[Reoperation of recurrent gastric cancer--a comparative study of a resected and nonresected group].

A retrospective study was made on 51 patients with recurrent gastric cancer following partial or subtotal gastrectomies during the past 17 years. Thirteen patients (25.5%) underwent resection of recurrent lesions, of which 12 were total resection of gastric remnant with combined removal of the distal pancreas, spleen, transverse colon and/or a part of the liver. The operative mortality rate was only 7.6%, and the one-year survival rate was 41.7%. There is a necessity for aggressive surgery in cases proved histologically to be stage I or II after the initial operation. The surgical procedure is not effective for stage III and IV patients, except for cases of local recurrence.

Detection and clinical significance of IgE rheumatoid factor.

High serum IgE rheumatoid factor (RF) levels (greater than 100 ng/ml) were found using solid phase sandwich ELISA in 11 of 20 patients with rheumatoid arthritis having significant extraarticular symptoms (rheumatoid vasculitis) and only in 23 of 142 usual rheumatoid patients without such symptoms (p less than 0.01). A significant influence of other classes of RF and non-RF IgE on the ELISA for IgE RF was not observed in sera with levels of IgE RF over 50 ng/ml. The levels of IgE RF were not closely correlated with those of other classes of RF and total IgE. This study suggests that IgE RF might be one of the factors necessary to produce extraarticular rheumatoid vasculitis.

[Prediction of Ovulation and timing of AIH].

The relation among ultrasonographic follicular diameter, serum estradiol (E2), luteinizing hormone (LH), cervical mucus changes, and basal body temperature (BBT) was studied. One hundred and eight cycles were examined in forty-two infertile patients. The last low day of the BBT curve was taken as day zero. In 83 of the 85 cycles, 158 of 163 times, follicles could be observed by ultrasonography. Follicular rupture was confirmed in 12 cycles. The mean diameter of the follicles measured the day before estimated ovulation was 24.00 +/- 3.10mm (mean +/- S.D.). Serum E2 levels were more than 400pg /ml from day -2 to day -1 and the peak serum LH levels occurred on day 0. The volume of cervical mucus (C.M.) was more than 100mm3 from day -2 to day +1. The volume was decreased in clomid stimulated cycles and the C.M. displayed a 3+ ferning 23/28 cycles on day 0. Among 5 pregnancies induced by homologous artificial insemination (AIH), three were done on day -2, one on day +1 and one on day +2. The best time to do AIH is when serum E2 is more than 400pg /ml, serum LH is more than 60m IU/ml, cervical mucus is 3+ ferning , and the follicular diameter is more than 20mm.

Studies on the Indonesian Antiaris toxicaria sap.

The present research is on a milky sap obtained from the Antiaris toxicaria tree (Moraceae) which is called Upas or Ipoh in Indonesia. The crude sap was administered to anesthetized rats, and changes in electrocardiogram (ECG) and systemic blood pressure was observed. Biologically and pharmacologically active components were extracted from the crude sap by means of water-acetone solution. Based on the strength of chemical qualitative detection tests of the sap extract (SE), cardiac glycosides are supposed to be the main components. The SE inhibited the Na+-, K+-ATPase (EC 3.6.1.3) which was partially purified from guinea pig heart muscle. When the SE and, concurrently, authentic ouabain were applied to isolated frog heart muscles, the fall of twitch tension was observed after the increased tension on mechanograms. These facts suggest that the main components of the milky sap are cardiac glycosides, and glycosides affect Na+, K+-ATPase activity of muscle membrane and heart muscle contraction.

Isolation of the ST Ent plasmid specific bacteriophage M124.

For simple detection of enterotoxigenic E. coli from patients, attempts were made to find a bacteriophage that specifically proliferate on these bacteria. As a result phage M124 was isolated. Analysis of plasmid DNA together with detection of ST enterotoxin showed that this phage can specifically attack E. coli carrying the ST Ent plasmid (80 Mdalton). These results indicate that phage M124 is very useful for detection of ST enterotoxigenic bacteria.

[Histopathological evaluation of minute stomach cancer].

