Effects of anti-inflammatory drugs on triple vaccine-induced pleurisy in rats.

Effects of various anti-inflammatory drugs on triple vaccine-induced pleurisy, a model of delayed hypersensitivity, were examined and compared with those on carrageenin-induced pleurisy in rats. Steroidal drugs depressed markedly the volume of exudate and the number of leucocytes in both types of pleurisy. Gold compounds also depressed both types of pleurisy. Non-steroidal anti-inflammatory drugs were apt to show depressive effects on carrageenin-induced pleurisy, especially on increased exudate volume. BW755C produced a depressive effect on carrageenin-induced pleurisy, but on triple vaccine-induced pleurisy, BW755C produced only a slight depressive effect. Cyproheptadine produced a slight depressive effect on carrageenin-induced pleurisy, but not on triple vaccine-induced pleurisy. Promethazine had a slight depressive effect on both types of pleurisy. D-penicillamine and levamisole did not show any depressive effects on triple vaccine-induced pleurisy. The results show that reported mediators in carrageenin-induced pleurisy (prostaglandin, serotonin, leukotriene B, etc.) are not relevant to triple vaccine-induced pleurisy. Specific lymphokines and/or degradated products of complement may participate in the latter. This triple vaccine-induced pleurisy seems to be a good model for screening non-steroidal anti-inflammatory drugs which have steroidal-like activity.

[Radiotherapy combined with pepleomycin administration for the treatment of esophageal cancer].

A combined therapy of pepleomycin (NK-613) and radiation was performed in 15 cases of esophageal and cancer. Twelve cases out of 15 were inoperable, and 3 cases were operable. NK-631 was administered by drip intravenous injection at a dose of 5 mg per day for 3 consecutive days weekly, aiming at total dose of 60-120 mg. Tumor regression rates, which were measured by planimeter on esophagogram, were 42-92% (mean 72%): two cases were more than 90%, and more than 50% in 12 cases. An average of the survival period of 15 cases was 57 weeks with 7 cases (46.7%) of 1 year survival, 2 cases (13.3%) of 2 year survival. The side effects of NK-631 observed in the present study consisted of fever 6, stomatitis 2, skin rash 2, and reversible pneumonitis 2. This study suggests that NK-631 exhibit remarkable anti-tumor effects on esophageal carcinoma, and seem to be less toxic.

[Clinical experience with ceftizoxime in the pediatric field (author's transl)].

Ceftizoxime (CZX) was given intravenously to 28 children with the following acute bacterial infections; 15 cases of bronchopneumonia, 4 cases of urinary tract infection, 3 cases of purulent meningitis, 2 cases of acute tonsillitis, each 1 case of purulent cervical lymphadenitis and acute tonsillitis, orbital cellulitis, abscess of eyelid, and peritonitis. Out of 28 patients, good clinical responses were obtained in 26 patients, and bacteriological effectiveness was seen in all 14 cases. Side effects with CZX were observed in 2 cases; 1 case was drug fever, and the other case was eosinophilia. From the above clinical results, CZX is useful antibiotic for treating the pediatric patients with various kinds of bacterial infections.

Electron microscopy. Ultrastructural observations of the extracellular sheath of dermal melanocytes in the nevus of Ota.

Under the electron microscope, we studied dermal melanocytes in lesions of six cases of nevus of Ota and also those in common blue nevus and in nevus caeruleus tardus associated with progressive systemic sclerosis. An extracellular sheath surrounded most of the dermal melanocytes in nevus of Ota in four patients ages 21, 22, 25, and 30, but not those in two younger patients ages 9 months and 10 years. Some dermal melanocytes possessed extracellular sheath as well as basal laminae. Some possessed only an extracellular sheath. The thickness of the extracellular sheath varied among dermal melanocytes. Not all dermal melanocytes possessed extracellular sheaths. In particular, only a small number of dermal melanocytes in the two younger patients possessed extracellular sheaths and these were thinner than those of the three older patients. Fibrous long-spacing collagen was recognized in some parts of the extracellular sheaths of dermal melanocytes. In contrast, dermal melanocytes in common blue nevus were not surrounded by either extracellular sheaths or basal laminae. Dermal melanocytes in nevus caeruleus tardus in patients with progressive systemic sclerosis were surrounded by thin extracellular sheaths, but not by basal laminae. The extracellular sheath is composed of fine filaments, 20-40 A in diameter, and appears to be derived from collagen.

[Evaluation of left ventricular morphology and function by cardiac computed tomographic examination].

Usefulness of cardiac computed tomography (CT) in the evaluation of left ventricular (LV) morphology and function was studied in clinical practice. One hundred and forty-nine adult cases of various heart diseases were examined by GE scanner with ungated scans covering whole LV and ECG-gated scans. In gating examination, "long-axial" and "short-axial" slices were scanned and end-diastolic area (EDA), end-systolic area (ESA) and "mean" area (MA) were obtained in each slices. 1) With regard to LV morphology, wall thickening in HCM and wall thinning and mural thrombus in myocardial infarction were easily visualized. 2) LV "mean" volume (LVMV) was easily calculated from the sum of sliced LV volume of ungated scans. The LVMV was well correlated with end-diastolic volume (EDV) obtained by LV angiocardiography (LVG) (r = 0.91), though the former was somewhat smaller than the latter. EDV by CT obtained after the correction with MA/EDA from gating examination showed a better correlation (r = 0.95) and both EDV showed almost the same value. 3) LV ejection fraction (EF) was calculated by exchanging ESA/EDA to ESV/EDV in "long-axial" and "short-axial" gated slices, and "long-axial" EF, "short-axial" EF and mean of both were well correlated with LVEF of LVG (r = 0.73-0.79). 4) LVEF calculated from "long-axial" EDA and ESA by application of "area-length" method was also correlated with EF of LVG (r = 0.68), but these EDV and ESV were smaller than those of LVG due to shorter long-diameter of LV in CT. 5) LV muscle volume was calculated from sum of sliced muscle volumes of ungated scans covering LV, and LV mass was easily evaluated from LV muscle volume and specific gravity of LV muscle. This LV mass was well correlated with that of LVG (r = 0.90) and the absolute values were almost the same.

Internal irradiation for cystic craniopharyngioma.

The authors report the results of internal irradiation with labeled chromic phosphate (32P) and gold-198 (198Au) colloid in eight cases of cystic craniopharyngiomas. They used a newly developed dosimetric formula, by which the radiation dose at the cyst wall and at any point far from the radioactive source can be calculated. Ten courses of irradiation in eight patients were carried out by injection of either 32P or 198Au colloid into the cyst through an Ommaya drainage system that had been placed at craniotomy. Follow-up studies ranging from 13 to 156 months revealed that all cysts were effectively treated, with elimination of fluid or collapse of the cyst. This was confirmed by Conray cystography and/or computerized tomography. Not only the dose delivered to the wall but also the thickness of the cyst wall and the location of the cyst are important factors in planning internal irradiation. A safe and adequate dose to the cyst wall could range between 9000 to 30,000 rads for craniopharyngioma. This treatment is suitable for large cysts that are thought to be difficult to remove radically, recurrent cysts resistant to previous treatment, or multiple cysts. Internal irradiation may also be applicable in other cystic intracranial tumors if dosimetry is calculated accurately.
