Evaluation of conservative therapeutic modalities for hepatocellular carcinoma--analysis of 206 cases.

The clinical effectiveness of conservative therapeutic modalities for hepatocellular carcinoma (HCC) was evaluated in terms of extension of survival. The therapeutic methods included one-shot therapy (OST) using Mitomycin C (MMC), Adriamycin (ADM), simultaneous ADM & MMC, with or without transcatheter arterial embolization (TAE). Prior to estimating the effectiveness, the subjects were graded into three stages according to the pretreatment severity of their residual liver function, based on total bilirubin, aspartate aminotransferase/alanine aminotransferase ratio, and ascites as constituent factors. OST with or without TAE significantly prolonged the mean survival time in stage I cases in good condition and in stage II cases in fair condition, but not in stage III cases in poor condition. Concerning OST without TAE, the results of ADM were slightly better than MMC in terms of extension of survival. OST combined with TAE was far more effective than OST without TAE. Extension of survival by simultaneous ADM & MMC is now under observation, but the toxicity of the modality has so far not proved serious. The long-term influence of repeated TAE on liver function was revealed to be mild within an average observation period of approximately one year. This study confirmed the validity of the present staging system in evaluating the efficacy of OST and TAE in terms of extension of survival.

Cholinergic system and constructional praxis: a further study of physostigmine in Alzheimer's disease.

Subcutaneous or intravenous administration of physostigmine improved the copying of geometric figures in three out of six patients with presumed Alzheimer's disease who showed a remarkable constructional disturbance. This improvement reached its maximum 30 to 60 minutes after the physostigmine administration. These results not only provide further evidence supporting the hypothesis that central cholinergic potentiation ameliorates some of the impairments of Alzheimer's disease, but implicate a possible important role of the cholinergic system in the integration of constructional praxis.

Sodium susceptibility and potassium effects in young patients with borderline hypertension.

To evaluate sodium susceptibility in subjects with borderline hypertension at increased risk of developing essential hypertension, the effect of salt loading after sodium deprivation with a diuretic was studied in 21 young patients with borderline hypertension and 12 age-matched normal subjects. Treatment with a diuretic caused significant decreases in mean blood pressure (MBP) in subjects with borderline hypertension but not in normotensive subjects. In borderline hypertensives, the subsequent sodium loads resulted in a significant rise in MBP (5.8 +/- 1.7%; p less than .01), but sodium did not change MBP in normotensives. There is a good correlation between the increments in MBP with sodium loads and the decrements in MBP with a diuretic for each patient (r = -.759, p less than .001). After diuretics, cardiac index (CI) as measured echocardiographically fell significantly but calculated total peripheral resistance (TPR) remained unchanged in subjects with borderline hypertension. After 180 meq sodium chloride each day was added for 7 days, CI (9.1 +/- 2.1%; p less than .01) and stroke index (21.0 +/- 3.4%; p less than .01) rose significantly but TPR remained unchanged. Overall, the increments of MBP with sodium loads did not correlate with the changes in CI but did correlate with the changes in TPR (r = .567, p less than .01). In these young patients with borderline hypertension, plasma norepinephrine and epinephrine concentrations and plasma renin activity (PRA) were significantly higher than in normotensive subjects.(ABSTRACT TRUNCATED AT 250 WORDS)

[The antiataxic mechanism of TRH in rolling mouse Nagoya: the effects of pretreatment with dopaminergic and cholinergic drugs].

Antiataxic mechanisms were investigated in Rolling mouse Nagoya (RMN). The present study was to elucidate the influence of dopaminergic (pimozide, apomorphine) and cholinergic (atropine, physostigmine) drugs on the antiataxic effect of TRH. The degree of ataxic gait and spontaneous motor activities in RMN were measured by the open field method and ANIMEX-II Pretreatment with pimozide and apomorphine had no influence on the antiataxic effects of TRH, while pretreatment with physostigmine suppressed these effects and in contrast, with atropine, increased then. The increase of spontaneous motor activities after TRH injection was antagonized by pretreatment with pimozide and physostigmine, but accentuated by pretreatment with atropine. These results may indicate that the antiataxic effects of TRH are, at least partially, mediated by the cholinergic mechanism.

[Prostatic acid phosphatase by enzymeimmunoassay].

