Decrease of gastrointestinal mucosal damage by salicyluric acid compared with salicylic acid in rabbits.

The gastrointestinal mucosal damage following the oral administration of salicylic acid or salicyluric acid was examined in rabbits using a scanning electron microscope. Six and 24 h after treatment with salicylic acid, morphological changes of gastric mucosa were recognized. In rabbits treated with salicyluric acid, however, severe damage in the gastric mucosa was not found after 24 h compared with the treatment with salicylic acid. Following the treatment with salicylic acid, some mucosal damage in the duodenum, jejunum and ileum was observed after 24 h. The surface character of the duodenal, jejunal, ileal, caecal and colonic mucosa were almost identical compared with the control following the treatment with salicyluric acid. It was reported that salicyluric acid is metabolized to salicylic acid by the intestinal microorganisms. From these results, it was suggested that prodrugs utilizing the metabolism of salicyluric acid to salicylic acid by intestinal microorganisms may be useful in reducing gastrointestinal mucosal damage.

Survey of Hanganutziu and Deicher antibodies in operated patients.

The appearance of Hanganutziu and Deicher (HD) antibody in the sera of patients suffering from various diseases, including malignancies of some organs and liver disorders, was investigated by enzyme-linked immunosorbent assay using N-glycolylneuraminyl-lactosylceramide (HD3) and 4-O-acetyl-HD3 as the antigenic molecules. More than 25% of sera from patients suffering from malignancies, cholelithiasis and liver cirrhosis had HD antibody, whereas none of 41 sera from healthy persons had HD antibody. The percentage of HD antibody-positive patients was similar in stages I, II and III of gastric cancer and recurrence cases. Antibody titers of the positive patients in each stage were also not different from those in each other stage. These results indicated that HD antigenic expression on cancerous tissue is not dependent on the cancerous malignancy. The HD antibody level was elevated after surgical removal of cancerous tissues in 5 of 6 patients examined, indicating that tumor growth absorbed the serum antibody. Serum antibody against 4-O-acetyl-HD3 was detected independently of HD3 antibody in some cases; however, in most cases, correlation between the two antibody titers was observed.

Preparation of glycyrrhetic acid glycosides having various beta(1----2)-linked disaccharides and their cytoprotective effects on carbon tetrachloride-induced hepatic injury.

Glycyrrhetic acid glycosides (1-7) having beta(1----2)-linked disaccharides such as 2-O-beta-D-glucopyranosyl-beta-D-galactopyranose, 2-O-beta-D-galactopyranosyl-beta-D-galactopyranose, 2-O-beta-D-glucuronopyranosyl-beta-D-galactopyranose, 2-O-beta-D-glucopyranosyl-beta-D-glucuronopyranose, 2-O-beta-D-galactopyranosyl-beta-D-glucuronopyranose, 2-O-beta-D-galactopyranosyl-beta-D-glucopyranose, 2-O-beta-D-glucuronopyranosyl-beta-D-glucopyranose, respectively, were synthesized by stepwise construction; from glycyrrhetic acid monoglycosides to the diglycosides. The cytoprotective activities of the glycosides 1-7 and 2-O-(beta-D-glucopyranosyl)-beta-D-glucopyranosyl-11-oxoolean-12-e n-30-oate (8) were compared with natural occurring glycyrrhizin (9). Among these glycosides 1-8, glycosides 3 and 7 having beta-D-glucuronopyranose (glcUA) as the only terminal sugar component were more effective materials against hepatic injury than glycyrrhizin 9.

[Correlation between odor thresholds and saturated vapor pressures of odorants].

