Renal functional changes in acute glomerulonephritis in children. A one-year follow-up.

Renal function was studied in three patients with post-streptococcal, four patients with IgA and one patient with non-streptococcal proliferative glomerulonephritis (GN) at the onset of the disease and two, six and 12 months later. Renal biopsies were performed at the onset of the disease and 12 months later. Standard clearance techniques were used for the functional studies. The latter were performed during hydropenia and continuous isotonic saline infusion. During hydropenia, the GFR was uniformly depressed shortly after the onset of the disease, but it normalized during the following two months. The filtration fraction was depressed in poststreptococcal GN at the onset and it normalized with the GFR. In IgA GN, the filtration fraction remained within normal limits during the entire course of the illness. The natriuretic response to isotonic saline volume expansion was low in all patients at the onset of the disease, but normalized in post-streptococcal and IgA GN during the one-year follow-up. In spite of normalized renal function, biopsy findings in IgA GN were unchanged 12 months later. An episode of macroscopic hematuria in one patient with IgA GN at the six-month investigation had no apparent effect on renal function.

Prognistic relevance of radiation induced immune suppression in breast carcinoma.

The extent of radiation induced immune suppression was analysed in 100 patients with carcinoma of the breast. The relative changes of lymphocyte counts and stimulations by PHA and PPD were similar in patients who differed with regard to age, size of tumour and its malignancy grade or axillary node condition. Moreover, no difference in the degree of radiation induced immune suppression existed between patients who developed recurrent disease and those who remained free of disease during a follow-up period of 4 1/2 to 7 years.

The effect of irradiation on T-cell suppression of ELISA-determined Ig production by human blood B-cells in vitro.

Human blood B-lymphocytes were co-cultured with in vitro irradiated allogeneic or autologous T-lymphocytes in the presence of pokeweed mitogen (PWM). The production of IgG, IgM and IgA, as assessed by the enzyme-linked immunosorbent assay (ELISA) was increased 2--7 times, as compared to values obtained with non-irradiated T-lymphocytes. It is suggested that the increase of Ig production is due to the selective radiosensitivity of T-lymphocytes with suppressor function.

Blood lymphocyte subpopulations in breast cancer patients following post-operative adjuvant chemotherapy or radiotherapy.

The peripheral lymphocyte population was examined in sixty-five randomized breast cancer patients before and after post-operative adjuvant cyclic chemotherapy or local radiation therapy. During 10 months of chemotherapy total lymphocyte and T cell counts were reduced to 70% and non-T cells to about 50% of pretreatment values. The population of T cells was unaltered whereas the proportion of non-T cells was significantly reduced at the end of the period. Immediately after radiotherapy total lymphocyte and T cell counts were reduced to 30%, and non-T cells to 15% of pretreatment values. During 10 months' observation total lymphocyte and T cell counts increased to 70%, and non-T cells to 50--60% of pretreatment values.

The effect of in vitro irradiation on PHA-mediated cytotoxicity and lymphocytes with receptors for the Fc part of Ig.

In vitro irradiation of purified human lymphocytes reduced PHA-mediated cytotoxicity (MICC) against chicken erythrocytes in a dose-dependent fashion. Lymphocytes with receptors for IgG present in both unfractionated and T-cell enriched cell preparations were depleted after X-ray exposure. T-cells with IgM-receptors, however, appeared to be more resistant. Different lymphocyte subpopulations seem to cooperate in MICC, and shifts of their proportions may be one explanation of the reduction of MICC after X-ray exposure.

In vivo and in vitro immune responses to trichophytin in dermatophytosis.

Cell-mediated immunity in patients with verified dermatophytosis was investigated by means of skin test and lymphocyte stimulation test (LST) using purified trichophytin, prepared by the ethylene glycol method, and commercially available trichophytin. The purified trichophytin seemed to be more specific and sensitive both in skin test and in LST. In the majority of cases skin-positive patients were also positive in LST, especially when T. mentagrophytes or E. floccosum was the causative organism. Patients with non-chronic infections also showed a close correlation between skin reactivity and LST. Patients with chronic mycosis or infected with T. rubrum, on the other hand, were often skin-negative. Their lymphocyte reactivity in vitro, however, was usually positive, thus showing a discrepancy in the ability of these two methods to measure cell-mediated immunity.

Prognostic relevance of immunologic variables in breast carcinoma.

A series of 203 consecutive patients with operable carcinoma of the breast was analysed with regard to correlations between a set of immunologic and clinical variables existing at the time of the diagnosis. No major correlations were revealed between immunologic variables on the one hand and clinical features or the course of the disease on the other. The well-known prognostic relevance of tumour size, involvement of the axilla and the histological grade of malignancy was evident.

Peripheral blood lymphocyte populations in influenza patients.

