Pulsed ultrasonic doppler flow measurements both on uterine arcadic arteries and umbilical arteries have been done in 10 pregnant women suspected for intrauterine fetal growth retardation before, during and after Solcoserylinfusions for ten days. Improvement of maternal uteroplacental blood flow may be caused by changed rheologic blood properties as well as by physical inactivity. Fetal umbilical cord blood flow could not be influenced by Solcoseryl.

Intraoperative lymphscintigraphy during radical isotope surgery was performed in 242 patients suffering from cervical cancer and 170 patients with breast carcinoma. The results show a decreased rate of recurrence mortality in cervical cancer stage I and a decreased rate of recurrencies and metastases in breast carcinoma. In ovarian cancer patients radiommunoscintigraphy was introduced in the diagnostic procedure and is now performed intraoperatively. The results show a high positive prediction value of 93%. In 5 out of 28 patients radioimmunoscintigraphy identified tumor regrowth while all other methods failed.
19 patients with a known history of ovarian cancer were investigated by radioimmunoscintigraphy (RIS) to look for recurrent disease a few days before second-look surgery. The tumor-associated monoclonal antibody HMFG-2 (400 micrograms/patient) was injected intravenously after labeling with radioactive 123I (0.5-2.2 mCi/patient). Scans were reviewed for activity accumulations due to uptake of the tumor-associated antibody by tumor sites. In 15 out of the 19 cases the scan results correlated with the intraoperative findings. There were 2 false-positive and 2 false-negative scans, the latter in patients with subclinical disease. The smallest lesion detected by radioimmunoscintigraphy had a diameter of 1.5 cm. In 3 patients, tumor sites were identified that had been missed by all other routinely performed methods of investigation including transmission computed tomography. These data indicate that RIS is of considerable clinical value in the early detection and localization of recurrent ovarian cancer and may, therefore, improve the management of these patients.
The foetal outcome was examined in 24 pregnant females with EPH gestosis and a placental weight below the 20th percentile. The results show a high perinatal mortality in such cases. 11 newborns with a mean birth weight of 1092 g and a mean gestational age of 33.8 weeks died perinatally versus 13 newborns with a mean birth weight of 1705 g and a mean gestational age of 35.6 weeks, who survived. Perinatal mortality was equally high in cases with primary Caesarean section as in cases with spontaneous labour. Perinatological research should, therefore, focus on the development of techniques for estimating placental weight. Patients with a small placenta and EPH gestosis could then be monitored exactly and be delivered at the optimal moment.
Sixteen patients suffering from breast cancer were investigated by the method of immunolymphoscintigraphy (ILS) to evaluate the axillary lymph node status. Two monoclonal tumour associated antibodies (HMFG-1 and HMFG-2) that are known to react strongly with a wide range of epithelial malignomas were used as tumour seeking agents in our recent study. All patients were operated a few days after the scanning procedure including a radical axillary lymph node dissection. Therefore the scan reports could be correlated in each case with the histology of the surgically removed probes. While HMFG-1 failed to show correct results, HMFG-2 gave accurate results in 9 of 13 cases of questionable lymph node involvement. Among the 4 false results, one turned out to be false positive and three false negative. If better antibodies or their fragments, respectively, are used, we are convinced this noninvasive approach will greatly improve preoperative diagnosis of lymphatic involvement in breast cancer patients.
Solcogyn is a new acid preparation for the treatment of benign changes in the portio uteri. Histological and histochemical investigations of the treated cervix in patients scheduled for hysterectomy revealed a differential effect of this acid mixture in that ectocervical squamous epithelium was scarcely affected, while ectopic and endocervical cylindrical epithelium and the subepithelial stroma were devitalized by in vivo fixation. The maximum depth of penetration of the fixative effect was 2.5 mm. The treated cervical ectopies and transformation zones re-epithelialized within 3-4 weeks. The comparatively slight but sufficiently deep devitalization of affected tissue explains the clinical advantages observed with this preparation as compared to thermocoagulation and cryotherapy.
In 25 pregnant females with a placental weight below the 20th percentile uteroplacental blood flow measurements using 113mIn-transferrin were performed in the 3rd trimester of pregnancy. Twelve patients with pregnancy-induced hypertension (PIH) were compared to 13 females with normal blood pressures. The results of uteroplacental blood flow measurements showed normal flow patterns in only 6 of the 25 subjects and were significantly worse in the group with PIH. Babies of patients with PIH had a mean birth weight of 1,100 g at a mean gestational age of 34.2 weeks versus 1,752 g and 37.4 weeks for newborns without PIH. Perinatal mortality was significantly higher in the PIH group. In combination with a small placenta PIH compounds the fetal risk.
In 20 healthy pregnant women placental blood pool was estimated by means of placental scintigrams before, during and after infusion of either prostacyclin (PGI2) or placebo for 10 min to study the effect of PGI2 on the local regulation of uterine blood flow. There was no difference in the mean number of counts between the PGI2- and the placebo-treated group. Therefore no effect of PGI2 on uteroplacental blood pool could be detected in healthy gravidae.
