A glycopeptide containing 15 amino acid residues derived from hepatitis B surface antigen particles: demonstration of immunogenicity to raise anti-HBs in mice.

The major polypeptides composing hepatitis B surface antigen (HBsAg) particles are P-I and P-II. P-II shares the same amino acid sequence as P-I and contains an additional carbohydrate moiety of mol. wt approximately 5000. When a purified preparation of P-II was digested with Nagarse and then with Pronase P, it gave rise to a glycopeptide containing 15 amino acid residues and the carbohydrate moiety of P-II. The N-terminal amino acid sequence of the glycopeptide was determined to be Lys-Pro-Thr-Asp-Gly-Asn-. The polysaccharide moiety contained 5 moles of N-acetylglucosamine and was connected with Asn at the sixth position from the N-terminus. When mice were immunized against this HBsAg glycopeptide, they raised humoral antibodies which bound to each of three preparations of P-I derived from HGsAg particles of subtypes adw, adr and ayw, thereby indicating that the sequence of 15 amino acids in the glycopeptide would constitute a common antigenic structure of HBsAg.

Immunologic and epidemiologic aspects of varicella infection acquired during infancy and early childhood.

The development of varicella zoster infection was studied in a population of infants under one year of age during three outbreaks of varicella in a semi-closed domiciliary institution for infants in Japan. Over a period of four years, many residents ranging in age from 27 days to 32 months were tested for cutaneous reactivity to VZV antigen, and VZV-specific antibody activity before, during, and after each outbreak of varicella. Of these, 85 subjects developed clinical varicella, with an overall attack rate of 100% for all susceptible subjects. All the infants under 2 months of age were infected following such exposure, despite the presence of pre-existing maternal antibody. The degree of cutaneous involvement appeared to be milder (less than 20 vesicles) in infants less than 2 months of age, and severe cutaneous disease (with over 300 eruptions or confluent rash) occurred more frequently in subjects 2 to 11 months of age. Pre-existing antibody did not prevent development of illness, or alter the degree of antibody or cellular immune response to subsequent infection. However, the peak cutaneous reactivity to VZV antigen after infection was found to be significantly lower in infants under 2 months of age.

Antibody response to varicella-zoster virus after natural or vaccine-induced infection.

The development of serum and nasopharyngeal antibody responses to varicella-zoster virus (VZV) was studied in groups of children after naturally acquired varicella or after immunization with the Oka strain of live attenuated VZV vaccine administered in varying doses via respiratory inhalation or subcutaneous injection. Natural infection, subcutaneous immunization, and respiratory inhalation of large doses of VZV vaccine consistently resulted in the development of VZV-specific IgG antibody responses in serum. Although the serum IgG antibody responses persisted for at least eight to 12 months (to date) after either form of infection, the antibody activity appeared to be four- to eight-fold higher after natural infection than after immunization. Transient IgG antibody responses were observed in serum after respiratory inhalation of smaller doses of VZV vaccine. Natural infection, but not VZV vaccine, was associated with the development of serum and nasopharyngeal IgA responses to VZV in most subjects.

Noninvasive visualization of the dilated main coronary arteries in coronary artery fistulas by cross-sectional echocardiography.

Real-time cross-sectional echocardiographic studies of the main coronary arteries were performed in 20 normal subjects, 12 patients with patent ductus arteriosus and 14 patients with coronary artery fistula in whom the diagnosis was established by angiography. In 12 patients, the coronary artery that formed the fistula was dilated: The right coronary artery was involved in eight and the left coronary artery in four. The dilated coronary artery appeared as two dominant parallel echoes of wide lumen originating from the aorta in the region of the involved artery. Th echo diameter of the coronary artery correlated well with the angiographically estimated diameter of the artery. In the normal subjects and the patients with patent ductus arteriosus, we found no echocardiographic findings of coronary artery dilatation. This study demonstrates that cross-sectional echocardiography is useful in identifying the dilated coronary artery in coronary artery fistula and distinguishing this entity from patent ductus arteriosus.

Trial of split-product trivalent influenza vaccine in high-risk children.

Split-product trivalent influenza vaccine, containing H3N2, H1N1 and B, was given to 188 high-risk children and 25 healthy ones in the 4 year period from 1978 to 1981. A field trial involving 80 of these children was performed for evaluating the safety and immunogenicity of this vaccine. The antibody response to influenza vaccine in the patients was similar to that in normal children. The reaction to this vaccine of high-risk children was low, and no severe clinical reactions were observed after vaccination.

Combined passive and active immunization for preventing perinatal transmission of hepatitis B virus carrier state.

Prevention of perinatal transmission of hepatitis B virus carrier state in neonates at high risk was attempted by a combined passive and active immunization. Immediately after delivery, ten babies born to mothers who were asymptomatic carriers of hepatitis B surface antigen (HBsAg) and seropositive for hepatitis B e antigen received an intravenous injection of F(ab')2 fragments (200 IU) derived from hepatitis B immune globulin (HBIG). On the following day, none of them revealed detectable levels of the antibody to HBsAg in their sera, and received an intramuscular injection of HBIG (200 IU) which was repeated at 2 and 4 months of age. Vaccination with 40 micrograms of purified, formalin-inactivated HBsAg particles was given to the nine babies at three months and repeated at 4, 5, and 7 months after birth. All of them maintained detectable levels of the antibody and escaped infection throughout the first 12 months of their lives. The one baby who did not have detectable F(ab')2 in serum for 24 hours developed persistent HBs antigenemia which was noticed as early as seven days after birth.

Annual of pathological autopsy cases in Japan. Computerization of autopsy data from 1974 to 1979 and their statistical study.

The Japanese Pathological Society has established a unique registration system for autopsy cases since 1958. The autopsy data are published every year as an annual which numbers 22 volumes and includes 443,604 cases. This annual entitled Annual of Pathological Autopsy Cases in Japan is composed of 2 parts. One is the description part in which autopsy records of individual cases (institution, autopsy number, age, sex, address, occupation, clinics, clinical diagnosis, pathological diagnosis, and remarks) are summarized in a formula with 108 Japanese characters. The other is the statistical part in which disease-specific frequency with age distribution, combination of multiple malignancies are recorded. For this statistical study, computerization of the autopsy data was started from 1974. Modified MOTNAC codes are used for malignant neoplasms and modified ICD codes for the other diseases. Computerization of autopsy data has led to accurate statistical studies of autopsy cases in Japan and will contribute to the development of medical science.