The purpose of our work is to develop an estimation method for odor thresholds on the basis of physicochemical properties of odorous compounds. In this report, we examined the correlation between odor thresholds (Cth) and the saturated vapor pressures (Pvp) of various odorants. Results. 1) There were very good correlations between Pvp and k. k represents an indicator of lipid affinity. In our earlier report, k was obtained by the analysis of the gas chromatography with a column packed with a support coated by a phospholipid. Accordingly the correlations show that Pvp may be an indicator of solubility into the lipid phase. 2) It was found that correlations between Pvp and Cth were very good in several homologous series of aliphatic compounds. However ionized substances, such as acids and amines, and sulfur compounds had no good correlations. 3) In odorants having more than 3-5 carbon atoms, odor thresholds of normal types were higher than those of iso types. 4) In the series of amines, the relationship between Cth and Pvp depended upon the length and the number of alkyl chains combined with the contained nitrogen atom. 5) In the series of alcohol and aldehyde, the decrease of Cth in the rate became smaller with the decrease of Pvp.

[Efficacy and safety of percutaneous endoscopic gastrostomy in the elderly].

Percutaneous endoscopic gastrostomy (PEG) was performed in 32 patients with nasogastric feeding (NGF) or total parenteral nutrition (TPN), who were unable to swallow. Our cases of PEG included 10 with dementia, 8 with cerebral infarction, 8 with cerebral bleeding, 3 with gastric cancer, and others. TPN was performed after PEG for a short time. Because of the combination of TPN and PEG, there was no mortality or major complication related to the procedure. Minor complications included subcutaneous abscess and TPN catheter fever. PEG can be safely and rapidly performed. Furthermore, painless life, better cosmetic features and physical condition can be obtained with this procedure. The majority of the patients are able to return home after PEG. PEG can improve the quality of life of the patients who cannot swallow but have an intact gut.

Epitope analysis of cluster 1 and NK cell-related monoclonal antibodies.

By flow cytometric assays, we tested the antibodies of the Second International Workshop on Small Cell Lung Cancer Antigens against 20 normal peripheral leukocytes, four small cell lung cancer (SCLC) cell lines (one classic type, and three variant type) and one gastric cancer line (KATO 3). Thirteen antibodies (Code # 4, 12, 21, 31, 34, 41, 48, 58, 60, 61, 74, 77, 82) among 98 registered antibodies showed a very similar pattern to antibody NE150, which was previously characterised as SCLC cluster 1. Since NE150 showed a positive reaction to the natural killer (NK) cell population, the serological specificity was compared with NK cell-associated antibodies, NKH1 (CD56), Leu7 (CD57) and Leu11 (CD16). Only NKH1 antibody showed a similar pattern to NE150, when tested against various target cells including SCLC lines and peripheral leukocytes, suggesting that NKH1 is a cluster 1 antibody, although it was already classified as CD56 of hematopoietic cells. By sequential immunoprecipitation, the antigen detected by NE150 antibody was depleted by preincubation with NKH1 antibody, but the reactivity of NE150 was not inhibited by NKH1 antibody, suggesting that NE150 and NKH1 detect different epitopes on the same antigen molecule. Epitope analysis was also conducted with 13 antibodies of cluster 1. Ten were found to detect the same epitope as NE150. The other three did not inhibit the binding of NE150 or NKH1, suggesting that there are at least three epitopes. Since the cluster 1 antibodies were demonstrated to detect NCAM, the present results suggest the presence of at least three epitopes on this molecule.

[Clinical evaluation of imipenem/cilastatin sodium as a second line regimen in severe infections associated with hematologic disorders].