In 19 late pregnant women with suspected placental insufficiency uteroplacental perfusion was measured and then the effect of a shortlasting infusion of aminophyllin on uteroplacental blood pool was examined. The results show evidence that in normal perfusion typ uteroplacental blood flow is not influenced by aminophyllin. But in pathologic perfusion typs there are two groups after aminophyllin. In the first one there is an increase of activity measured over the placenta (positive test). In these cases the cause of the lowered perfusion may be a functional one, treatment will be successful. In the second group there was no change or a decreasing activity following administration of aminophyllin (negative test). In these cases the reduced perfusion may be caused by morphological factors. Treatment will be without success with a high likelihood. The aminophyllintest following the isotope perfusion measurement is useful both for prognosis of newborn weight and appropriateness of therapy.
Twenty-five patients, having either unilateral ovarian tumors of unknown etiology or suspected of having ovarian cancer recurrence were investigated by the method of immunoscintigraphy to rule out primary and/or metastatic tumor sites. Four-hundred micrograms of the tumor-associated monoclonal mouse antibody HMFG-2, raised against human milk fat globulin membranes and labeled with 123I, were used for each patient to display the tumor sites by external scintigraphy. The dose ranged between 0.5 and 2.2 mCi, the specific activity between 1.25 and 5.5 mCi per mg of antibody. Nineteen of the patients underwent operations a few days after immunoscintigraphy. The remaining six patients were investigated by transmission computed tomography (TCT) to establish the presence or absence of tumor of the imaging. In 22 of the 25 cases the scintigraphic results correlated with the situation found at the subsequent operation, or by TCT, respectively, as well as with the histological diagnosis of the tumor type. Overall, there were just two false-negative and one false-positive scan report, the latter due to faulty reading of the scintigrams. Sixteen out of 18 tumor sites in 25 patients could be revealed by immunoscintigraphy, the smallest one being 1.5 cm in diam. In four of the patients immunoscintigraphy was the only noninvasive investigation method that could reveal the malignant tumor sites prior to the operation.
Since 1979 diagnosis, operative therapy and postoperative treatment and follow-up is performed at the Ist University-Clinic of Obstetrics and Gynecology in Vienna. This management brought a close cooperation with radiologists and other oncotherapists. A standard operation method, in our case a modified radical mastectomy with a complete dissection of the axilla after radioisotope marking of the axillary lymph nodes enables us to perform a randomised prospective study of adjuvant therapy. Though the observation time is relatively short, first results show, that metastases or local recurrences were observed only in the non-treated control group. That leads in our opinion to the cautious conclusion, that any form of adjuvant therapy is of therapeutic value. Which form of adjuvant therapy should be qualified best cannot be decided yet because of the too short follow-up period.
The production of monoclonal antibodies by the hybrid technique introduced by Köhler and Milstein in 1975, led to new dimensions in the diagnosis and therapy of malignant diseases. Labelling such monoclonal antibodies, directed against tumour cells, by a radioactive isotope, makes them usable for visualising malignant tumours and their metastases. Early detection of cancer may represent a major step forward in cases where early detection is not possible with common noninvasive methods, such as ovarian cancer in gynaecological oncology. Radioimmunoscintigrams (RIS) were performed in seven patients, all of whom were suspect of having ovarian cancer. The RIS-results showed very good correlations with the operation reports. According to these first results we assume that the RIS method using I123-labelled monoclonal antibodies directed against tumour cells may be successful in the early detection of ovarian cancer. Other important information supplied by radioimmunoscanning is determination of tumour status, localisation, extent or metastatic spread.
In 22 pregnant women beta-ELIR and N-terminal-beta-Lipotropin serum levels were determined by radioimmunoassay (New England Nuclear) in the last three weeks of pregnancy, at the beginning and at the end of delivery and during 48 hours after delivery. The difference between the results of the two examinations (beta-ELIR and N-terminal-beta-lipotropin) represents with close approximation the beta-endorphin concentration. In 12 patients the beta-endorphin level during delivery was significantly higher than during late pregnancy; in three cases an extremely high beta-endorphin concentration was found in pregnant women with a dystrophic infant. Therefore, beta-endorphin is discussed as an indicator for foetal distress.
The effects of nicotine consumption on uteroplacental blood flow were studied in 20 pregnant women by means of placental blood flow measurements using indium-113m-transferrin. Smokers were found to have a higher rate of poor perfusion patterns than subjects of a control group. In addition, the number of cigarettes smoked daily was significantly correlated with the placental perfusion type. The mean birth weight of infants born by smokers was 250 g lower than in the control group. The results suggest that smoking during pregnancy may compromise uteroplacental blood flow and thus result in poor fetal development.
