Indications of an increase of occupational pleural mesothelioma in Japan.

In order to obtain an epidemiological picture of occupational pleural malignant mesotheliomas in Japan, the author surveyed the Annual of Pathological Autopsy Cases (published by the Japanese Pathological Society) from 1974 through 1980. Two hundred and twenty-two malignant mesotheliomas (0.114% of all autopsy cases) were found in that period. One hundred and forty-five cases (0.074%) of them were of pleural origin. Until 1977, there were no pleural malignant mesotheliomas associated with asbestosis, but there were one in 1978, three in 1979 and two in 1980. Two of them were housewives and the others were a ship builder, a welder, a ceramist and a steel factory worker. Two lived in Sakae City, and the others in Kure City, Kaizuka City, Nagasaki City and Kanagawa Prefecture, where large shipyards are located. Compared to the Western countries, there is a time lag of 10 to 20 years in the increase of consumption of asbestos in Japan, where the increase has occurred rapidly after World War II. The epidemiological picture obtained by this study clearly states that the number of occupational pleural malignant mesotheliomas began to increase in the past few years in Japan.

Identification of hemorrhagic site of the intestine by RI-angiography.

A 36-year-old man was transferred to our department because of massive malena followed by shock. Gastroduodenal endoscopy revealed no bleeding source in the stomach or the duodenum. RI-angiogram performed immediately after admission revealed a bleeding area at the jejunum and laparotomy was done. The bleeding lesion was detected by exploratory jejunotomy and a jejunectomy was successfully performed over 15 cm, including the lesion. RI-angiogram is useful as selective angiography in the diagnosis of intestinal bleeding and is less invasive.

Effect of shakuyaku-kanzoh-toh, a prescription composed of shakuyaku (Paeoniae Radix) and kanzoh (Glycyrrhizae Radix) on guinea pig ileum.

The actions of shakuyaku-kanzoh-toh (SK), a prescription of the traditional Chinese medicine, on an isolated guinea pig ileum were studied by comparing those of shakuyaku (S) (peony root, Paeoniae Radix) and kanzoh (K) (licorice root, Glycyrrhizae Radix). SK, S and K suppressed the neurogenic contractions of ileum induced by electrical stimulation and ganglionic stimulating agents such as DMPP and nicotine. Although S did not influence acetylcholine (ACh)-induced contraction of ileum, K inhibited ACh-induced contraction to the same extent as the neurogenic contraction. Also SK inhibited ACh-induced contraction but its inhibition was smaller than the inhibition of the neurogenic contraction. SK and K inhibited 40 mM KCl-induced contraction of ileum and the specific binding of 3H-QNB on muscarinic receptors in ileum, but S at 3 X 10(-4) and 10(-3) g/ml, which were enough to suppress the neurogenic contraction of ileum, did not inhibit them. These results suggest that the inhibitory actions of S and K on the neurogenic contraction are due to an inhibition of ACh release from cholinergic nerve and an inhibition of ACh action on ileum smooth muscle, respectively, and that the inhibitory actions of SK are responsible for both inhibitions by S and K. The inhibitions of ACh action by SK and K are presumed to be due to inhibitions of ACh binding on muscarinic receptors and of contractile machinery of smooth muscle.

Use of a prosthesis for repair of complete common atrioventricular canal.

From April, 1977 to November, 1979, 5 cases with common atrioventricular canal (Rastelli C-type) underwent surgery, in which the leaflets of both the mitral and tricuspid valves were extended using an auto-pericardium patch in order to control regurgitation. Their ages ranged from one year 3 months to 6 years (mean, 3 years 7 months). One case died on the 37th postoperative day from intracranial bleeding unrelated to the cardiac procedure. In the remaining 4 cases, postoperative examination showed an improvement, and their clinical courses which were followed from 2 years 7 months to 5 years 3 months, were favourable, with only minimal valvular regurgitation remaining in all cases. Although the fate of the pericardium patch must be followed over a long period, this procedure appears to be effective for control of regurgitation in this disease.

Effect of chronic administration of riboflavin 2',3',4',5'-tetrabutyrate on the hepatic enzymes of fatty acid oxidation in the rat.

