[Technetium scintigraphy in experimental hyperergic arthritis and by rheumatoid arthritis].

Guniea pigs showed an increased uptake of 99m-Tc-04 in the inflamed joints during the first days of experimental arthritis. Tc-04 was found in the joint fluid and inflamed synovia. The uptake of Tc-04 and Tc-MDP was reduced by therapy in 13 patients with RA. Classical RA showed an increased uptake of Tc-MDP compared with probable RA. Scintigraphy offers the possibility of early diagnosis and study of progression.

[The immune competence of patients with Crohn's disease (author's transl)].

The cell-mediated and humoral immune responses were assessed in 15 patients with Crohn's disease and in 28 age-matched control subjects by means of several in vivo and in vitro methods. The disease activity in most patients was absent or moderate. Studying cellular immunocompetence we investigated the skin reactivity to various recall antigens (Candida, Trichophytin, Mumpsantigen, Streptokinase-Streptodornase, PPD), the primary immune response to Dinitrochlorobenzene (DNCB) and Keyhole Limpet Hemocyanin (KLH), and the lymphocyte transformation induced by mitogens (Phytohemagglutinin, Concanavaline A, Pokeweed Mitogen) and specific antigens. Humoral immunity was studied by measuring immunoglobulins, isohemagglutinins, and the antibody response to KLH. In addition, complement components and (in 10 patients) the proportions of T- and B-lymphocytes in the peripheral blood were evaluated. Cutaneous responsiveness to Candida, Mumps-antigen, SK-SD, and DNCB as well as the cellular immune response to KLH were impaired in patients with Crohn's disease (significance was reached for SK-SD, DNCB, and the mean area of induration). The lymphocyte transformation test with PHA, ConA, and PWM revealed normal results. For specific antigens (PPD, SK-SD, KLH) a good correlation could be demonstrated between delayed hypersensitivity and the in vitro lymphocyte responsiveness. Humoral immunity was not unequivocally impaired in Crohn's disease. Five patients with Crohn's disease proved constantly decreased total absolute lymphocyte counts in peripheral blood. The proportions of T- and B-lymphocytes and the complement-levels were corresponding to those in normal controls. No correlation was found between immunological and clinical parameters. In conclusion, patients with Crohn's disease exhibited a partial impairment of the cellular immune response, whereas humoral immunity was not affected. However, it remains to be elucidated whether this immune defect represents a basic pathogenetic factor in the onset of the disease.

[Autotransplantation of Langerhans' islets in a case of total duodenopancreatectomy (author's transl)].

A 36-year-old man suffering from chronic pancreatitis involving the whole organ had total duodenopancreatectomy. The Langerhans' islets were isolated from the extirpated organ and transplanted into the liver via portal artery. Insulin substitution could be lowered from 50 I.U. to 12 I.U. by the 5th post operative day. Repeatedly, raised insulin levels could be verified in response to high blood sugar loads. C-peptide and glucagon were also found. 7 months after transplantation the islets still seem to be functioning.

Islet transplantation in experimental diabetes of the rat. VI. Rate of regression in diabetic kidney lesions after isogeneic islet transplantation: quantitative measurements.

Intraportal transplantation of isogeneic adult islets in diabetic rats resulted in long-lasting amelioration of the metabolic disorder. The effect upon diabetes-induced kidney changes (enlargement of mesangial space, capillary changes, cell-proliferation) was examined quantitatively by morphometric studies. The major effect was a remarkable reduction of the mesangial space and re-widening of the capillary lumina. The number of endothelial cells was lowered. Regarding epithelial and mesangila cells no difference was observed between normal, diabetic and transplanted animals of age-matched groups.

[Cell kinetics in experimental rheumatoid lesions and application in antirheumatic drug research (author's transl)].

The kinetics of chronic inflammatory cellular infiltrates in antiglobulin-induced arthritis, peritonitis and subcutaneous fibrin granuloma were investigated. Only small numbers of the mononuclear cells were labeled after 3H-thymidine impulse labeling, whereas a high labeling index results after 3H-thymidine labeling of the bone marrow. It is concluded that in experimental arthritis the inflammatory so-called cellular hyperplasia consists in bone marrow derived mononuclear cells. In experimental peritonitis glucocorticoid treatment decreases the 3H-thymidine labeling indices. By means of such labeling studies the antiproliferative and antiexsudative properties of steroids were demonstrated quantitatively. Therefore we suggest to make use of experimental cell kinetics for the recognition of anti-rheumatic drugs.

