Oxytocin production and oxytocin messenger ribonucleic acid levels in bovine granulosa cells are regulated by insulin and insulin-like growth factor-I: dependence on developmental status of the ovarian follicle.

Oxytocin is a major peptide product of the ruminant corpus luteum, and the release of oxytocin from serum-free cultures of bovine granulosa cells is stimulated by insulin and insulin-like growth factor-I (IGF-I). Here we have assessed the effects of insulin and IGF-I on oxytocin gene expression in bovine granulosa cells and the dependence of these effects on the developmental status of the cells. When cells from individual follicles were cultured, the estradiol concentration of the follicular fluid was highly correlated with insulin-stimulated oxytocin release. Subsequently, cells were pooled from follicles selected on the basis of estradiol content, and two subsets of cells were distinguished. The first contained highly differentiated cells, as judged by the high estradiol (HE-cells) concentration of the follicular fluid (greater than 40 ng/ml), high levels of LH receptors, and high hCG-stimulated cAMP accumulation. The second subset contained cells from follicles with low estradiol (less than 1 ng/ml; LE-cells) which have fewer LH receptors and low hCG-stimulated cAMP accumulation. Oxytocin production was increased more than 50-fold by insulin (EC50, 230 +/- 57 ng/ml) and IGF-I (EC50, greater than 10 ng/ml), but only in the HE-cells. Oxytocin mRNA was also greatly increased by insulin and IGF-I in the HE-cells only. In contrast, insulin and IGF-I stimulated progesterone release from both HE- and LE-cells. Since oxytocin production is a characteristic of bovine luteal cells, our results support possible roles for IGF-I and insulin in regulation of luteinization or luteal activity. The data suggest that effects of insulin and IGF-I on oxytocin production reflect their effects on oxytocin gene transcription, and that granulosa cells must be appropriately primed (presumably by the in vivo hormonal environment) before they are able to produce oxytocin in response to these polypeptides.

Fosfonochlorin, a new antibiotic with spheroplast forming activity.

A new antibiotic, fosfonochlorin, was found in the culture filtrate of four strains of fungi freshly isolated from soil samples. These strains were identified as Fusarium avenaceum, Fusarium oxysporum, Fusarium tricinctum and Talaromyces flavus. Fosfonochlorin was a low molecular weight antibiotic (MW 158), soluble in water and methanol, but insoluble in acetone, ethyl acetate and chloroform. It was named after its possession of phosphorus and chlorine atoms, each one molar in its structure. The structure was determined as chloroacetylphosphonic acid mainly by the 1H NMR and mass spectrometric analyses. It was moderately active against some species of Gram-negative bacteria and its synergistic effect with glucose-6-phosphate was observed on Staphylococcus aureus and Escherichia coli. Spheroplast formation of the susceptible organisms with this antibiotic suggested that it might inhibit their cell wall synthesis.

[Electron microscopic study of canine dentin and odontoblast following the insertion of various composite resin monomers].