We measured prostatic acid phosphatase levels by enzymeimmunoassay (PAP-EIA). Intraassay reproducibility of PAP-EIA was markedly good. In thirty normal males, PAP-EIA levels ranged from 0.24 ng/ml to 3.3 ng/ml, mean and S.D. being 0.94 ng/ml, 0.50 ng/ml, respectively. We made the upper limit of PAP-EIA for the normal range, 1.94 ng/ml (Mean + 2 S.D.). O 40 patients with untreated prostatic cancer, 34 patients (85%) gave positive results (1/3 33% of stage A, 3/4 75% B, 10/11 90% C, 20/22 90% D). One out of 12 patients with BPH, and one out of 11 with prostatitis gave positive results. The false positive rate was 9%. PAP levels of 324 samples from 111 patients with prostatic cancer were measured by EIA and radioimmunoassay (RIA). A significant correlation was noted between EIA and RIA (r = 0.997 p less than 0.001).

[Clinical experience of bromazepam for psychogenic impotence patients].

A clinical trial was performed with the tranquilizer Bromazepam on 39 patients undergoing diagnosis of psychogenic impotence, and the drug effect was evaluated according to the criteria based on our protocol. The protocol specifies 8 tests (1 for libido, 4 for erection, 2 for ejaculation, 1 for orgasm) which are scored according to an arbitrary logarithmic scoring system. At the end of the study the points made in the 8 tests were added to obtain the total score as the basis for evaluation of the overall drug effect. After treatment all tests showed an improvement, and the improvement in erection during masturbation, reflective erection, and condition of ejaculation was statistically significant. The total score also improved from 16.77 +/- 2.62 (mean +/- S.D.) before treatment to 11.42 +/- 1.96 after treatment, and the change was again statistically significant (P less than 0.05). The rate of satisfaction as a subjective symptom of improvement also increased from 25.38 +/- 4.40% to 39.10 +/- 4.53%. The results of the present study provide evidence to indicate that Bromazepam is beneficial for psychogenic impotence.

[A case of primary lipogranuloma of the spermatic cord].

A 54-year-old male who noticed a growing mass in his right inguinal region one month earlier visited our clinic with a pigeon-egg sized, hard elastic, and movable mass with an unsmoothed surface. The removed mass was 4 X 2 X 2.5 cm, weighed 11.5 g and its cut surface was greyish white. The histopathological diagnosis was primary lipogranuloma. Postoperative course was uneventful with no sign of recurrence one year later.

[Floating knee fracture (ipsilateral fracture of the femur and tibia)--treatment by closed Ender nailing].

Fourteen cases of ipsilateral fractures of the femur and tibia (floating knee fracture) were reviewed and patients were graded according to the type of fracture and the method of treatment. Using the criteria for assessment described by Karlström and Olerud, patients were graded as excellent, good, fair and poor. Excellent results were achieved in five patients, good in three and fair in six. When the fracture extended into the knee, it caused knee problem in three out of five. In ten cases the femoral shaft fractures were nailed and in five both bones were nailed. Nailing of both femur and tibia gave good or excellent result in all five cases. As the comminution is often severe in the cases of floating knee fracture, intramedullary nailing by Küntscher may be difficult. We have introduced a closed Ender nailing for femoral and tibial shaft fractures and we have found that this method is also useful for the floating knee fracture. It is advantageous for this type of fracture, because it is technically simple, it has wide indications and it results in rapid bone union without knee disturbance.

Curious manifestations in cavernous hemangioma of the liver.

We report a cavernous hemangioma of the liver with some curious findings. Angiography showed a hypervascular lesion and the arterioportal shunt which commonly is regarded as a pathognomonic sign of malignancy. Laparoscopy showed a well-defined, reddish-purple elevated lesion with a central umbilication. As the patient was a painter and dealing with vinyl chloride monomer, we suspected the possibility of hemangiosarcoma. The lesion proved to be a cavernous hemangioma with central fibrous replacement probably in consequence of thrombosis of some cavernous spaces.

[Does fast-neutron radiotherapy merely reduce the radiation dosage?].

We examined whether fast-neutron radiotherapy is superior to low-LET radiotherapy by comparing the relationship between cell survival and tumor control probabilities after exposure of tumor-bearing (species) to the two modalities. Analysis based on TCD50 assay and lung colony assay indicated that single dose of fast neutron achieved animal cures at higher survival rates than other radiation modalities including single and fractionated gamma-ray doses, fractionated doses of fast neutron, and the mixed-beam scheme with a sequence of N-gamma-gamma-gamma-N. We conclude that fast-neutron radiotherapy cured animal tumors with lower cell killing rates other radiation modalities.
