[Pre- and intra-operative chemotherapy using FT-207 suppositories--with special reference to lung cancer].

This study was conducted to determine efficacy of FT-207 to prevent hematogenous peri-operative metastasis of lung cancer. A total of 16 patients consisting of 10 with squamous cell cancer, 6 with adenocarcinoma were evaluated in this study. The protocol of this study consisted of the pre-and peri-operative chemotherapy, giving 1000 mg of FT-207 suppository daily for 10 days. 5-FU plasma concentration in the systemic circulation during surgery was 0.01 mcg/ml. 5-FU tissue concentration of the normal lung was 0.07 mcg/g, and cancerous tissue (squamous cell ca 0.11 mcg/g, adenocarcinoma 0.12 mcg/g), and lymph nodes 0.15 mcg/g. It was found in this study that the pre-and peri-operative adjuvant chemotherapy with FT-207 was effective to prevent operation-induced hematogenous metastasis of lung cancer.

Free amino acid content of lymphocytes nd granulocytes compared.

For extracting free amino acids from human leukocytes, we find that disruption of the cells by ultrasonication is more reliable than freezing and thawing. The amount of free amino acids extracted by the latter method depends on the temperature and duration of thawing. We extracted free amino acids from leukocytes of healthy men by ultrasonication and compared their concentrations in lymphocytes and granulocytes. The amino acid content of granulocytes significantly (p less than 0.001) exceeded that of lymphocytes. Of the amino acids extracted from granulocytes, 76% was taurine; for lymphocytes taurine, aspartic acid, and glutamic acid were predominant, respectively composing 44%, 17%, and 26% of the total. Taurine is proposed as an index of cell disruption.

Isolation and structural studies of the neutral oligosaccharide units from bovine glycophorin.

The O-glycosidically linked carbohydrate units of glycophorin from bovine erythrocyte membranes were released as reduced oligosaccharides by alkaline borohydride treatment. These oligosaccharides were separated by ion-exchange chromatography followed by gel filtration. Three oligosaccharides, a penta- a hepta- and a decasaccharide were obtained as the major components from the neutral fraction, and seven fractions were separated from the acidic fractions. All of the fractions were found to contain galactose and N-acetylglucosamine in variable amounts, as well as N-acetylgalactosaminitol. Studies of the neutral oligosaccharides by methylation analyses, nitrous acid deamination and Smith degradation, indicated the structure of the pentasaccharide to be Gal(1 leads to 3)Gal(1 leads to 4)GlcNAc(1 leads to 3)GalNAcol and that of the heptasaccharide to be Gal(1 leads to 3)Gal(1 leads to 4)GlcNAc(1 leads to 3)Gal(1 leads to 4)GlcNAc(1 leads to 3)Gal(1 leads to 3)GalNAcol. The highest molecular weight fraction, decasaccharide in the neutral fraction had a branching point at C-6 of a galactose residue.

Host factors and susceptibility to rubella virus infection: the association of HLA antigens.

To examine the role of host-dependent factors to natural rubella infection, the positive rates of HAI antibody according to HLA-A and -B antigens were studied in 93 children, aged 9 to 10, who were exposed only to a recent epidemic (single-exposure group), and in 199 adult females, aged 18 to 23, who were exposed also to a former epidemic (dual-exposure group). Furthermore, the phenotype frequencies of HLA antigens were investigated in seronegative subjects in both groups, as well as in 58 adult female vaccinees, in comparison with those of the Japanese population. The seropositive rates by the HLA antigens, 38.7% and 73.4%, respectively, in each exposure group were found in subjects having some HLA antigens. Several of these candidate HLA antigens varied in phenotype frequencies in seronegative subjects of all three groups from those of the average Japanese. Although a few types were statistically significant, no contradictory findings in susceptibility were obtained among the groups during the study, which suggested strongly that subjects with HLA-Bw51 and -Bw16 are susceptible. In addition, when the seropositive rates were compared in all combinations of HLA-A and -B antigens classified according to expected susceptibility, the positive rates were arranged in order of susceptibility with significant statistical differences in several groups.

Increased body movements during sleep in Gilles de la Tourette syndrome.

Overnight sleep polygrams were recorded form 9 patients with Gilles de la Tourette syndrome (GTS). Six of 9 patients had abnormal electroencephalograms, but no specific abnormalities were detected. Body movements and twitch movements during sleep were analyzed. At all stages of sleep, body movements during sleep were more frequent in cases of GTS than those in normal controls. Twitch movements in stage REM of sleep were significantly increased in GTS. These results are consistent with the idea that GTS is due to an imbalance between the central neurotransmitters, catecholamine and serotonin.

Influence of ACTH therapy on overnight sleep polygrams in infantile spasms.