The purpose of this study was to evaluate the effect of composite resin monomer on dentinal tubules, odontoblasts and pulp with scanning electron microscopy (SEM) and transmission electron microscopy (TEM). Resin monomers of ethyleneglycol dimethacrylate (EDMA), triethyleneglycol dimethacrylate (Tri-EDMA), urethane dimethacrylate (UDMA), and 2, 2-bis [4- (3-methacryloxy-2-hydroxypropoxy) phenyl] propane (Bis-GMA) were used, and placed on a class V cavity in eighty teeth of adult dogs. Then, these treated animals were sacrificed after 30 to 120 days. The findings obtained were as follows: In the odontoblast body; 1. After 30 days, there were uneven unclear envelopes in all odontoblasts. 2. After 60 and 90 days, there were more invaginations of unclear envelope in Bis-GMA group than those in EDMA, Tri-EDMA and UDMA, and lysosomes and autophagic vacuoles increased in number. 3. In Bis-GMA group after 120 days, there were many degenerative changes of chromatin aggregation and hydropic, and many necrosis cells were seen. Uneven nucleous and lysosomes increased in EDMA, Tri-EDMA and UDMA groups. In the odontoblast process; 4. The odontoblast process disappeared in the dentinal tubule one third of cavity floor side of residual dentin with SEM and TEM after 30 days. However, after 120 days, there were few granular substances and calcified closure was not seen in dentinal tubules. In the region of central one third; 5. Microfilaments and microtubules in the odontoblast process decreased in number, and disarrangement and reticular degeneration of these fibers in all experimental groups after 30 days were seen. After 60 days, shrinkage and reticular degeneration of the odontoblast process were seen. Especially in Bis-GMA group, the odontoblast processes disappeared and the one in other three groups showed reticular degeneration after 90 and 120 days. 6. At the portion of pulp side one third on residual dentin, all odontoblast processes were almost seen normal ultrastructure after 60 days. In the group of Bis-GMA after 90 days, reticular degeneration was seen. Shrinkage of microfilaments and microtubules in EDMA, Tri-EDMA and UDMA groups were slightly seen. In the dentinal tubules after 120 days, reticular degeneration and empty in Bis-GMA group, and disarrangement of microfilaments and microtubules was seen in EDMA, Tri-EDMA and UDMA groups. The above findings indicated that composite resin monomers used in this experiment have effects on the odontoblast respectively. In the clinic, when we restore the cavity with these composite resins, we should intercept the stimulation of residual monomers with perfect dentin lining.

[A clinicopathological study on tumor of the renal pelvis and ureter].

We report 42 patients with urothelial tumors in upper tract admitted to our hospital between August, 1969 and August, 1988. The patients consisted of 33 males and 9 females; their ages ranged from 42 to 85 years with a mean of 66.2 years. Total nephroureterectomy with bladder cuff resection was employed as the surgical method in 24 cases, total nephroureterectomy without bladder cuff resection in 11 cases, total nephroureterectomy with total cystectomy in 2 cases and partial ureterectomy only in 2 cases. Tumor lesions had a positive correlation with grade and stage. The survival rate for all the patients at 1, 3 and 5 years was 76.0, 58.8 and 54.6%, respectively, as measured by the Kaplan-Meier's method. The prognosis of the patients with renal pelvic tumor and ureter tumor was dependent upon grade, stage and intravascular tumor-emboli. Vesical recurrence was observed in 10 cases and found frequently in low grade tumor and/or low stage tumor cases. The frequency of vesical recurrence was not positively correlated with cuff resection The 5-year survival rate was not different between the patients with vesical recurrence and those without vesical recurrence.

[CEA and CA19-9 in BALF from patients with idiopathic interstitial pneumonia].

Carcinoembryonic antigen (CEA) and carbohydrate antigen 19-9 (CA19-9) were measured in bronchoalveolar lavage fluid (BALF) in 21 patients with idiopathic interstitial pneumonia (IIP) and 7 healthy volunteers. The concentrations of both CEA and CA19-9 in the BALF were significantly higher in patients with IIP than those in healthy subjects. Significant correlations were present between the concentration of CEA and neutrophil percentage of the total BALF cells and between the concentration of CA19-9 and neutrophil percentage of the total BALF cells in the patients with IIP. Immunohistochemical study of tissue CEA and CA19-9 in the postmortem lungs of patients with IIP showed that CEA staining was present in the epithelia of respiratory bronchioles and alveoli, and it was especially increased in the region of alveoli where type II pneumocytes proliferate. CA19-9 staining was present in the epithelia of the respiratory bronchioles but absent in the epithelia of alveoli. It seems that assay of CEA in BALF may be useful to estimate the degree of pathological change and the activity of IIP.

[Preoperative radiotherapy of bone and soft tissue sarcomas].

