Intraoperative lymph scintigraphy during radical surgery for cervical cancer.

Intraoperative lymph scintigraphy during radical surgery for cervical cancer was developed in the course of a program covering three periods. During the last period technetium-99m antimony sulfide has been used to visualize pelvic lymph nodes. Surgery is done with a modified gamma camera serving as an operating table. This ensures intraoperative monitoring and greater thoroughness of lymphadenectomy. The introduction of the technique has improved the rate of total lymphadenectomies and has increased both the yields of involved nodes and the 3-yr survival rates.

Control by radioisotope labeling tissue of complete or incomplete lymphadenectomy.

Nuclide-assisted radical surgery for cervical carcinoma was developed in three stages. The improved rate of lymphnode removal thus obtained was associated with a reduced mortality from recurrent tumor both in cases with and without lymphnode involvement. Results in the third period under review suggest that patients undergoing incomplete lymphadenectomy have a significantly poorer prognosis than those with complete removal lymphnodes.

[Intraoperative lymphoscintigraphy in radical surgery of cancer].

The use of a gamma-camera operating table and the intraoperative lymphoscintigraphy using 99mTc-Antimony sulfide in radical surgery of carcinoma of the breast allows a better control of axillary lymphadenectomy. Remained lymph nodes are localized by an indicator filled with technetium and removed secondarily. Due to this technique it is possible to control and increase the extent of the axillary lymphadenectomy. Since the introduction of intraoperative axillary lymphoscintigraphy 83 patients were treated by mastectomy and axillary lymphadenectomy. In 48 cases complete lymphadenectomy could be obtained.

Estimation of the uteroplacental perfusion by use of 113mIn-transferrin and an iterative regression method.

For the clinical evaluation of the uteroplacental blood flow the radioactivity curve over the placenta is recorded by means of a gamma scintillation camera and is calculated by iterative regression method. The flow curves were attributable with good clinical relevance to 3 perfusion types: perfusion type I indicates unimpaired uteroplacental flow. Type II is a transitional type to the pathological type III. Over two-thirds of the type III cases gave births to babies with signs of moderate to severe malnutrition and in 1 case an intrauterine death occurred.

Prevention of urological complications after radical operation of cervical carcinoma.

Cervical carcinomas of stage Ib and II have been treated for extended radical therapy by radioisotope radical surgery at the First Department of Obstetrics and Gynecology, University of Vienna, Austria. This extension of a more radical surgery requires to take measures in order to prevent urological and other postoperative complications. Chromocystoscopy, intravenous urography and functional scintigraphy of the kidney are carried out as routine preoperative investigations. Preservation of the adureter and an exact drainage of the field of operation are taken as intraoperative measures. Main emphasis of postoperative prophylaxis is the stimulation of ureter activity by distigminbromide and hexoprenaline. Distigminbromide stimulates the prevesical part of the ureter and increases the number of urinary excretions into the bladder. Hexoprenaline is a betamimetic substance which increases both local blood flow and number of ureter contractions. In addition, treatment by antibiotics, thrombosis prophylaxis with heparin and marcoumar and urine drainage by catheter are performed during surgery. The results of 187 radioisotope radical operations show that an uretero vaginal fistula was observed ruly in one patient (0.53%). Hydronephrosis was registered in 10 females (5.3%) and 42 subjects (22.5%) lack of bladder feeling was noted and incontinence was observed in 22 patients (11.8%) in the postoperative period.

[New aspects of registration and drug influence of ureteral motility ].

A new method to determine ureteral motility by means of radioisotopes is presented. After administration of 99m-Tc-MDP or 99m-Tc-DTPA urine transport within the ureter is documented by functional way-time-matrices. The special resolution results of a series of 7 to 12 regions of interest covering the ureter out of which the information about its motility is extracted from a series of 300 to 480 single pictures. The time resolution is 1 to 2.5 seconds. The observation period varies between 5 and 20 minutes. Urine transport is outlined as an declined stripe in the matrix. Retroperistaltic waves or dyskinesies can also be detected with this method. 31 patients in whom radical surgery was performed for carcinoma of the cervix were examined by the described functional ureter test. The results show that changes in ureteral function like frequency or quality or even retroperistaltic contractions of the ureter can be observed by the urokinetogram, changes which can be traced neither by isotope renogram nor by intervenous pyelography. Similar observations could be made in 10 patients treated by radiotherapy. In 26 patients the influence of sympathico- and parasympathicomimetic agents on the ureteral motility was studied. The results show that an increase of contraction frequency can be obtained by alpha- and betamimetics and also by cholinesterase inhibitors. These results which are in consistance with the available literature will be analysed and discussed in detail. The urokinetogram opens new possibilities to get insights into the physiology of urine transport and is therefore of great interest as an addition to radiologic and urodynamic methods.

