An observational study of sun and heat protection during Canada Day outdoor celebration, 2003.

Attendance at summer outdoor mass gatherings may lead to heat- and sun-related illness. The purposes of this study were: (1) to estimate the proportion of people in attendance at the 2003 Canada Day celebration in the National Capital Region who used sun and heat protective items; (2) to identify factors associated with the utilization of these protective items; and (3) to provide research data to public outdoor event organizers when developing evidence-based plans for safer events. A naturalistic observational cross-sectional method was used to gather information at the 2003 Canada Day celebration in the National Capital Region on attendees' demographics, the sun and heat protective items they used and the protective resources available at the event sites. Of the 398 observed attendees, the proportion using any one of the protective items ranged from 3 percent (an open umbrella) to 51.5 percent (sunglasses). Females were more likely to use protective items more than males, and adults more likely than children. Planners of public outdoor events should consider the factors that influence the utilization of sun and heat protective behaviours and the environmental modifications that would allow participants to make safe choices.

[Mortality by birth weight and survival by gestational age and route of delivery in premature infants].

Specific mortality rates for birthweight (BW) and survival for gestational age were determined in babies under 1500 g and less than 34 weeks gestational age (GA), which were born in a private hospital at Santiago, Chile, between 1983 and 1988. Mortality rates were 875/1,000 in babies under 750 g, 391/1,000 for BW between 750 and 999 g, 185/1,000 for BW 1,000 to 1,249 g, and 125/1,000 in newborns who weighed 1,250 to 1,499 g. Survival increased with gestational age from 39% under 26 weeks to 57% at 26 and 27 weeks, reaching 83% at 28 and 29 weeks, 90% at 30 and 31 and 87% at 32 and 33 weeks. No significant difference was found neither in mortality nor in intracranial hemorrhage (ICH) incidence between newborns delivered by cesarean section or vaginal way. Survival of babies born after 26 weeks GA and weighing over 750 g was comparable with that reported by others. The type of delivery is not likely to play a role by itself in mortality or in incidence of severe ICH.

Occlusal contacts in mandibular retrusion.

An investigation into mandibular retrusion and the relationship between the maximum intercuspal position and the most retruded mandibular position was conducted on 15 dental students and nurses who had reasonably intact dentitions and a well-defined intercuspation. A photographic method was used to measure the retrusion that occurred at the occlusal surfaces of the posterior teeth. An average of approximately 0.2 mm retrusion was found, and the explanation of the relatively large "apparent" retrusion led to a discussion of the mandibular axes of opening.

Holistic physicians and family practitioners: an empirical comparison.

We report on a survey of 340 self-identified, holistic physicians (members of the American Holistic Medical Association) and a group of 142 California family practitioners (FPs). The groups are quite similar in age, gender, socioeconomic origin, and marital status. Holistic physicians are more likely to be from rural origins, not board certified, in private practice, see fewer patients, and use hospitals less. Holistic physicians also evaluate and utilize a number of "holistic" medical techniques and interactional styles with patients quite differently than FPs. In addition, the two groups are differentiated by the importance they attribute to religious and/or spiritual factors, as well as their own experiences in psychotherapeutic situations, that influence their views of health, illness, and healing.

The use of patch angioplasty to alter the incidence of carotid restenosis following thromboendarterectomy.

Eighty-nine consecutive patients were followed for twelve to twenty-four months after carotid thromboendarterectomy. Forty-seven underwent operations using a standard carotid closure; a second group of forty-two patients had a saphenous vein patch graft applied in closure. The patients were evaluated with carotid duplex scans at regular intervals to evaluate the incidence of early restenosis. Patients with positive duplex scans underwent arterial angiography or digital subtraction angiography. Operative mortality was zero. Twelve late deaths occurred; eleven due to cardiac disease. In the unpatched group, 19.1% of the patients developed significant restenosis (a luminal narrowing of greater than 50%) with only a 2.4% incidence occurring in the patched group, a statistically significant difference (p less that 0.05). There was a higher incidence of symptomatic restenosis in the unpatched group, though the results were not considered significant. Venous patch rupture was seen in three patients who have not been included in the study group. All of these had venous patches harvested from a distal greater saphenous vein site at the ankle. The authors recommend venous patch closure with a thicker patch taken from the proximal saphenous vein at the groin to reduce the incidence of early restenosis and to avoid venous patch rupture. Regular non-invasive evaluation is recommended to detect restenosis with careful clinical follow-up to evaluate neurologic complications.