UNLABELLED: The role of preoperative radiotherapy was evaluated in 16 cases with soft tissue sarcoma and 13 cases with osteosarcoma. Nine osteosarcoma cases underwent radiotherapy of whole lesion, and 4 cases had radiotherapy only of the surgically incurable portion. There were no local recurrences in M0 cases, but skin necrosis occurred in the whole radiation group. As for the soft tissue sarcomas, local recurrence was not seen in virgin cases, but two cases which had received previous treatment showed local recurrence. There were no cases with severe side effects. CONCLUSION: 1. Partial radiotherapy was effective as preoperative treatment for osteosarcoma. 2. preoperative radiotherapy is better than postoperative radiotherapy from many standpoints.

[Evaluation method of surgical margin in musculoskeletal sarcoma].

The evaluation of surgical margin is useful in determining the curability of surgical treatment of musculoskeletal sarcoma and the degree of possibility to reduce later surgery as part of preoperative therapy. However, at present there has been no worthwhile evaluation method developed for these purposes. In this paper we propose a new evaluation system for surgical margin as drafted by the Bone and Soft Tissue Tumor Committee of JOA. This system was applied in 197 cases at the Cancer Institute Hospital to determine the clinical significance in the treatment of musculoskeletal sarcoma. In this method, surgical margin was classified into four types according to the distance between the surgical margin and the reactive zone of the tumor. The types of surgical margins (in order of surgical extent) are curative margin, wide margin, marginal margin, and intralesional margin. Surgical margin is said to be curative if the margin is more than 5cm outside the reactive zone. It is referred to as wide if the margin is less than 5cm and marginal if the margin is at the reactive zone. In our evaluation, a thin barrier is estimated as a 2cm thickness of normal tissue, a thick barrier as a 3cm thickness of normal tissue, and joint cartilage as 5cm thickness. Moreover, in all cases the surgical margin is considered to be curative if the margin is outside a barrier and there is normal tissue between the barrier and the reactive zone of the tumor. Macroregenerative nodule of the liver. A clinicopathologic study of 345 autopsy cases of chronic liver disease.

The livers of 345 autopsy cases of chronic liver disease were examined for macroregenerative nodule (MRN), a large nodular lesion more than 10 mm in diameter. A total of 86 lesions of MRN was found in 49 cases (14.2%): 32 were from 191 cases of hepatocellular carcinoma (HCC), 16 were from 148 cases of cirrhosis only, and one was from six cases of chronic hepatitis. The incidence (19.6%) and the size (12.1 mm) of MRN in macronodular type of cirrhosis were significantly higher and larger than those in other types of cirrhosis. Also, the average size of MRN lesions of cases with HCC (12 mm) was significantly larger than that of cases with cirrhosis only (10.5 mm). The incidence of liver cell dysplasia (LCD) in cases of MRN (67.3%) was significantly higher than that in cases without MRN (40.9%). The MRN lesions were divided into Type I and Type II, the latter having proliferative foci distinguishable from the surrounding tissue. Three of them contained atypical cells regarded as cancer. Type II lesions were larger in size, and cases with multiple MRN lesions were seen more frequently in cases of HCC. The average age of the patients with Type II lesion was 5 years older than those with Type I lesions. These findings suggest that MRN should not be ignored in the morphogenesis of HCC.

Electrophysiologic diagnosis of cervical OPLL myelopathy using evoked spinal cord potentials.

The purpose of this study is to establish the correct diagnosis of the location and extent of intraspinal cord lesions in cases of continuous or mixed-type ossification of the posterior longitudinal ligament and to estimate the postoperative prognosis using evoked spinal cord potentials (ESCP). Twenty-six patients, who underwent surgery from 1985 to 1987 and who have been followed for more than 6 months, were examined using a conductive ESCP, which demonstrates lower extremity, bowel, and bladder function, and a segmental ESCP and dermatome segmental ESCP, which demonstrate upper extremity function. A five-pole recording electrode was placed in the cervical epidural space. The stimulation sites were the thoracic epidural space for conductive ESCP, the median nerve at the elbow for the segmental ESCP, and the finger surface for the dermatome segmental ESCP. In cases in which the ESCP disappeared at the middle of the narrow cervical spinal canal, another stimulating electrode was placed in the cisterna magna, and a descending conductive ESCP was recorded to monitor the upper border of the spinal lesion. New findings, which could not be observed by roentgenograms, myelography, and CT scan, were detectable using this technique.