Imipenem/cilastatin sodium (IPM/CS), which has a broad spectrum of activity against both Gram-positive and -negative bacteria including methicillin-resistant Staphylococcus aureus and Pseudomonas aeruginosa, was used as the second choice for severe infections associated with hematological disorders. Sixty-five patients were treated with IPM/CS. Among them, 53 patients were evaluable for the clinical efficacy. Twelve patients were not evaluable due to the following reasons: 5 patients were treated with combinations of other regimens such as cefzonam, cefmenoxime, ciprofloxacin or gamma-globulin, 5 were patients to whom IPM/CS was administered as the first choice, and the remaining 2 patients were thought to be suffering not from febrile infections but from febrile tumor. Excellent responses were observed in 10 (18.9%) patients and good responses in 23 (43.4%) patients, with an overall rate of efficacy of 62.3%. The efficacy in septic patients was 75% (3/4), and that in patients whose peripheral granulocytes were continuously below 100/microliter was also 75% (6/8). Two patients who suffered from tumor fever and 5 patients who had received no chemotherapy before IPM/CS administration were included in the final evaluation of side effects. Side effects were observed in 16 patients (16/60, 26.7%). In a 61 years, female patient, a skin eruption was found 4 days after IPM/CS therapy was started. In 15 patients, mild gastrointestinal symptoms such as nausea and vomiting were identified within a few days after IPM/CS treatment was started. Abnormal laboratory data such as eosinophilia, liver dysfunction or renal dysfunction were also identified in 4 patients (4/60, 6.7%). Degrees of these abnormalities were very slight, however, and the continuation of treatment was not disturbed. These results indicated that IPM/CS was an effective second line regimen of chemotherapy for the treatment of severe infections in patients with hematological disorders.

[Centrifugal pump as right ventricular assist pump in the treatment of right ventricular failure following resection of left atrial myxoma].

A 31-year-old woman had a left atrial myxoma associated with severe pulmonary hypertension and respiratory failure because of incarceration of the tumor to the mitral valve. Emergency surgery was performed, but the patient could not be weaned from cardiopulmonary bypass because of right ventricular failure. A Biomedicus centrifugal pump was used as a right ventricular assist pump. The result was a successful termination of cardiopulmonary bypass. With the support of an intra-aortic balloon pump, the right ventricular assist pump was removed 38 hours after the operation, and the intra-aortic balloon pump was terminated 2 days later. The centrifugal pump is very useful at the majority of centers where the pneumatically activated bi-valved ventricular assist device is unavailable. The Biomedicus centrifugal pump can be easily applied for treatment of perioperative right ventricular failure and is very useful for short-term use without systemic anticoagulants.

Reciprocal t(14;19)(q32.3;q13.1) in a patient with B-cell lymphoma.

A patient with B-cell lymphoma with a chromosome rearrangement of t(14;19)(q32.3;q13.1) is reported. This patient had leukemic features and an aggressive clinical course. The histopathologic diagnosis was malignant lymphoma, small noncleaved cell. Chromosome analysis of the cells from a cervical lymph node and peripheral blood showed a reciprocal translocation between chromosome 14 with a break at band q32.3 and chromosome 19 with a break at band q13.1, to which the bcl-3 gene has been mapped. Monoclonal rearrangement of the JH gene was detected by Southern blot analysis. However, we could not detect rearrangement of the bcl-3 gene. This case also had a t(2;8)(q13;q24.1), but the c-myc gene remained in its germline. This is the first case with the reciprocal t(14;19) and 8q24 chromosomal breakpoint in a B-cell lymphoid malignancy.

Arterial injury-induced smooth muscle cell proliferation in rats is accompanied by increase in polyamine synthesis and level.

Proliferation of smooth muscle cells (SMC), enhancement of polyamine biosynthesis and increase in polyamine level in response to deendothelialization in the rat aorta were studied. [3H]Thymidine incorporation into SMC in aortas denuded with a balloon catheter began 25 h after injury, and maximal incorporation occurred 33-37 h after injury. Afterwards, [3H]thymidine incorporation declined, approaching the baseline level, but was slightly higher than that of sham-operated controls until 14 days after injury. Intimal thickening started 7 days after injury, and peaked at 21 days. Prior to these proliferative changes in aortic SMC, a rapid and transient increase in ornithine decarboxylase (ODC) activity was observed within 8 h after injury. There was no significant difference in ODC activity between injured and intact aortas after 4 days. The levels of polyamines, putrescine, spermidine, and spermine increased and were maximal at 48 h after injury, 8.1, 3.4 and 1.4 times the control levels, respectively. Increased levels of polyamines, in particular spermidine, continued until 7 days after injury. These results suggest that the enhancement of polyamine synthesis and the increased polyamine content of the aorta play important roles in the proliferation of SMC and in the development of intimal thickening, particularly in the initial proliferative response of medial SMC after deendothelialization.

