The effect of sodium and potassium loading on urinary kallikrein-like activity in young patients with borderline hypertension.

We studied the effects of a potassium supplement on urinary kallikrein excretion in a setting of high sodium intake after sodium deprivation with diuretics in young patients with borderline hypertension. Eleven patients, who took the potassium supplementation during the high sodium diet period, showed lower increments in mean blood pressure with salt loading than 12 patients without the potassium supplementation. In the non-potassium-supplemented patients, urinary kallikrein was increased significantly when plasma renin activity (PRA), plasma aldosterone concentration (PAC), and urinary aldosterone were increased during the diuretic treatment. It was decreased significantly when the other hormones were decreased during the sodium load. During the high sodium diet period, PRA, PAC and urinary aldosterone were greater in the potassium-supplemented patients than in the non-potassium-supplemented ones, but urinary kallikrein excretion was not higher when potassium was supplemented. Thus, the present results did not support the theory that the kallikrein-kinin system may be involved in the natriuretic and antihypertensive effects of potassium. In addition, these finding suggest that some kallikrein-modulating factor(s) may counteract the increased urinary kallikrein excretion with the augmented renin-angiotensin-aldosterone system during salt loading with potassium supplementation.

Abnormal renal hemodynamics in salt-sensitive patients with essential hypertension.

Thirty hypertensive patients were studied during a low-sodium diet for three days and a high-sodium diet for six days. They were classified as "salt-sensitive" (SS) (n = 10) or "non-salt-sensitive" (NSS) (n = 20) based on the increase in mean blood pressure (BP) with changes in sodium intake from 25 mEq/day to 250 mEq/day (14.7 +/- 1.3% versus 4.0 +/- 0.8%, p less than 0.001). With the high-sodium diet, the SS patients had a greater increase in cardiac output (p less than 0.001), and the increment in mean BP with the salt loading significantly correlated to that in cardiac output (r = 0.672, p less than 0.01). Thus, the increase in BP during short-term salt loading may be attributed to the increase in cardiac output. The role of renal hemodynamics in the increment of BP with salt loading after sodium restriction was evaluated in six SS and 14 NSS patients. Although the glomerular filtration rate was not different in the two groups, the renal vascular resistance was greater (p less than 0.05) in the SS patients than in the NSS ones. Moreover, renal vascular resistance positively correlated to the increment in mean BP with salt loading (r = 0.612, p less than 0.01). Thus, it is suggested that renal vasoconstriction may be an important factor influencing "salt-sensitivity" in essential hypertension, possibly via the impaired renal sodium excretion.

An ascochlorin derivative, AS-6, reduces insulin resistance in the genetically obese diabetic mouse, db/db.

An ascochlorin derivative, AS-6, is a new hypoglycemic agent orally active in both obese hyperinsulinemic and insulin-deficient diabetic animal models. AS-6, when given as a 0.025-0.2% admixture in the diet, dose-dependently ameliorated polydipsia, polyuria, and glycosuria in the genetically obese diabetic mouse, C57BL/KsJ db/db, while neither insulin nor tolbutamide showed any beneficial effects. The amelioration by AS-6 was associated with a marked decrease in serum glucose and triglyceride. The effects persisted at least 10 wk, accompanied by a steady decrease in drinking water consumption. The chronic treatment prevented pancreatic islet degeneration, e.g., degranulation of the beta-cells, basophilic appearance of the exocrine border around the islets, and small round cell infiltration. The isolated islets from AS-6-treated mice released much more insulin in response to glucose than those from untreated controls. A significant correlation between serum immunoreactive insulin and glucose/triglyceride from both treated and untreated mice suggests that AS-6 restores sensitivity and responsiveness to insulin to the mice. In fact, the combined treatment with insulin synergistically decreased serum glucose by 50% below AS-6 treatment alone. Furthermore, the epididymal fat pad slices from AS-6-treated db/db mice increased CO2 generation and lipogenesis over the untreated controls, and the glucose metabolic rate (CO2 generation plus lipogenesis from U-[14C]-glucose) in the slices and the serum glucose level inversely correlated at r = 0.8799.(ABSTRACT TRUNCATED AT 250 WORDS)

Zoonotic subcutaneous filaria of undetermined classification.

A necrotic adult male filaria was found in sections of a dense, fibrous, subcutaneous nodule excised from below the breast of a 61-year-old woman, resident of Mie Prefecture in Honshu, Japan. Owing to its necrotic condition, and to a lack of essential knowledge of the comparative micro-anatomy of filarial worms, a differentiation between Dirofilaria and Dipetalonema species, or member of a related group, was not possible.

[Reactivation of prostatic cancer and chemotherapy in reactivated prostatic cancer].

From January, 1960 to March, 1984, we studied 65 cases of prostatic cancer resistant to endocrine therapy. Treatment with ifosfamide or combination chemotherapy with vincristine, ifosfamide and peplomycin was performed in some of the 65 cases above mentioned. More cases were systemic reactivation than local reactivation. Poorly differentiated cancer accounted for the majority of the reactivated cancer. Concerning adjacent effects, treatment with ifosfamide was superior to combination chemotherapy. There was no difference between the distant effects of both chemotherapies. The patients with no effects showed predominance of pathologically low differentiated cell type. The 6-month survival rate of patients with chemotherapy was significantly higher than that of patients with other therapies. Toxicities of chemotherapies were leucopenia, digestive disturbances and falling out of hair. Hematuria and pulmonary fibrosis occurred in some cases.

