In utero diagnosis of Gaucher disease.

Beta-Glucosidase activity measured by synthetic substrate at pH 4.6 was low in the cultured amniotic cells from two pregnant women at risk for juvenile and adult type Gaucher disease. The diagnosis was confirmed by showing a low activity of beta-glucosidase in the skin fibroblasts with a synthetic substrate or in the spleen with a natural substrate, and by ascertaining the presence of Gaucher cells in the fetal tissues. However, considerable activity of beta-glucosidase measured with synthetic substrate was found in the liver of both affected fetuses and in the spleen of one. It is advisable that the determination of beta-glucosidase to confirm prenatal diagnosis of Gaucher disease be done either in the cultured skin fibroblasts or in the spleen, and if in the spleen, with a natural substrate rather than a synthetic one.

Bile lysosomal enzymes: characteristics and pathological significance for various hepatobiliary disorders.

The activities of several glycosidases (beta-galactosidase, beta-glucosidase, N-acetyl-beta-glucosaminidase) were demonstrated in human bile. The enzyme activities are increased about 100 times after exclusion of bile salts and other small molecular compounds by Sephadex G-50 gel filtration. The use of 4-methylumbelliferyl derivatives as substrates was useful as measurement of the bile enzyme activities are not altered in the presence of bile pigments. Enzyme characteristics of bile glycosidases were determined: pH optimum and isoelectric point. The bile glycosidase activities were also measured in various hepatobiliary disorders (cholelithiasis, cancer of gallbladder, acute hepatitis, liver cirrhosis and fatty liver). The glycosidase activities in bile from patients with liver diseases, as well as with cholelithiasis, were generally decreased. Isoelectric focusing patterns of biliary glycosidases were similar for specimens from patients with hepatobiliary disorders as compared to normal.

Five-year survivors among children with acute leukemia in Japan.

This Japanese nation-wide survey was carried out on long-term survivors among children with leukemia, i.e. those who had survived more than 5 years by October of 1974. Eighty-three children were surveyed. This was about 2.5 times as many as the 31 children surveyed in the first Japanese nation-wide survey in 1971. Clinical and laboratory data were also analyzed statistically and compared to that of the first survey. Most of the children had received inductive therapy according to the old protocol, but 10 children were treated with a combination of VCR and steroids. Nine of these 10 were still in a state of remission. CNS involvement occurred in 15.3% of the cases and a late onset of this complication implied a prognostic significance: among the 9 children whose complication had occurred 3.5 years after onset of the disease, 6 died. The relapse rate decreased between the 4th and 5th year after initiation of maintenance therapy. The percentage of the risk of death became zero after the 9th year.

Breast cancer incidence according to weight and height in two cities of the Netherlands and in Aichi prefecture, Japan.

A study was made in the cities of Rotterdam and The Hague, the Netherlands, and in Aichi prefecture, Japan, to assess the effect of weight and height (and their combinations) on the age-specific incidence of breast cancer. It is based on a comparison between 1006 cases of breast cancer and 4201 women from the general population, 33-69 years old. The results suggest that about one-half of the differences in incidence between the regions in Holland and Japan can be attributed to differences in body weight and height. In breast cancer patients in Rotterdam and The Hague those with metastases in axillary involvement. A hormonal factor related to body weight and/or height might be responsible for the increased incidence and the more rapid course of breast cancer in women with a large body mass.

Induction of hepatic tumors in a teleost (Oryzias latipes) after treatment with methylazoxymethanol acetate: brief communication.

Liver neoplasms including trabecular hepatoma and cholangioma were induced in medakas (Oryzias latipes) by the addition of methylazoxymethanol acetate to their aquarium water at levels of 0.1-3 ppm for periods ranging from 1 to 120 days. More than 80% of the surviving fish developed tumors at 3 or 5 months after commencement of the treatment, irrespective of levels of the drug, when appropriate times of exposure were chosen.

Immunofluorescent studies of urinary casts.

Immunofluorescent staining of urinary casts of patients with chronic glomerulonephritis or with acute renal failure was performed. Urinary and cast Tamm-Horsfall mucoproteins were purified by a modified McQueen's method. Rabbits were immunized by both of these materials to obtain antisera. The antigenic specificity of the prepared material was examined by Ouchterlony gel diffusion method and immunoelectrophoresis. Indirect and direct immunofluorescent staining methods were done using Tamm-Horsfall mucoprotein antiserum and FITC-GARG, FITC-labelled IgG, IgA, IgM, C3 and albumin. Hyaline casts of both chronic glomerulonephritis and acute renal failure demonstrated fluorescence with Tamm-Horsfall mucoprotein antiserum. The nature of the granular casts of chronic glomerulonephritis seemed to be different from those of acute renal failure because the granules of the former fluoresced for some serum protein fractions, but the latter fluoresced only for Tamm-Horsfall mucoprotein. The absence of fluorescence with epithelial casts for any serum protein fractions implied a different pathogenesis for epithelial casts as compared to granular casts, contrary to Lippman's assumption. A routine technique of immunofluorescent staining of urinary casts was developed. The interpretation of urinary casts should be more specific by this new technique.

Lung cancer and air pollution.

The relationship between incidence of lung cancer and the volume of traffic as indicated by auto exhaust concentration was examined; the results, though suggestive, did not yield consistent evidence of the association between them. Traffic jams in Nagoya began 15 years ago, a period that may not be long enough to provide definitive data on the incidence of lung cancer. The high standardized mortality ratio (SMR) of lung cancer was observed in cities with a population of less than 1 million and guns (rural areas) along the coast, although those in the metropolitan areas with populations of more than 1 million were average. The SMR did not correlate with various socioeconomic conditions and industrial air pollution. Meteorologic or geologic conditions and ocean currents were not associated with SMR of lung cancer by city and gun. The population of a gun or of some cities was not large enough to be statistically significant, and the mortality rate of lung cancer was not always stable.