[Autotransplantion of Langerhans islets after total duodeno-pancreatectomy in a patient with chronic pancreatitis (author's transl)].

Total duodenopancreatectomy was performed in a 36-year-old man because of global chronic pancreatitis. Islets cells were isolated by collagenase digestion from the resected pancreas and re-implanted into the liver via the portal vein. Until the sixth postoperative day the amount of insulin required fell from 54 IU initially to 10 IU per day. Serial insulin determination demonstrated secretion by the implanted cells.

[Age dependence of immunological methods for detecting an immunodeficiency state (author's transl)].

Cell-mediated and humoral immune responses were studied in 43 healthy volunteers aged between 15--77 years. Besides various skin tests with the antigens candida, trichophyton, mumps, streptokinase-streptodornase, tuberculin, DNCB and KLH also in vitro experiments measuring the immunoglobulin- and complement concentrations, the antibody production to KLH, the lymphocyte transformation rate to PHA, Pokeweed, Con A, PPD were done nearly in all patients. The responsiveness of lymphocytes to PHA and Con A and the sensitization of the skin with DNCB declines in the aged humans, beginning already at the 4th-5th decade. The skin tests with candida, trichophyton were not significantly different in the various age groups. The number of persons with positive skin reactions to the antigens mumps and streptokinase-streptodornase decreased also with advancing age. Lymphocyte transformation and skin test with PPD or tuberkulin were markedly increased beyond the age of 30. Serum levels of immunoglobulins, complement components, the antibody production to KLH, and the lymphocyte transformation rate to Pokeweed were not significantly different in the age groups. Correlation between the tests within each person could not be detected. We made the experience that only a few tests are not enough for the evaluation of an immunodeficient state, but that several tests, a so called TEST BATtery", are necessary.

[Immediate type allergy to insulin (author's transl)].

Allergic reactions of the immediate type to insulin are known since its introduction of the treatment of diabetes mellitus. After the development of the chromatographically purified insulins they are to be found significantly reduced, though still particular cases are observed. The simultaneous occurrence of insulin allergy and insulin resistance seems to be not too rare. The immediate type allergy to insulin is mostly released by the presence of IgE-antibodies. Another but not at all rare allergic reaction is the Arthus phenomenon, released by IgG-antibodies. For diagnosis of these allergic states intradermal skin tests with various insulins are used. Also specific IgE-antibodies are to be measured radioimmunologically lately. There is a strong correlation between the outcome of the allergic reactions and the definite degree of the specific IgE-antibodies. Low levels of specific IgE are found in insulin treated diabetics without any visible allergic reactions to insulin. This fact proves that the hormone, though being a clean insulin of a very high degree, is the antigen itself. Not only immunologic mechanism, but also other exogenic influences cause allergic reactions of the immediate type to insulin.

[Immunological aspects of inflammatory bowel diseases (author's transl)].

The gastrointestinal tract is replete with immunocompetent tissue represented by appendix, Peyer's patches and nonaggregated lymphocytes in the intestinal epithelium. This local immune system plays a vital role in the physiologic immune response and in the control of food, drug, microbial or viral antigens. Under certain situations this lymphoid tissue may contribute to the immunological reactions underlying the pathogenesis of various diseases. Generalised deficiencies of the immune apparatus frequently give rise to disturbances of gastrointestinal function. Furthermore plenty of immune reactions occurring in connection with human inflammatory bowel disease have been described. Some immunologic features are reported as typical for a disease, the majority seems to indicate only nonspecific autoimmune reactions. The relevance of immunological mechanisms for the pathogenesis of these diseases is still not full evaluated.

[Detection of circulating immune complexes using a laser-nephelometric assay (author's transl)].

Using Laser nephelometric measurement a method is described to detect immune complexes, which seems to be suitable for routine studies as well as for specific complex characterisation. Tetanus-Antitetanus complexes are used as standard thus the result is expressed as tetanus complex units (TCU). The method is simple and permits the direct measurement of complexes in native sera. Furthermore it is possible to characterise the complexes either by the immuno-globuline class of the antibody involved or the antigen respectively. Complexes were found with this method in various frequency in all but one group of patients under study (SLE, RA, other connective tissue diseases, malignomas, myelomas, thyroiditis, Grave's disease and healthy controls). Their was a high correlation with two other common techniques to measure immune complexes, e.g. C1q-deviation and C1q-PEG precipitation.