Overnight sleep polygrams were recorded before and during therapy in nine patients with infantile spasms. Results showed that ACTH therapy increased the waking time and decreased rapid eye movement sleep. Thus it caused sleep disturbance in patients with infantile spasms. During ACTH therapy the number of rapid eye movements/min and the pulse rate decreased significantly. Body movements/min also decreased, but not significantly. These results suggest that ACTH therapy may inhibit functions of the central nervous system. The respiratory rate increased during ACTH and clonazepam therapy, probably in association with the decrease or the absence of seizures. These findings indicate the necessity for further studies on whether ACTH therapy is really of value in patients with infantile spasms, and show that if ACTH is given, the period of therapy should be as short as possible.

Follow-up study of pharyngeal carriers of beta-hemolytic streptococci among school children in Sapporo City during a period of 2 years and 5 months.

A total of 124 normal school children in three classes (aged 8 to 9) were examined for beta-hemolytic streptococci for 29 months from August 1977 to December 1979 be means of serial monthly throat cultures. No significant difference between the carrier rate of boys and that of girls was observed. Although no monthly variation of carrier rates appeared in class II, marked temporal increases of carrier rates appeared from June 1978 (in Class I) and from August 1978 (in class III) in August 1979. These periodical variations of carrier rates observed in this investigation do not coincide with the low-in summer and high-in winter seasonal variations which has generally been observed by many other investigators. The average values of carrier rates throughout the period of this investigation were 30.9, 29.7, and 24.5% in classes I, II, and III, respectively. The carrier rate of group A streptococci by month and by class showed some positive correlation with the carrier rate of total beta-hemolytic streptococci. T-type 4 and T-untypable strains were not identified up to a certain period; then each strain appeared at a given time during the course of our investigation in one class, spread to all three classes, and continued to be isolated until the end of this investigation. The origin of these type of strains could not be identified in this study. Of the children, 36 (29.0%) never became streptococcal carriers during the period of examination and 88 (71.0%) became streptococcal carriers at least one time during the 29 examinations; among these positive carriers, 29 children (23.0%) showed positive results in more than 50% of the examinations.

Changes in cardiac beta-adrenoceptor concentrations in spontaneously hypertensive and experimental renal hypertensive rats.

Cardiac beta-adrenoceptors were studied in membrane fractions from spontaneously hypertensive rats (SHR) and rats with two-kidney, one clip hypertension (2K, 1C HT), using radioligand binding method. beta-Adrenoceptor concentration measured by [3H]-dihydroalprenolol (DHA) binding was significantly lower in cardiac membranes from two months old SHR than those from Wistar-Kyoto rats (WKY) (38.2 +/- 2.6 vs 45.1 +/- 1.8 fmol/mg protein, means +/- SEM, p less than 0.05). Cardiac membranes from 2K, 1C HT rats had also a lower concentration of beta-adrenoceptors than those from the sham-operated control rats at a week after operation (30.9 +/- 2.2 vs 47.8 +/- 1.6 fmol/mg protein, p less than 0.01). But receptor affinity remained unchanged. These reduced concentrations of beta-adrenoceptors were restored to control levels at 12 months old in SHR and at 6 weeks after operation in 2K, 1C HT rats, although age-dependent decrease in beta-adrenoceptor was observed. The decrease in beta-adrenoceptor was associated with increase in plasma noradrenaline levels during the earlier stages of hypertension. But there is no correlation between beta-adrenoceptor concentrations and plasma noradrenaline levels in the chronic stages of hypertension. No significant difference was found in activities of 5'-nucleotidase, which is a marker enzyme of cell membrane, in membrane fractions between the hypertensive hearts and the controls, suggesting that the cardiac hypertrophy is not a determinant factor for change in beta-adrenoceptor. The observed decrease in beta-adrenoceptor concentration may reflect an increase in sympathetic nerve activity during development of hypertension. In the chronic stages of hypertension, additional factors may be involved in the restoration of beta-adrenoceptors.

Histopathological studies on the myocardial fibrosis and vascular lesion of rheumatic valvular disease.

To clarify the types and frequency of myocardial fibrosis and vascular lesions caused by different types of the rheumatic valvular disease, 41 autopsied hearts with rheumatic valvular lesions were examined and the following results were obtained. As to myocardial fibrosis, 1) perivascular fibrosis, caused mainly by perivascular inflammation and partly by myocardial strain, varied in grade according to the types of the valvular disease, i.e., most severe in mitral regurgitation (MR) and combined valvular disease (CVD) and least in mitral stenosis (MS); 2) Aschoff's nodes: 3 typical and 8 atypical cases were found; 3) perimycial fibrosis due to myocardial strain: most severe in MR and CVD and least in MS; and 4) irregular patchy fibrosis, caused mainly by myocarditis and partly by ischemia, was noticed in all types of the valvular disease. As to vascular lesion, 1) angitis was found in 7 cases and most frequently in MR; and 2) thickening of the intima and media was found in all types of valvular disease. Thus, myocardial fibrosis and vascular lesion varied according to the types of valvular disease, and not only hemodynamic changes but also myocardial fibrosis and vascular lesions might determine the prognosis of the valvular disease.