Effects of long-term administration of riboflavin, sodium butyrate or riboflavin 2',3',4',5'- tetrabutyrate ( RTB ) on the activities of renal and hepatic enzymes that catalyze the beta-oxidation of fatty acid were determined in the rat. Feeding of riboflavin or sodium butyrate for 5 weeks had no effect on all the enzymes examined. By contrast, feeding of RTB resulted in an increase in the hepatic activity of 3-ketoacyl-CoA thiolase [EC 2.3.1.16] by 50% of the control level, while the activities of renal 3-ketoacyl-CoA thiolase and of hepatic and renal acyl-CoA synthetase [EC 6.2.1.3] and acyl-CoA dehydrogenase [EC 1.3.99.3] remained unaffected. The increase in hepatic 3-ketoacyl-CoA thiolase activity suggests that prolonged RTB administration results in an increased beta-oxidation of fatty acid in the liver, which may explain the reported reduction in the concentration of tryglyceride in plasma during RTB treatment.

[Human yolk sac tumor of the thymus heterotransplanted into nude mice].

A human yolk sac tumor of the thymus (YSK-1) was transplanted into athymic nude mice. Histological and ultrastructural investigations revealed that the YSK-1 tumor, even after serial transplantation in nude mice, characteristically displayed Schiller-Duval bodies, endodermal sinus structures and ultrastructural profiles, numerous microvilli, desmosome-like cell attachments, annulate lamellar structures, many lipid droplets and interspersed glycogen particles. Serological study by radioimmunoassay demonstrated AFP, CEA, HCG, alpha 1-AT and transferrin in the sera and cyst fluid of the tumor-bearing nude mice. Immunohistochemical investigation using the PAP method showed that the YSK-1 cells produced CEA, HCG, alpha 1-AT, transferrin as well as AFP. The increase in the level of AFP paralleled the increase in tumor size.

A hepatitis B surface antigen polypeptide (P31) with the receptor for polymerized human as well as chimpanzee albumins.

Hepatitis B virus, and hepatitis B surface antigen particles co-occurring with hepatitis B e antigen, have the receptor for polymerized human and chimpanzee albumins that may be involved in the hepatotropism of hepatitis B virus. We identified the receptor on a hepatitis B surface antigen polypeptide with a molecular size of 31,000 daltons (P31). P31 bound to polymerized albumins from human and chimpanzee, but did not react with polymerized albumins from the experimental animals without susceptibility to hepatitis B virus. P31 was composed of the major polypeptide of hepatitis B surface antigen with a molecular size of approximately 22,000 daltons (P22) and additional 55 amino acid residues coded by the pre-S region in the hepatitis B virus-specific deoxyribonucleic acid. Because P22 did not react with polymerized albumin, the sequence of 55 amino acid residues in the pre-S region appeared to bear the receptor. Polypeptides with the receptor for polymerized albumin may make an efficacious hepatitis B vaccine, because they could raise antibodies against the putative site on hepatitis B virus capable of binding with hepatocytes to initiate the infection.

Neutralizing antibody responses to Japanese encephalitis vaccine in children.

Two shots of the current Japanese encephalitis (JE) vaccine were given to children and their immune responses to the Nakayama strain (the vaccine strain) and two wild strains (JaGAr-01 and E-50) of JE virus were examined by neutralizing (N) antibody titrations. Seventy vaccinees had no N antibody to JE virus before the first vaccination and were bled one month after the second vaccination. The N antibody responses to the JaGAr-01 and E-50 strains were found to be similar and to be less than that to the Nakayama strain after the second vaccination: the geometric mean titers (GMT) of N antibodies to the JaGAr-01 and E-50 strains (as logarithms) were 1.87 and 1.75, respectively, while the GMT to the Nakayama strain was 2.89. The seroconversion rates to the Nakayama, JaGAr-01 and E-50 strains were 70/70 (100%), 69/70 (99%) and 68/70 (97%), respectively, after the second vaccination. Twenty-seven of the 70 vacciness were also bled before the second vaccination. Most of them showed a considerably high N antibody response against the Nakayama strain and only one vaccinee failed to show seroconversion after the first vaccination. However, the antibody response to the E-50 strain appeared to be rather low and 9 of 25 vaccinees did not show any seroconversion. Similarly 3 of 25 failed to show any seroconversion against the JaGAr-01 strain. These results indicate that at the initial immunization two shots, at least, of the current JE vaccine are necessary to stimulate effective immune responses to wild strains of JE virus.
