Two cases of transient acantholytic dermatosis were reported. The first case was a 78-year-old male and showed pruritic papulovesicular lesions on the face, head, neck and back. Darier-like pattern was observed histologically. Oral administration of etretinate (40 mg/day) was effective. The second case was a 57-year-old male showed asymptomatic, herpetiform confluent vesiculo-papules on the right forearm. Darier-like pattern was dominantly observed with a focal spongiotic acantholysis histologically. Also 20 cases (10 males and 10 females) reported in Japan were collected and analyzed.
We evaluated 10 patients with verified brachial plexus injuries and root avulsion. In 100%, dermatomal somatosensory evoked potentials (DSSEPs) were abnormal. Compound muscle action potentials (CMAPs) by magnetic stimulation revealed 90% abnormal findings on the affected side, but also revealed abnormality in adjacent segments. Dissociation of CMAPs and DSSEPs revealed the apparent continuity of motor and sensory nerves. The use of both techniques for the examination of the function of proximal peripheral nerve revealed increased latencies over the motor and/or sensory pathways in all patients. The technique of non-invasive stimulation of the motor pathway therefore provides an additional tools to detect and quantify subclinical and clinically apparent lesions in patients with defined brachial plexus injuries and root avulsions.
In conclusion, it appears that the endothelium-derived vasoactive substances may have important physiological roles to play in the fetoplacental circulation. They may also play an important role in the pathogenesis of some disease states. The complexity of the umbilical and placental vessels as well as the complexity of the placental vascular bed can slow research progress in this area. It is also important to realize that no single mediator may be able to explain a physiological process, and research workers in this area need to consider possible interactions between the different endothelium-derived vasoactive substances in any physiological or pathological phenomenon.
The afsR gene of Streptomyces coelicolor A3(2) complements afsB mutations affecting production of pigmented antibiotics. It also directs pigment production in Streptomyces lividans when carried on a plasmid vector. Nucleotide sequencing of the afsR gene revealed that it codes for a 993-amino acid protein (Mr 105,600) with A- and B-type ATP-binding consensus sequences at its N-terminal portion and two DNA-binding consensus sequences with a helix-turn-helix motif at its C-terminal portion. Each of the N- and C-terminal halves was capable of conferring pigment production, to some extent, in S. lividans, when carried separately on a multicopy plasmid. In addition, expression in trans of the two regions on the same plasmid conferred pigment production to almost the same extent as did the intact afsR gene. Mutations at the two ATP-binding consensus sequences, that were generated by in vitro site-directed mutagenesis, revealed their functional importance. Disruption of the S. coelicolor A3(2) chromosomal afsR gene in either the N- or C-terminal half using phage phi C31 KC515 resulted in significant, but not complete, loss of pigment production. These data suggest that the AfsR protein comprises two domains, viz., an ATP-binding and a DNA-binding domain, each of which could function as a positive regulator for pigment production. These afsR mutants sporulate normally. In addition to an internal promoter, which we previously detected in the middle of the AfsR coding region, S1 nuclease mapping revealed two tandem transcriptional start points, separated by 64 bp, upstream from a putative ATG start codon of the AfsR product.
The oxytocin gene is maximally expressed in the cells of the early bovine corpus luteum (1-5 days post-ovulation) and provides an excellent marker for luteinization, having been up-regulated in vivo at ovulation. However, it is down-regulated again later in the luteal phase. To help understand the mechanisms involved in regulating this gene, and hence differentiation in the early bovine corpus luteum, oxytocin secretion into the medium as well as oxytocin mRNA were measured in serum-free cultures of early luteal cells in the presence or absence of various effectors. Insulin-like growth factor I (IGF-I) deferred the endogenous down-regulation of the gene and hence increased oxytocin peptide secretion in the first days of culture. Prostaglandin F2 alpha had no influence on oxytocin mRNA levels but reduced the stimulatory effect of IGF-I on peptide secretion, indicating an effect at the post-transcriptional level. Oestradiol had no effect either on oxytocin mRNA levels or on oxytocin secretion.

