is now free from recurrences after a period of two years.

[Transexuality and X-Y antigen (author's transl)].

Eleven transexuals with transexuality from man to woman and a karotype 46 X-Y and 11 transexuals with transexuality from woman to man and the karotype 46 X-X were investigated for H-Y antigen. Eight of the 46 X-Y transexuals were H-Y antigen negative, one intermediately slightly positive and two H-Y antigen positive. Of the 46 X-X transexuals 9 were H-Y antigen positive, one was intermediately slightly positive and one was H-Y antigen negative. Two groups of transexuals can be distinguished. Genuine transexuals and secondary transexuals with a transvestite past who have suffered a sexual identity crisis due to the environment. The H-Y antigen is an expression of a complex of the genes which is responsible for virilization and is likely located in the Y chromosone. The gene responsible for the H-Y antigen expression is not identical with the inductor for testicular development. An explanation for the disordance of the H-Y antigen findings in transexuals is translocation or gene exchange from a Y-chromosome to an X-chromosome during meisis of the spermatogonia. In this way a morphological substrate for transexuality has been detected. H-Y antigen negative 46 X-Y males and H-Y antigen positive 46 X-X females can now be diagnosed as genuine morphological transexuals.

Distribution of antiepileptic drugs between plasma, plasma water, cerebrospinal fluid, saliva and brain.

The following conclusions can be drawn from our results. The concentrations in cerebrospinal fluid correspond to the concentrations of the unbound part and saliva at a high lipid solubility. Cerebrospinal fluid and brain were interpreted as a uniform compartment. The unbound concentrations of low lipid soluble drugs in plasma do not reflect the concentrations of drugs in cerebrospinal fluid. The correlation between the unbound drug in plasma, CSF and brain must be determined. The concentrations of lipid soluble drugs in plasma, water or saliva is representative for the concentration of antiepileptic drugs in central nervous system.