Comparative study of hepatocellular carcinoma in Shikoku, Japan: a study of 198 autopsy cases in Tokushima and Ehime Prefectures.

A total of 198 cases with hepatocellular carcinoma (HCC) autopsied in two different areas in Shikoku, Tokushima and Ehime, was examined clinicopathologically. The incidence of HBsAg-positives in overall HCC cases was 49.0%, being higher in Tokushima (52.4%) than in Ehime (43.1%). The difference seemed to be caused by the high incidence in female cases in Tokushima (46.7% in Tokushima vs. 23.1% in Ehime). The mean age of HCC cases with HBsAg (53.9 years) is younger than that of cases without HBsAg (58.8 years). The incidence of HBsAg-positive HCC cases in Shikoku decreased step by step toward older age groups from 63.4% in 41-50, 59.5% in 51-60, 34.6% in 61-70, to 15.8% in over-71 age groups, and has been decreasing gradually with the passage of time from 57.1% in 1960-1971, 50.0% in 1972-1976, to 40.8% in 1977-1981. The incidences of HBsAg-negatives in all autopsy cases increased from 2.0% in 1960-1971 to 3.3% in 1977-1981 (P less than 0.05) although the incidences of HBsAg-positives showed almost the same level. Both in Tokushima and Ehime, the incidences of HCC associated with cirrhosis and HCC with HBsAg were smaller in grade IV cases, and the incidence of HBsAg-positive HCC was high in cases with macronodular type of cirrhosis.

Changes in plasma cyclic AMP and cyclic GMP during spontaneous labor and labor induced by oxytocin, prostaglandin F2 alpha and prostaglandin E2.

To investigate the response of cyclic nucleotides to the oxytocic agents administered for induction of labor, plasma concentrations of cyclic AMP (cAMP) and cyclic GMP (cGMP) were determined by radioimmunoassay during spontaneous labor and labor induced by oxytocin (OT), prostaglandin F2 alpha (PGF2 alpha), or PGE2 (PGE2). Subjects were 7 Japanese women in each labor group. Plasma cAMP levels significantly rose at the time of crowning of the fetal head in all 4 groups. They did not increase until that time in the 3 labor groups (spontaneous, OT-induced, and PGF2 alpha-induced labor groups). In the PGE2-induced labor group, plasma cAMP levels were significantly higher at labor onset (mean +/- SEM = 16.5 +/- 1.3 pg/ml) when compared to the pretreatment values (13.7 +/- 1.0 pg/ml), and increased thereafter gradually toward the time of crowning of the head (26.3 +/- 2.0 pg/ml). Plasma cGMP levels in the OT-induced group significantly rose after the onset of labor and remained at a high level until expulsion of the fetus. Plasma cGMP levels in the other groups did not change significantly throughout labor. These results suggest that cAMP may be involved in the labor process induced by PGE2, and that cGMP may be in that induced by OT.

A case of intralobar pulmonary sequestration with high serum CA19-9 levels.

A 38-year-old female presented with cough and fever. A chest X-ray examination revealed an abnormal shadow in the posteroinferior portion of the left hemithorax, and a laboratory examination showed that the serum carbohydrate antigen 19-9 (CA19-9) level was markedly high (1000 U/ml). A left thoracotomy showed an intralobar pulmonary sequestration of the left lower lobe, and after a left lower lobe lobectomy, the serum level of CA19-9 decreased to normal. Increased CA19-9 activity was detected by immunohistochemistry in the epithelia of bronchioles in the pulmonary sequestration. This communication is the first to report a case of increased activity of CA19-9 in pulmonary sequestration.
