Extradural morphine for the relief of pain following caesarean section.

Thirty patients took part in a double-blind trial to compare morphine 10 mg i.m. with morphine 5 mg into the extradural space for pain relief following Caesarean section. With the extradural route the mean time to first analgesia was 7.95 h compared with 4.75 h for the i.m. route. The mean number of further doses of diamorphine 7.5 mg required for pain relief in the first 24 h was 1.93 (+/- 0.30) in the i.m. group and 1.46 (+/- 0.31) in the extradural group. There was a small but definite advantage in the use of the extradural route for the initial dose of morphine. No serious side-effects were noted in either group.

Involvement of the central nervous system in congenital muscular dystrophies.

Three children, two siblings and one unrelated child, with congenital muscular dystrophy with central nervous system (CNS) involvement are discussed. The siblings appeared to suffer from a relatively mild myopathy with progressive brain disease, of which brain biopsy in one showed astrocytic proliferation in the white matter. In the patient with severe muscle disease, autopsy showed widespread patchy demyelination in the white matter and developmental abnormalities in the cerebral and cerebellar cortex. These patients differ from the Japanese (Fukuyama) cases of CMD in the severity of the changes in the cerebral white matter, and from Santavuori's cases in the absence of ocular abnormalities and hydrocephalus. Their unique nosology is discussed.

Dissection of the aorta in Turner's syndrome.

Three deaths from dissection of the aorta in a series of 157 adult women with Turner's syndrome are reported. These are greatly in excess of the numbers expected. None of the three patients had a coarctation of the aorta. One had aortic regurgitation but there was no reason to believe that the aorta in the other two patients had been subjected to unusual haemodynamic stresses. Cystic medial necrosis of the aorta was described in two patients on whom necropsies were carried out. It is concluded that there is probably a greatly increased risk of dissection of the aorta in Turner's syndrome even in the absence of any other abnormality of the aorta and aortic valve. Previously reported cases of aortic dissection in Turner's syndrome are discussed.

Readability testing of auxiliary labels.

Using a word familiarity index, we studied pharmacy auxiliary labels and selected brief information sheets to determine the reading difficulty of these forms of written communication, which cannot be evaluated by traditional readability testing methods. Results of the study show that 11.2 percent of the 110 words selected from these information sources are above the eighth-grade reading level. The word familiarity index also identified multiple meanings of words and appears to be a valuable tool for evaluating written information that is not suitable for traditional readability testing.

Effect of charcoal-treated bovine follicular fluid on secretion of LH and FSH in ovariectomized heifers.

The objective of the experiment was to determine whether pools of charcoal-treated bovine follicular fluid (bFF) removed from large (greater than 10 mm) follicles during the preovulatory (d 18 to 1), postovulatory (d 2 to 6) or mid-cycle (d 7 to 17) stages of the bovine estrous cycle altered secretion of luteinizing hormone (LH) or follicle-stimulating hormone (FSH) in ovariectomized heifers. After a 20-ml injection of bFF given at three consecutive 6-h intervals, secretion of FSH was depressed from approximately 116 to 53 ng/ml by each pool of bFF. In contrast, bFF did not affect secretion of LH. Serum from an ovariectomized cow (control) had no effect on secretion of LH or FSH. We concluded that bFF contained a nonsteroidal component (perhaps inhibin) that suppressed FSH secretion in heifers.

Spiral nerve endings and dapple motor end plates in monkey extra-ocular muscles.

The suggestion that spiral nerve endings in extra-ocular muscles are sensory was tested. Spiral, serpentine and pincer-like endings were identified in extra-ocular muscles of rhesus and cynomolgus monkeys in light and electron microscopic preparations. Spiral and other complex nerve endings each terminated in the motor end plate zone as well separated clusters of boutons applied to various aspects of the muscle fibre, in contrast to the single group of boutons of other endings. They displayed a dappled appearance with acetylcholinesterase stain. Dapple endings had morphological features and staining characteristics consistent with motor end plates. Hence, spiralling of nerve fibre endings is shown to be a device for conveying boutons to dapple motor end plates rather than for monitoring the contraction of muscle fibres.

An X-linked syndrome characterised by hyperuricaemia, deafness, and neurodevelopmental abnormalities.

Purine overproduction, with normal levels of the purine salvage enzymes, has been found in a 3-year-old boy and his mother. Both have high-frequency hearing loss from infancy. The child is hypotonic and shows locomotor delay. Two male siblings with similar neurodevelopmental problems died in early childhood. Biochemical studies suggest a combined syndrome associated with a superactive PP-ribose-P synthetase which in its severest form may produce neurodevelopmental complications, as well as uric-acid overproduction. Severe depletion of erythrocyte nicotinamide adenine dinucleotide and guanosine triphosphate appears to be associated with the neurological abnormalities and may be useful in diagnosis.