The peripheral blood lymphocyte populations were determined in 10 patients during the acute phase of influenza and 6-8 weeks after recovery. The percentage of T-lymphocytes, demonstrated by their binding sheep erythrocytes non-specifically (E-rosette forming cells), was decreased during influenza. A relative and absolute increase of non-T-lymphocytes could be shown by the determination on these cells of receptors for activiated C'3 (EAC-rosette forming cells).

Immunologic investigation in children with recurrent pneumonia.

In vitro lymphocyte stimulation by phytohemagglutinin (PHA) and pokeweed mitogen (PWM) and tests for granulocyte function were performed in 12 children with recurrent pneumonia. Reduced lymphocyte stimulation was found at repeated tests in 2 children and transiently decreased lymphocyte stimulation in 6 more children. In 3 patients, reduced bactericidal capacity of granulocytes was demonstrated, and in 1 cast stimulation showed decreased NBT activity of granulocytes. One patient examined had immunoglobulin deficiency.

Blood lymphocyte subpopulations in breast cancer patients following radiotherapy.

Both T and non-T lymphocytes decreased immediately following radiotherapy in breast cancer patients. The relative depletion of non-T lymphocytes, however, was more marked than that of T cells. 3 years later the number and the proportion of non-T lymphocytes was higher than immediately after radiotherapy, while T lymphocytes were still depressed. The proportion of cells with membrane-associated Ig was higher in patients 3 years following radiotherapy than in non-treated patients and healthy controls. There was no difference in the proportion of T and non-T lymphocytes between patients with and without metastases, respectively.

[The Stockholm preoperative radiotherapy of operable breast cancer. Preliminary report].

The 4 year results of a prospective clinical trial on the treatment of operable breast cancer, comparing the effects of preoperative radiotherapy followed by surgery with those of surgery followed or not by postoperative radiotherapy are presented for patient groups of approximately 100 each. At 4 years 85 per cent survive after preoperative radiotherapy plus surgery, as against 77 per cent after surgery only and 81 per cent after surgery plus postoperative radiotherapy. These differences are not statistically significant. The incidence of local recurrence after radiotherapy was reduced to 1/4 of that seen after surgery only. The radiation could be shown to induce temporary damage in a number of immunological parameters. No correlation could be demonstrated between these findings and the clinical course of the disease.

DNA-synthesis of lymphocytes in hyperthyroid and euthyroid subjects. Effect of 131I therapy on hyperthyroidism.

The DNA-synthesis of human lymphoid cells as estimated by the measurement of thymidine incorporation in vitro was investigated in healthy controls and in patients with various thyroid disorders before and after therapy. Hyperthyroid patients treated with 131I and surgery (euthyroid at initial blood sampling before surgery), patients with atoxic nodular goitre treated by surgery and healthy untreated control individuals comprised the material. The synthesis of DNA in lymphocytes was higher in hyperthyroid patients in comparison with euthyroid individuals, and decreased subsequent to 131I therapy in the hyperthyroid patients. No decrease was recorded in the other groups of patients. No evidence suggesting a change in the lymphocyte reactivity to thyroglobulin was found in any of the patient groups.

Lymphocyte populations in peripheral blood in hyperthyroid and euthyroid subjects.

Thymus-derived and non-thymus-derived peripheral blood lymphocytes were examined by means of E- and EAC-rosette tests. The frequency of these lymphocyte populations was the same in hyperthyroid and euthyroid individuals. It was also demonstrated that PHA responsiveness of lymphocytes from hyperthyroid patients was equal to that of lymphocytes from euthyroid controls and did not change after treatment with 131I.

Mitogen stimulation of lymphocytes from patients with epidemic influenza.

The in vitro responses to PHA (phytohemagglutinin) of lymphocytes from 14 patients with influenza A and 4 patients with influenza B infection were found decreased as compared with the responses to the same mitogen demonstrated in lymphocytes obtained from the same patients after recovery. Lymphocytes from 6 of the patients with influenza A were examined for PWM (pokeweed mitogen) stimulability and a decrease was found during the influenza infection. The decrease of PHA response during the acute stage of the disease was observed in the presence of autologous as well as of fetal calf and allogeneic AB serum. Possible mechanisms causing this impaired function of lymphocytes are discussed.

An evaluation of delayed hypersensitivity in guinea pigs to various trichophytin preparations.

Guinea pigs immunized with mycelium from Trichophyton mentagrophytes in Freund's complete adjuvant were tested intradermally with the following trichophytin preparations: TrI extracted by ethylene glycol, TrII extracted by phenol and TrII, a commercially obtained preparation fromSächsisches Serumverk KG. In our experience, TrI was found to be superior to the other preparations. It showed signifiicant delayed skin reactions in all sensitized animals and none in the controls. Lymphocyte stimulation test with this preparation (TrI) gave positive results.