A method of continuous image subtraction of fura-2 fluorescence made it possible to observe real-time (video rate) changes in intracellular calcium (Cai). This simple method is very useful for simultaneous measurements in electrophysiology with a system containing cells of different states. A method for synchronous recording of Cai change and single channel activity is also described. In cultured mammary epithelial cells, these methods revealed a propagating Cai signal induced by mechanical stimulation and spontaneous Cai oscillation with synchronous activation of calcium-activated potassium channels.
The effect of various inspired oxygen concentrations on protein metabolism after surgery was investigated in rats. Surgical injury was produced by transecting the stomach. Rats were divided into three groups, i.e., a hyperoxic group, a hypoxic group, and a normoxic control group, which were supplied with 40%, 10%, and 21% oxygen, respectively, for 7 days after operation. All rats were fed intravenously (257 kcal/kg/day, kcal/N: 185.5) and whole-body protein turnover was measured using the constant infusion of 15N glycine technique developed by Picou and Taylor-Roberts. For six days after operation, the cumulative nitrogen balance was negative for the hypoxic rats, while hyperoxic rats showed a positive balance that was significantly higher than in control rats (p less than 0.01). Protein synthesis and breakdown rates increased markedly and the rate of synthesis exceeded that of breakdown in the hyperoxic group, but in the hypoxic group synthesis remained unchanged while breakdown increased moderately. These results indicate that oxygen, as a nutrient, is effective for improving postoperative protein metabolism.
One hundred twenty-three cases of cervical spondylotic and ossification of the posterior longitudinal ligament (OPLL) myelopathy cases with long tract signs subjected to surgical treatment were studied to identify the most important factors having an influence on postoperative outcome using evoked spinal cord potentials (ESCP). Disappearance and positive wave changes of these potentials at the level of responsible lesions and slow conduction velocity under 40 m indicated an unsatisfactory outcome. Localized-lesion cases diagnosed by ESCPs had excellent results, significantly more so than extensive-lesion cases, regardless of operative methods. In 123 cases, 76% were found to have localized lesions, while the other 24% showed extensive lesions. Concerning the difference between CSM and OPLL, 54% of OPLL and only 14% of CSM demonstrated extensive lesions.
T lymphocytes and alveolar macrophages accumulating in the lower respiratory tract of patients with pulmonary sarcoidosis are known to be activated to produce several cytokines, presumably leading to granuloma formation within the lung. We hypothesized that these cells produce colony-stimulating factors (CSFs), which have been shown to affect the proliferation and function of monocyte-/macrophage-lineage cells. To test this hypothesis, we tried to detect mRNA encoding CSFs in cells obtained by bronchoalveolar lavage using a reverse transcription-polymerase chain reaction. Granulocyte-macrophage CSF (GM-CSF) mRNA was detected in five of six patients with pulmonary sarcoidosis, whereas it was detected in none of the five normal controls. Macrophage-CSF mRNA was detected in all subjects examined, and interleukin-3 mRNA in none. These results suggest some relation of GM-CSF to sarcoid lesion formation.
A variety of molecular techniques were used to search human and baboon gonadal tissues for evidence of transcription of the genes for the peptide hormones oxytocin and vasopressin. Only a highly sensitive assay based on a modification of the polymerase chain reaction succeeded in detecting mRNA copies of the oxytocin gene in both human and baboon corpus luteum. Vasopressin gene transcription was not detected in human testis and corpus luteum and was found only once in four different experiments in baboon corpus luteum. Evidence for oxytocin gene transcription in the human testis was found in three of five experiments. The method employed and subsequent sequence analysis of the polymerase products verified the presence of oxytocin mRNA with normal hypothalamic-type exonic structure in primate corpus luteum. Nevertheless, the very low levels of mRNA present are unlikely to support other than local functions for the encoded nonapeptide hormones.