[Calculation of the probability of survival after combination surgical-radiotherapeutic treatment of endometrial carcinoma].

The probability of survive of 106 patients suffering from endometrial cancer after combined treatment by operation and irradiation was calculated and compared with the percentage of healed patients after 5 years. It could be demonstrated that by set up of graphs of survive the different tendencies of different kinds of treatment were visible very soon, while the usual calculation of the percentage of healed patients after 5 years did not indicate any significant difference. To find out the optimal kind of treatment of endometrial cancer the evaluation of a maximum of parameters of the patient as well as of the therapy is demanded.

[Intranatal Administration of midazolam (author's transl)].

First experiences with Midazolam during parturition of 109 women show that this drug can help to ease delivery by psychical and muscular relaxation. This results also in a shorter length of the delivery period. Midazolam has a half-life of about two hours and can be applied simultaneously with oxytocin by motor driven syringe. Therefore, the application can be well controlled; and the placenta transfer is small. No fetal or maternal side-effect could be observed other than restricted undulatory amplitudes of the cardiotocogram. Midazolam itself does not exhibit analgetic action, but its application allows to break through the circulus vitiosus anxiety-tension-pain. This results in a reduction of breath depressing analgetics such as Pethidin.

[The incidence of congenital abnormalities following gestagen administration in early pregnancy (author's transl)].

198 women who had been trying for years to have a baby were submitted to a trial of prophylactic allyloestrenol therapy in an attempt to lower the incidence of spontaneous abortion. Allyloestrenol (Gestanon) was given at a dosage of 10 mg daily until the 8th week of pregnancy; the dosage was then stepped up to 15 to 20 mg daily until the 12th week of pregnancy and then rapidly tailed off. A decrease in the miscarriage rate was seen only in the group of women in whom treatment had been commenced immediately following ovulation (27 women: 1 miscarriage). If therapy was initiated only after the first missed period then it was without effect (161 women: 23 miscarriages). There were 4 cases of severe congenital abnormality; 3 of these cases arose in the group given allyloestrenol immediately after ovulation (with a drastic reduction in miscarriage rate). The results are discussed. They speak against a cytotoxic effect of the gestagen. It appears likely that the lowering in miscarriage rate occurs at the expense of a failure to prevent the nidation of abnormal fetuses who would normally be aborted.

[Monitoring of post-operative prevention of thrombosis by low dosage heparin with the radioactive iodine fibrinogen test (author's transl)].

In a prospective study low dosage subcutaneous heparin was administered to 148 patients after major gynaecological operations. The preventive effect on deep vein thrombosis was monitored by the radioactive iodine fibrinogen test. In 10.1% (15) of the cases higher than normal activity of the radioiodine fibrinogen test was detected. Only 4 patients (2.7%) had clinical signs of deep vein thrombosis. Because of the early occurrence of silent signs of deep vein thrombosis in the radioactive iodine fibrinogen test the onset of the subcutaneous heparin prophylaxis of thrombosis was shifted from the post-operative period to the pre-operative administration.

Progress in lymphadenectomy potential in radioisotope surgery of cervical carcinoma.

The introduction, in 1969, of radionuclide-assisted radical surgery using colloidal gold resulted in a substantial reduction of mortality from relapses with nodal involvement. While Wertheim's operation practiced between 1961 and 1965 was associated 50% recurrence mortality, mortality for all clinically operable stages dropped to 43.5% between 1966 and 1973, stage Ib mortality decreasing from 47.4% (1961-1965) to 26.9% (1966-1973). Within technetium labeling, which was introduced in 1977, the nodectomy potential was increased from an original 52.5% obtained with 198 colloidal gold and a scanner or eye probe to 88% obtained with the camera technique and technetium in 1979.

[Progress in radio-isotope surgery for cervical carcinoma (author's transl)].

The third phase in the use of radio-isotopes for radical surgery of cervical carcinoma, using 99m Technetium antimony sulphide to localise pelvic lymph nodes on the operating camera table has yielded results which indicate good progress. The potency of lymphadenectomy was increased from an original level of 52.5 per cent to 81.6 per cent. Recurrence mortality went down over the first three years after treatment, according to preliminary results.