Increased radiosensitivity of a recurrent murine fibrosarcoma following radiotherapy.

The radiosensitivity of a recurrent tumor following local radiotherapy was investigated in order to determine whether or not the cell(s) surviving radiotherapy are radioresistant. A tumor which recurred in a C3Hf/HeMsNrs male mouse 200 days after local irradiation of a transplanted syngeneic fibrosarcoma was examined. The recurrent tumor showed the same biological properties, such as karyotype, histological features, and ability to produce colony stimulation factor, as the original tumor. On the other hand, the tumor control dose and the D0 value of the recurrent tumor were much smaller than those of the original tumor. Moreover, the recurrent tumor grew more slowly than the original tumor. It was concluded that initial radiotherapy might have changed the intrinsic radiosensitivity of the original tumor cells.

[Fast neutron radiotherapy at NIRS, indication and prospects].

Eleven hundred and seventy one patients, 921 previously untreated and 250 recurrent, have been treated with 30 MeV (d-Be) fast neutron beam between 1975 and 1984 at NIRS. Some trends have been identified: non-randomized results have been at least as good as those of photons in carcinomas such as: supraglottic carcinoma in the larynx, pancoast type tumor of the lung, malignant melanoma of the skin and so on. Randomized results with mixed beam studies for carcinoma of the uterine cervix have indicated no significant advantages against those of photons. Treatment technique for the beam concentration must be improved to demonstrate the merit of neutrons in the next step.

[Clinical examination of thymic abnormalities and thymectomy in patients with ulcerative colitis].

Results and significance of thymectomy in 66 patients with ulcerative colitis were studied. The thymus delineated by means of pneumomediastinography was almost always successfully removed by our procedure of thymectomy via the transverse incision in the suprasternal notch, frequently combined with dissection by the mediastinoscope through the parasternal incision. Sixty-six thymectomized patients with ulcerative colitis consisted of 29 females and 37 males, ranging in age from 13 to 62 years. In the resected thymuses, lymphoid follicle formations, which are often encountered in high incidence in the thymus obtained from other autoimmune diseases, were found in over 40% of cases. Follow-up observations, ranging from 1 year to 12 years after thymectomy, revealed 84% inactive condition (counted by patient month) compared with that of these same patients before operation.

Structural and functional abnormalities in the adipocyte plasma membrane from db/db mouse, and the effect on the abnormalities of oral treatment with AS-6.

The protein bands of adipocyte plasma membranes from the genetically obese diabetic mice C57BL/KsJ db/db (db/db mice) showed slight but significant changes compared with their lean littermates. The treatment for 1 week with a new antidiabetic agent, AS-6, caused the changes to revert toward the condition in the lean littermates. In the absence of insulin, the plasma membrane and mitochondria mixture (P3 fraction) of the lean littermates densely labeled 55000 and 57000 dalton protein bands by phosphorylating with (a-32P)-ATP, whereas the labeling was less in the P3 from AS-6 treated and untreated db/db mice. Insulin inhibited phosphorylation of these bands in P3 from the lean littermates and untreated db/db mice, while the hormone enhanced the labeling in AS-6 treated db/db mice compared with the basal condition without insulin. Ca2+ greatly enhanced the labeling in all three groups, whereas Mg2+ mimicked the insulin action diminishing the labeling of these bands in the lean and untreated db/db groups. However, Mg2+ enhanced the phosphorylation in the P3 from AS-6 treated db/db mice compared with the basal condition.

Genetic controls of T cell-independent Thy-1 alloantibody responses.

Early and late primary IgM antibody responses of mice to Thy-1.1 antigens showed different antigenic and cellular requirements. We studied genetic controls of the early primary responses, which could be induced by subcellular thymocyte antigens independently of host T-cell activity. All Thy-1.2 mouse strains of Igha (BALB/c and BC8), Igh-VaCb (BAB14), Ighd (AKR/Cum), Ighj (CBA/J, C3H/HeN, C3H.SW, and C3H.JK), and Ighn (NZB) definitely responded early to Thy-1.1 antigens from AKR/J (Ighd), A.Thy-1.1 (Ighe), or B10.Thy-1.1 (Ighb) mice or SD rats, whereas all strains of Ighb (C57BL/6, C57BL/10, B10.D2, B10.BR, B10.A, CB20 and CWB), Ighc (DBA/2), Ighe (A/J), and Igho (C.AL20) responded poorly to the same antigens. This contrasts with the observation that both strains of Ighj (C3H/HeN) and Ighb (B10.BR) responded well at later times. As was the case for late responses, the matching of H-2 between donor and recipient resulted in early responses of exceptional quality in high-responder strains. It was concluded that under the influence of H-2, whose incompatibility between donor and recipient partially interferes with responses, early but not late primary Thy-1.1-specific antibody responses are selectively controlled by Igh-V or closely linked Ir gene(s) as a new VH marker.
