[Clinical experience with T-1982 (cefbuperazone) in the pediatric field].

T-1982 (cefbuperazone) was given intravenously to 21 children with the following acute bacterial infections; 13 cases of bronchopneumonia, 2 cases of urinary tract infection, 1 case of subcutaneous abscess, 1 case of cervical purulent lymphadenitis and acute tonsillitis, and 4 cases of acute bronchitis. Clinical effectiveness was obtained in 20 cases out of 21 cases and bacteriological effectiveness in 7 cases out of 8 cases. As for the side effects, 1 patient had asymptomatic eosinophilia, but no other laboratory abnormalities were observed. Above results suggest that T-1982 is a useful antibiotic for treating pediatric patients with various kinds of bacterial infections.

[Pharmacokinetics and a phase I study of tegafur-uracil enterogranules in cancer patients].

UFT-E, enterogranules of tegafur-uracil (at molar ratio of 1: 4) was developed in order to minimize GI toxicity of UFT. Pharmacokinetic study after single oral administration of 300 to 1200 mg of UFT-E was carried out in cancer patients measuring tagafur, uracil and 5-FU levels in serum, normal tissue and tumor tissue using HPLC and GC-mass. In the all doses studied, curves of serum tegafur after administration of UFT-E were consistently higher than those of uracil and 5-FU. Peak 5-FU levels were observed at 2-4 hours after administration of UFT-E. Maximum 5-FU levels ranged 0.1-3.0 mcg/ml and were variable in each patient even at the same doses. The serum 5-FU level was correlated with tegafur and uracil levels, especially with the latter. The curves of serum 5-FU of UFT-E were different from those of UFT, and the peak time of UFT-E occurred 2 or 3 hours later than that of UFT and the decrease of 5-FU level was slower. The concentrations of 5-FU in tumor tissue (T) were observed in most of the cases compared with those in normal tissue (N) (T/N ratio of more than 2.0 was observed in 16/23 patients), especially in the patients received higher doses of UFT-E and in the patients with GI cancers. In the phase I study of oral daily consecutive administration of UFT-E in 23 cancer patients, the GI side effects such as anorexia, nausea, vomiting and diarrhea, were observed in 5 out of 23 patientsr.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical experience with an ampicillin suppository (KS-R1) in pediatrics].

Ampicillin suppository (KS-R1) was given to 16 children with the following acute bacterial infection; 4 cases of acute pharyngitis, 2 cases of acute tonsillitis, 5 cases of acute bronchitis, 4 cases of bronchopneumonia, 1 case of group A beta-streptococcal infection. Out of 16 patients, good clinical responses were obtained in 16 patients, and bacteriological effectiveness was seen in all 3 cases. Side effects with KS-R1 were observed in 4 cases: 3 cases were perianal redness, and 4 cases were soft stool. From the above clinical results, KS-R1 is useful antibiotic suppository for treating the pediatric patients with various kinds of bacterial infections.

[Heart valve replacement with the Ionescu-Shiley bovine pericardial xenograft. Hemodynamic evaluation and early clinical follow-up of 170 patients].

The Ionescu-Shiley bovine pericardial xenograft (ISPX) has been widely accepted clinically because of its superior hemodynamic characteristics and low incidence of thrombogenicity. After confirmation by our own experiment of its superior valve performance, we have adopted in clinical application of this valve since August of 1979, and implanted 189 of this valve in 170 patients up to November of 1981. The overall mortality rate including of hospital and follow up death was 14.1%. The pressure gradient and calculated orifice area of ISPX were compared to those of Hancock xenograft (HX), and the valve characteristics of ISPXs was found to be better than those of HX. The cardiac function following implantation of this valve apparently showed improvement. No incidence of thromboembolic episode or malfunction was observed, however, prosthetic valve endocarditis (PVE) was encountered in four patients and two died. This incidence of PVE in ISPXs was not significantly higher in comparison to that in other type of prosthetic valve. However the low incidence of thromboembolic episode as well as acceptable valve performance were confirmed by our study, the durability and susceptibility to PVE has to be carefully observed in the longer follow up.