Long arm deletion of chromosome 7 unrelated to original karyotype in recurrent t(8;21) acute myeloblastic leukemia.

We describe a chromosomal abnormality, 7q-, unrelated to the original karyotype in a patient with recurrent t(8;21) acute myeloblastic leukemia. The 7q-, del(7)(q22q34), was seen in otherwise karyotypically normal cells. Cyclophosphamide was administered for 5 months during the maintenance therapy. Our observations indicate that unrelated karyotypes, besides karyotypic evolution, may be implicated in tumor cell heterogeneity and may support the previous documentation of a possibly causal relationship between chemotherapy and development of 7q-. A larger study will be required to elucidate the biologic significance, if any, of the unrelated karyotypes.

Biliary excretion of mitomycin C dextran conjugates in relation to physicochemical characteristics of carrier dextran.

Biliary excretion characteristics of polymeric prodrugs of mitomycin C (MMC), mitomycin C-dextran conjugates with cationic or anionic charge (MMC-Dcat, MMC-Dan) and with different molecular weights of dextran (10,000, 70,000, and 500,000) were studied in rat. Following intravenous injection, bile was periodically collected and concentrations of free and dextran-conjugated MMC in it were determined by bioassay. MMC administered as a free form was excreted rapidly into bile and 1.8% of the dose was recovered within 2 h. A small amount of MMC was gradually excreted into bile after administration of all MMC-Ds and total recovery at 8 h was less than 1% of dose. In this case, a major part of excreted MMC was recovered as a conjugated form. MMC-Dcat gave a larger total excretion of MMC than MMC-Dan and excretion was also affected by the molecular weight of carrier dextran. The biliary recovery of MMC-Dcat labeled with 14C at a spacer moiety was significantly higher than that of conjugated MMC determined by bioassay suggesting release and/or inactivation of MMC in MMC-D during the circulation in the body. These results were compared with biliary excretion of model macromolecules with the same molecular weight but different electric charge in order to clarify the effect of electric charge on the biliary excretion of macromolecules. Cationic macromolecules exhibited higher biliary excretion in relation to greater hepatic uptake.

[Sensory scaling of odor based on mathematical studies of rating words].

Sensory scales are very important indicators for the evaluation of environmental odors, and of the effect of deodorizers. The purpose of this study is to examine reasonable methods of sensory scaling. For this purpose, the psychological values of rating words for odor intensity, odor hedonics and similarity were measured. The interval distances between categories of each scale were estimated in the method of successive categories as well. Results of this study are as follows. 1) The 6-step odor intensity scale, issued by the Environmental Agency, may be regarded as an ordinal scale, but not as an interval scale. The 9-step hedonics scale may be regarded as an interval scale. 2) Psychological values of rating words are not affected by the size and the number of category steps in the graphic scales applied to the judgements of their values. 3) The number of category steps needs to be determined based on the distribution of the psychological values. The meanings and the order of the rating words have to be clear. 4) In the construction of a numerical scale, the category values should accord nearly as possible with the psychological values of rating words. 5) In results from the method of successive categories applied to the measured values of odor intensity and similarity, interval distances between categories are unequal, and the interval distances are wide on both edges of the scale. In these cases, the values must be converted into "category values of stimuli."

Cauda equina neurinoma associated with normal pressure hydrocephalus--case report.