We treated 91 congenital club feet in 59 children using a modified Denis Browne splint, and followed them for an average of 6 years and 3 months. The modified splint has an aluminium crossbar holding a pair of plastic shoe inserts moulded into corrected positions, and its use was started in children whose ages ranged from four weeks to nine months. Operation was later required in only 31 feet in 20 children. We have reviewed the other 60 feet in 39 children treated by splintage alone. All 60 feet had excellent or good function and from radiographic assessment, equinus, adduction, varus and cavus deformities had all been well corrected. Our results show that the modified splint can give good results.
The Western blotting (WB) method was used to evaluate the prevalence of human alveolar echinococcosis in Hokkaido, Japan. One hundred and sixteen WB-positive serum specimens were found in 1987 and 1988, and were grouped into two serologically different types from their WB-immunostaining patterns: "complete type", that showed multiple bands with various molecular weights ranging from 29 to 205 kilodaltons (kDa), and "incomplete type", that revealed a few bands with low molecular weights of 30-35 kDa and/or with molecular weights higher than 90 kDa. Therefore, we found that the WB method is applicable for the sero-epidemiological studies of human alveolar echinococcosis in Hokkaido.
Presented is the case of a 71-year-old female with an associated, concomitant early multiple esophageal carcinoma and a gastric carcinoma. An esophageal endoscopy revealed a reddish region in the upper and middle esophagus. This lesion, in part, remained unstained by lugol. Further, three lesions were observed at the upper and middle portions of the esophagus in a resected specimen, and the macroscopic diagnosis was 0-II c, These lesions were histologically diagnosed as being moderately differentiated squamous cell carcinomas, 5 x 15 mm, 2 x 5 mm, and, 3 x 5 mm in size. Furthermore, a lesion in the cardia was found superimposed on the esophageal cancer, and diagnosed as being an adenocarcinoma (II b), 10 x 15 mm in size. In the literature, reports of a concomitant association of an early esophageal carcinoma and a gastric carcinoma amount to 19 cases in Japan, our case being the twentieth case. In formatively, our case was found to display a multiple carcinoma in the esophagus.
The properties of Ca2+-activated K+ channels in mouse mammary epithelial cells in primary culture were studied by the patch-clamp technique. In cell-attached patches, spontaneous channel openings were sometimes observed; the slope conductance of the currents was about 12 pS at negative membrane potentials with a physiological solution (152 mM Na+, 5.4 mM K+) in the pipette. External application of A23187, a calcium ionophore, activated this channel. In excised inside-out patches, the channel was activated by increasing the internal Ca2+ concentration (10(-7) to 10(-6) M). No voltage dependence of the channel was activated was observed. Internal Na+ blocked the outward K+ current in a voltage dependent manner and this block led to the non-linear I-V relationship at positive membrane potentials. The channel was blocked by internal Ba2+ (0.1 mM) and tetraethylammonium (TEA+, 20-50 mM). Ba2+ reduced the open probability but not the single channel conductance, whereas TEA+ reduced the single channel conductance. The single channel conductance of this channel, measured from the inward current with a high-K+ solution (150 mM K+) in the pipette, was large (about 40 pS), and showed inward rectification. These results suggest that this channel is different from the usual small conductance Ca2+-activated K+ channels observed in many other cells.
C5 segment motor loss (deltoid muscle) after a cervical spinal operation, either anterior decompression or posterior decompression, has rarely been reported. However, most such cases are diagnosed immediately after surgery, and they appear to be due to inadequate surgical technique or insufficient decompression. Recently, the authors have experienced three cases in which weakness of the deltoid muscle started several days following anterior cervical decompression surgery. On the basis of postoperative plain films, myelography, and computed tomography (CT) myelography, the cause of the motor weakness was considered to be C5 root disturbance due to postoperative malalignment of the cervical spine, which gradually increased. The malalignment of the cervical spine seemed to be due to weakness of iliac bone grafts, inadequate design of the bone graft and recipient site, and insufficient postoperative fixation. To prevent this malalignment, eg, lateral inclination of the bone graft to axial rotation deformity, adequate bone graft design is required. The authors have been performing internal fixation using sapphire screws on cases in which fusion extending over four vertebral bodies is required.
