Pericarditis purulenta in children.

The case history of seven children aged 1 5/12 to 5 9/12 years with non tuberculous bacterial pericarditis, observed in the last 8 years at the University children's hospitals of Basle, Berne and Zurich is reported. The history showed febrile illness of 3--14 days duration, which led to an admission diagnosis of pneumonia, angina or pseudocroup. From the signs of heart failure and cardiomegaly on chest X-ray the differential diagnosis of myocardial disease or pericardial effusion was made. The ECG-changes were uncharacteristic, and a friction rub and pulsus paradoxus was encountered once only. The effusion diagnosis should preferably be substantiated by a non-invasive method (scintigram, echocardiogram) as diagnostic pericardiocentesis does often not allow to aspirate the thick pus through the needle. Diagnostic and therapeutic surgical pericardiotomy with consecutive drainage is therefore mandatory. Halothane should be avoided as an anesthetic for this procedure of hemodynamic reasons. With surgery and antibiotics the recovery rate in our series was 100%, and no pericardial constriction was observed on follow-up 1 to 8 years later.

[Analysis of two different surgical corrections of the coarctation of the aorta (follow up examination of 49 children) (author's transl)].

36 children operated upon with the diagnosis coarctation of the aorta have been controlled in a follow-up examination. We payed special attention to the surgical technique and brought the results in connection with the method of operation. One group had the stenosis corrected by resection and end-to-end-anastomosis, the second group with plastic operation, either by a longitudinal incision, resection of a diaphragm and transverse closure, or by improving the dilatation using a dacron patch. The results in both groups are satisfactory. Actually we prefer to carry out the resection with end-to-end anastomosis. Plastic correction remains reserved for selected cases. We believe that the high mortality rate in infants can be reduced by operation in hypothermy and by removing the stenosis as completely as possible.

[Pharmaco-angiography with angiotensin (author's transl)].

The results of pharmacoangiography using angiotensin were compared with the findings in 101 patients during selective angiography of the kidneys (48 cases), the pancreas and liver (46 cases) and of peripheral soft tissue lesions (7 cases). Angiotensin in a dose of 0.5--l mug (renal artery), 1--5 mug (coeliac artery) and 5--10 mug (peripheral vessels) was found to be a constant and potent vaso-constrictor in 99% of cases. Intraarterial injection of angiotensin improved evaluation in 18 out of 27 cases (66%) with a final diagnosis of a tumor, in 18 out of 22 patients (77%) with cysts and in 16 out of 24 (66%) with an inflammatory process. In three out of 27 cases (11%) an angiographic diagnosis was possible only after using pharmacoangiography. The use of angiotensin represents a valuable possibility of clarifying otherwise uncertain angiographic diagnoses. This is particularly the case where superselective catheterisation is impossible.

[Arthrography in humeroscapular peri-arthritis].

Degenerative changes in the shoulder joint affect the capsule much more frequently than might be expected and are not seen on a plain radiograph. Arthrography of the shoulder has become of increasing significance in view of modern surgical management. Arthrography is used particularly to demonstrate a tear of the rotator cuff (especially the supra spinatus and infra spinatus). It also shows other important abnormalities which cause pain on movement, such as adhesions, tight capsule and the formation of septa. The indications, technique and necessary records, as well as typical pathological findings, are demonstrated in forty shoulder arthrograms. Hemuro-scapular peri-arthritis is a collective term for complex, primarily degenerative or posttraumatic, changes including peri-articular calcification. Demonstration of calcification alone does not justify the radiological diagnosis of humeroscapular peri-arthritis.

Genetic analysis of adenovirus type 2 III. Temperature sensitivity of processing viral proteins.

Using sodium dodecyl sulfate-polyacrylamide slab gel electrophoresis of [35S]methionine-labeled adenovirus type 2-infected KB cell extracts, a total of 23 virus-induced polypeptides was detected. This technique was applied to the analysis of the temperature-sensitive mutant, ts 1, which has previously been shown to be defective in a late function. By means of pulse-chase experiments, ts 1 was shown to be defective in the processing of the precursor polypeptide (Pre VII) to the major core protein VII. Two other putative precursor polypeptides, Va (27K) and Vb (24K), were also not processed. Thus, the ts 1 mutation blocked the appearance of six post-translational clevage products, i. e., polypeptides VI, VII, VIII, X, XI, and XII. All of these polypeptides are virion components. Processing was temperature sensitive in a shift-up experiment, whereas it was normal in a shift-down experiment. The kinetics of the temperature-shift experiments suggested that infectious virus could be recovered if enough time is provided for processing to take place. Processing was not inhibited by cycloheximide. The analysis of purified virus particles and empty shells (TCs) revealed the presence of the precursor and putative precursor polypeptides Pre-VII, Va and Vb, instead of their cleavage products, in both types of particles. Based on these results we propose that the ts 1 gene codes for or regulates an endoprotease which is responsible for the completion of the last step in virus maturation, that is, the conversion of "young virions" into mature infectious virions by a series of maturation cleavages.