A 45-year-old male was hospitalized because of gait disturbance, urinary incontinence, and memory disturbance. A computed tomographic (CT) scan revealed dilatation of the cerebral ventricles, and a metrizamide CT scan showed reflux into all ventricles. Cerebrospinal fluid obtained at the time of metrizamide CT was highly xanthochromic. Myelograms demonstrated a tumor between the inferior portion of the fifth lumbar vertebra and that of the second sacral vertebra. After removal of a spinal tumor involving nerve fibers at the cauda equina, his symptoms disappeared and the spinal fluid protein normalized. Three months after surgery, a CT scan revealed reduction of cerebral ventricular size. There are 10 reported cases of spinal tumors resulting in normal pressure hydrocephalus. All these and present cases involved protein in the spinal fluid, which might have been the cause of the hydrocephalus.

[Cognitive process and the five types of solution at contradictory situation consisting of two propositions].

The purpose of this paper was to examine the cognitive process and solution at contradictory situation consisting of two propositions. In the first investigation, subjects (N = 50) were asked to rate one hundred contradictory situations in terms of three cognitive aspects: contradictoriness, solvability, ordinariness. In the second investigation, five types of solution were proposed by combination of negative-positive orientation and past-future orientation, and subjects (N = 149) were asked to assign the solutions obtained in the pre-investigation to one of following five types, that is, past-oriented negative, past-oriented positive 1, 2, future-oriented negative, future-oriented positive. The obtained results suggested the plausibility of five types of solution, and the cognitive process in contradictory situation was discussed.

[Significance of measuring Cu, Zn-superoxide dismutase (SOD) levels in open heart surgery].

Cu, Zn-SOD levels, free hemoglobin (Hb) and CPK-MB were measured in 18 patients undergoing open heart surgery. In order to evaluate the serum level of Cu, Zn-SOD, the ratio of Cu, Zn-SOD/free Hb was used to eliminate an influx of Cu, Zn-SOD released from erythrocytes during cardiopulmonary bypass. The value of Cu, Zn-SOD/free Hb was decreased significantly 30 minutes after aortic declamping and the minimal value was indicated 1 hour after the termination of cardiopulmonary bypass compared with the preoperative status. We thought that the decrease was due to the consumption of SOD competing against free radical generation. The correlation between the minimal value of Cu, Zn-SOD/free Hb and the ischemic time was apparent but the correlation between the minimal value of Cu, Zn-SOD/free Hb and the value of peak CPK-MB was not determined. It was suggested that the serum free radicals were not only derived from the heart but also from the lung or other parts of the body during open heart surgery.

[Acute myelogenous leukemia (M2) simultaneously associated with multiple myeloma with special reference to chromosome abnormality of t(6; 14) (p21.1; q32.3)].

A 73-year-old male was admitted to our hospital in October 1987 because of severe anemia, anorexia, and loss of weight. The hemoglobin level was 5.7 g/dl, the white blood cell count 2,500/microliters with 5% myeloblasts positive for peroxidase, and the platelet count 8.6 x 10(4)/microliters. The LDH was 656 mU/ml, the total protein in the serum 7.4 g/dl, IgG 419 mg/dl, IgA 104 mg/dl, IgM 10 mg/dl, and urine Bence Jones (BJ) protein 8.8 g/day. The X-ray survey of the bones showed multiple osteolytic lesions. A bone marrow aspirate was hypercellular with 91.4% plasma cells, and was cultured a whole day for chromosome study. It revealed an abnormal karyotype of 46, XY, -15, t(6; 14) (p21.1; q32.3), +der(15)t(1; 15) (q23; q24). Immunoelectrophoresis demonstrated lambda type BJ protein. He was treated with melphalan and prednisolone. Proteinuria and marrow plasma cells decreased in amount. In December a white cell count was 6,030/microliters with 80% myeloblasts. A bone marrow aspirate revealed an increase of 82.6% myeloblasts or promyelocytes. The patient was refractory to chemotherapy and died of sepsis in April 1988. An unrelated abnormal karyotype; 48, XY, +8, +13 appeared concomitant with an increase of the leukemic cells, but no cells showed the t(6; 14). We cytogenetically discussed the simultaneous presence of multiple myeloma with acute myelogenous leukemia.