We histologically examined 17 lesions from minute gastric cancer (less than 5 mm in diameter). Flat or IIb type was frequently seen in minute cancer (47%); the depressed type was predominant in ordinary early gastric cancer (72%). Differentiated adenocarcinoma was noted more often in minute cancer (88%) than in ordinary early gastric cancer (61%) or advanced cancer (45%). Differentiated type minute cancer was frequently associated with severe atrophic gastritis (93%) and intestinal metaplasia (73%). There was no evidence for the preexistence of chronic peptic ulcer, because no fibrosis was present in the gastric wall beneath the minute cancerous lesions.

[Statistical observation of the effect of maternal heart disease on pregnancy and fetus].

Fifty-four pregnancies with maternal heart disease, managed at the Tohoku University Hospital during the years 1976 to 1981, were reviewed and statistically analyzed for the effects of the disease. Twenty-six (48.1%) were complicated by rheumatic valvular disease, 26(48.1%) by congenital disease, and the remaining 2(3.7%) were miscellaneous. Forty-four cases who delivered after 24 weeks gestation were divided into two groups in terms of the occurrence of maternal decompensation during pregnancy. In the group of pregnancies with decompensation, when compared with the group without decompensation, the age of mothers was higher (p less than 0.02), length of gestation was shorter (p less than 0.005) and birth weight was smaller (p less than 0.01). On the other hand there were no significant differences between the group without decompensation and the control group. Analysis of covariance among the three groups, in which birth weight was assumed to be a function of the number of gestational weeks, suggested the shortening of gestation was relatively important in the occurrence of low-birth-weight in the group with decompensation. Apgar scores at 1 and 5 minutes and the occurrence of fetal distress were not significantly different among the groups. The effects of maternal decompensation on fetal growth and length of gestation were discussed.

[Classification of human oocyte maturity].

Maturity of human oocytes was classified into five grades according to the appearance of the oocyte and the cumulus oophorus (Grade I-V). The relation between this "Grade" of oocyte and the state of fertilization was investigated and the following results were obtained. The proportion of fertilized eggs cleaving in vitro was higher when oocytes graded I or II were collected than when oocytes graded III, IV or V were obtained (76% vs. 22%). When oocytes were aspirated from preovulatory follicles 26 hours after the onset of the luteinizing hormone (LH) surge 64% of them were Grade I or II. When oocytes were collected 36 hours after an injection of human chorionic gonadotropin (HCG), 88% of them were Grade I or II. Therefore it seems that the times chosen for oocyte recovery were appropriate. There was no correlation between the "Grade" of oocyte and follicle diameter or volume of follicle fluid.

[Sequential observation of human sperm penetration into zona pellucida-free hamster oocytes by scanning electron microscopy].

Yanagimachi et al. have shown that zona pellucida-free hamster oocytes can be penetrated by capacitated and acrosome-reacted human spermatozoa, and both pronuclei be formed. Recently, it has been said that this technique may be used to test the fertilizing ability of human sperm for clinical use. We have made scanning electron microscopic studies of the interaction of these heterologous gametes, especially the mechanisms of sperm entry into the oocytes. Our results are as follows: The surface of the oocyte is covered with numerous microvilli, which are found to be evenly and densely distributed on the vitellus surface. The surface of the first polar body already released is smooth and microvilli-free. Most spermatozoa lie flat on the vitellus surface, but a few are oriented perpendicularly to the vitellus surface. Most bound sperm had lost their acrosomal caps, because a ridge exists at the leading edge of the equatorial segment. Many microvilli are shown to participate in sperm-egg contact. Initially most microvilli appeared to grasp and immobilize the anterior tip of the sperm head. But as gamete interaction proceeded, microvilli were overlying the postacrosomal region, and observed adjacent to the plasma membrane of the postacrosomal region. The postacrosomal region is first incorporated into the ooplasma, the anterior tip of sperm head being the last portion to be incorporated. The microvilli of the oolenmal surface where sperm penetrated did not show major changes in size or appearance, and the so-called "incorporation cone" was not observed. Our scanning electron micrographs show that the microvillar portion of the oolenma greatly participates in this heterologous gamate interaction.