Properties of aerosols produced with three nebulizers.

Investigations on the physical factors influencing the efficiency of 3 kinds of nebulizers (Wiesbadener Doppelinhalator, Heyer jet nebulizer, and Monaghan ultrasonic nebulizer) used to administer pharmaceutical agents are described. The airflow, the waterflow, and the concentration of the nebulized pharmaceutical agents in the air were determined. It was found that, for the jet nebulizers, the concentration of nebulized pharmaceuticals decreased considerably during the vaporization. The particle size distribution was measured with spiral centrifuge. The mass median diameter of the unevaporated aerosols was 3.4 mum for the Wiesbadener, 7.4 mum for the Heyer, and 5.2 mum for the Monaghan. The deposition of an aerosol produced with the Wiesbadener was measured in a glass model of the upper airways (mouth, trachea, and bifurcation). An estimate of the fraction of the aerosol that can pass the bifurcation was made. A correction for the change in the aerosol particle diameter due to the high humidity in the human lung was introduced. It was found that 35 per cent of the mass of the aerosol produced with this nebulizer can pass the human bifurcation (tidal volume, 500 ml; inhalation frequency, 12 per min).

A new technique for the localization of the cervix at 113mIn placental scintigraphy.

Sixty women in the third trimester of pregnancy were examined with 113mIn placental scintigraphy because of bleeding. In 24 of the cases, the scintigraphic procedure was initiated by application of a Meyers ring containing 113mIn round cervix. In the other cases, the fundus and the symphysis pubis were marked by means of a small radioactive source. Complete placenta praevia was correctly detected in 8 cases and marginal placenta in 4 cases. The scintigraphic localization of the placenta was correct in all the women where the ring was used, whereas when the external marked was used, seven errors occurred. The authors therefore recommend this new technique in order to facilitate the evaluation of the scintigrams.

[Present status of pharmacoradiology of the gastrointestinal tract (author's transl)].

Based upon the findings in over 1200 patients, technique, indication and validity of pharmacoradiology in examinations of esophagus, stomach, duodenum, small and large intestine are critically evaluated and summarized. Dosis, effects, side effects and contraindications of the mostly applied pharmaca (Buscopan, Pro-Banthine, Paspertin, Glucagon, Cholecystokinin and Caerulein) are listed. The value of pharmacoradiology of the gastrointestinal tract for clinical roentgenology is stressed.

[Visceral and peripheral pharmacoangiography with angiotensin and bradykinin (author's transl)].

In a prospective study of 156 patients a total of 110 pharmacoangiographic examinations using Angiotensin and 46 investigations with Bradykinin were performed. In 21 cases both pharmaca were applied. In 111 patients with pathologic findings (tumors, cysts and inflammatory lesions) diagnostic information compared with conventional angiography, was improved in 80, equal in 26 and reduced in 5 cases. In 21 studied patients using Angiotensin and Bradykinin, the overall diagnostic information was improved. Angiotensin is a suitable pharmacon in demonstration of malignant lesions. Bradykinin improves the demonstration of vessels affected by local inflammation and allows better visualization of the veins. The application of both pharmaca may help in certain cases to differentiate between inflammatory and malignant lesions.

[Accurate reconstruction of traumatic bony contour defects of periobital area with prefabricated silastic].

The silicone inlay method of prefabricating subcutaneous and bony implants for congenital and acquired defects of the maxillofacial area was used in 25 cases. Heat vulcanized Silastic 372 or 373 was used, and seems to be satisfactory. The method is presented as a useful addition to the surgical restoration of subcutaneous and bony defects, particularly complex contours of the periorbital area when autogenous tissue is unavailable. Complications seem to be less common when the implant is immobile and secured to bone. The method is evolving, and modifications can and should be made in the technique.

Extra-hepatic storage of copper: a male foetus suspected of Menkes' disease.

The distribution of copper among the organs of an aborted, male foetus, expected to develop Menkes' syndrome, was entirely different from the distribution in 4 normal foetuses. Copper concentrations determined by neutron activation analysis showed a considerably reduced content in the liver, but increased concentrations in the other organs analysed; total foetal copper was normal.
