[A case of malignant schwannoma of the mesentery in a patient with von Recklinghausen's disease].

A 23-year-old man with Von Recklinghausen's disease was admitted because of abdominal pain. The preoperative diagnosis was malignant mesenteric tumor. Laparotomy was done and the tumor was located in the mesentery of the ileum which showed numerous tortuous cords or bulbous projections from the surface. Partial resection of the involved ileum and the mesentery was performed. Histological examination revealed plexiform neurofibroma and malignant Schwannoma of the mesentery. It is very difficult to preoperatively diagnose early associated lesions of Von Recklinghausen's disease. Therefore, careful examination of the GI tract is important so that earlier therapy can be carried out.

[Preoperative diagnosis for stage II endometrial carcinoma using endocervical conization].

It is generally difficult to identify stage II endometrial carcinoma correctly. To differentiate cervical involvement, endocervical conization was performed on 31 patients with endometrial cancer between May 1982 and September 1983. The preoperative diagnosis of stage II endometrial carcinoma was confirmed by postoperative histologic examination. In stage I patients, there was no microscopic cervical involvement after the operation. Fractional curettage and hysteroscopy are commonly used to detect stage II endometrial carcinoma. However, these examinations may overlook cervical stromal invasion. Endocervical conization is a simple procedure and highly useful to detect cervical stromal involvement. We recommend the of endocervical conization combined with hysteroscopy to diagnose stage II endometrial carcinoma.

Favorable effect of dimethyl sulfoxide on secondary amyloidosis in juvenile rheumatoid arthritis.

A girl with secondary amyloidosis as a complication of juvenile rheumatoid arthritis was administered dimethyl sulfoxide by topical application to the skin. Her gastrointestinal symptoms and massive proteinuria improved. Decreased left ventricular function and creatinine clearance also improved remarkably. The favorable effect of dimethyl sulfoxide in this single patient deserves further study in a controlled trial.

[Two cases of malignant cardiac tumors attached on the anterior mitral valve].

Two cases were reported in which malignant cardiac tumors attached to the mitral valve were diagnosed during their lifetimes using two-dimensional echocardiography. Case 1 was a 29-year-old man with speech disturbance and left hemiparesis. Histological examination of the specimen excised from the cardiac tumor during the operation revealed mesenchymal chondrosarcoma, which is extremely rare etiologically and has never been reported so far. Case 2 was a 62-year-old woman complaining of paroxysmal nocturnal dyspnea and chest pain. The diagnosis of malignant histiocytosis was made from the pathological examination of biopsy specimen taken from the rib metastasis. Using two-dimensional echocardiography, characteristic findings for the cardiac tumor were obtained. The tumor echo in Case 1 showed, unlike to that reported for myxoma, two different echogenic layers; the outer dense and the inner light in the echo density. On surgery, tumor echo was revealed to reflect the cystic lesion. In Case 2, two-dimensional echocardiography on admission revealed two separate tumor echoes which attached to the mitral valve and left atrial wall region, respectively. Within two months, they grew rapidly and finally fused into one mass resulting in so-called ball valve syndrome. Phonocardiographically, the tumor plop in Case 1 was high-pitched in quality, and was extinguished completely after the tumor was resected. The tumor plop in Case 2 was not audible on admission, but became evident after fusion of the tumor echoes and was associated with a presystolic murmur.

[Intracavitary radiotherapy with Cf-252 for uterine cervical carcinomas].

Cf-252 intracavitary radiotherapy using a remote afterloading system was performed in patients with Stage IIb, IIIb cervical carcinoma. Compared to the control group which was treated with Co-60 high-dose intracavitary radiotherapy, Cf-252 was less effective for squamous cell carcinoma and slightly effective for adenocarcinomas. Recto-sigmoidal injury was frequent in the Cf-252 group so that further study of the Cf-252 effect on the recto-sigmoidal mucosa is required.

Free amino acid concentrations in blood cells of two brothers with gyrate atrophy of the choroid and retina with hyperornithinaemia.

In this study we selected blood cells as accessible tissues and measured the free amino acid concentrations in erythrocytes, lymphocytes, and granulocytes of two patients with gyrate atrophy with hyperornithinaemia. We found that ornithine concentrations were elevated in blood cells, to different degrees depending on the type of cell, and that aspartic acid concentrations were low in erythrocytes. We also investigated the fluctuation of free amino acid concentrations in blood cells up to 24 h after the oral administration of ornithine. The ornithine concentration in the blood cells of the patients reached a peak 2 h after the oral administration, and remained high for at least 4 h, although after 2 h the concentration curves differed in the three types of cells.

Postural effects on behavioral states of newborn infants--a sleep polygraphic study.

Physiological and behavioral correlates of neonates in the supine and prone positions were examined. Polygrams were recorded in 10 newborn infants in the prone and supine positions. Newborn infants slept more in the prone position than in the supine, and quiet sleep was significantly more in the prone position. Gross movement, jerky movement and twitch movement were less in the prone than the supine position. There was no difference in localized movement or tremor-like movement in the two positions. Respiration was more regular in the prone than the supine position. Sleep apnea (greater than or equal to 6 seconds) was less in the prone position. The pulse rate during quiet sleep was higher in the prone position.

Short latency somatosensory evoked potential in children.

The short latency somatosensory evoked potential was studied in 90 normal children of 1 month to 16 years old and 7 adults. Somatosensory stimuli were delivered through a disc electrode placed over the median nerve at the wrist joint. The uniform recording sites used were the central region of the scalp, and the seventh cervical spine or Erb's point. Reference electrodes were placed on the hand contralateral to the median nerve stimulated. Three positive peaks (P1, P2 and P3) and one negative peak (N1) were consistently recorded, a further positive peak (P4) after N1 was not always observed. The latency of each peak per 1 m body length decreased with age until 2 or 5 years of age. The latency of each peak after 2 years of age was positively correlated with the body length and arm length. The value of P1 peak latency per 1 m body length reaches adult values at an earlier rate than the value of P3 peak latency and P2-P3 latency per 1 m body length. This suggests that central lemmiscal pathways mature at a slower rate than peripheral nerve fibers. The wave form pattern of the short latency somatosensory evoked potential changed to the adult pattern at 10 years of age. The peak latency of P4 during deep sleep was slightly prolonged. In recording on infants during sleep, the EEG should be monitored to determine the stage of sleep.
