The preovulatory follicular fluid in the human: influence of hormonal pretreatment (clomiphene-hCG) on some biochemical and biophysical variables.

Some biochemical and biophysical variables of human follicular fluid have been studied both in normal women after the LH peak (preovulatory follicles) and in women treated with clomiphene and hCG (clomiphene treated follicles). The results showed that lactate and glucose levels are different from those found in serum but are not influenced by the clomiphene treatments. Osmolarity and enzymatic contents are neither different from those of serum nor modified by the hormonal treatments. The transfer of water across the follicle wall was found to be closely regulated because only slight variations were observed in the wide range of volumes collected in the samples examined.

[Rupture of the biliary tract in the liver hilum region following closed abdominal injuries. General review about 2 cases].

Two cases of traumatic rupture of the biliary tract in the liver hilum region are reported. Similar cases (27 patients) reported in the published literature are discussed and the physiopathological, pathological, and clinical characteristics described. Road accidents account for the majority of these cases of biliary tract injury, which are seen mainly in young males. Diagnosis is confirmed either during emergency laparotomy for haemoperitoneum, the presence of bile leading to cholangiography which establishes the diagnosis, or after several days from the presence of jaundice and signs of a peritoneal effusion, when urgent surgery was not required. Treatment is by direct suture or hepatojejunal anastomosis, choice being dictated by two parameters: the type of lesion and the time when it is repaired.

[Prediction of ovulation].

Predicting ovulation is the basis on which the fertile period is determined. When we carried out observations on 60 spontaneous cycles it was possible to detect with precision the discharge of LH which would produce ovulation by taking series of plasma levels of LH. The start is defined as the time when the level of LH becomes 180% greater than the mean level observed in the previous 24 hours. Ovulation occurs between 37 and 39 hours after this threshold has been crossed. The discharge of LH is the most constant criterion and the most precise one, and it does make it possible to work out the chronology of the events that precede ovulation. Ultrasound and estimation of levels of oestradiol in the plasma or in the urine do allow one to appreciate how the follicle is maturing.

[Delayed rupture of mechanically performed digestive tube suture (author's transl)].

Three months after a colo colic anastomosis with E.E.A stapler and sixteen months after a terminal closing of a jejunal Rous Y loop with G.I.A., the outcome of a intestinal distension caused leaks between staples. So it appears that the absence of a sclerotic scar around staples is a cause of weakness and then a disadvantage of his kind of intestinal suture.

[Intermittent claudication of the buttock (author's transl)].

Intermittent claudication of the buttock of arterial origin is a syndrome too often overlooked though easily diagnosed from the patient's history and clinical examination. It is due to lesions of the iliac arteries coexisting with permeable femoral and popliteal arteries. In about one-third of the cases the common iliac artery is completely occluded; in the remaining two-thirds the lesion consists of pre-occlusive stenosis and carries a high risk of acute ischaemia in the corresponding limb.

[Spontaneous abortion after intra-uterine transfer of an ovum fecundated in vitro (author's transl)].

Spontaneous expulsion of a human ovum fecundated in vitro and transferred into the uterus occurred after 10 1/2 weeks of amenorrhoea. The oocyte had been collected after stimulation by human menopausal and chorionic gonadotrophins (hMG and hCG). The techniques of pre-ovulatory oocyte collection, fecundation, in vitro culture and of reimplantation of the embryo are briefly described.

Diverticulitis in a patient with a 47-year defunctionalized colon.

A patient who had a cecostomy because of a complicated appendicitis at the age of 17 functioned with an excluded colon for 47 years. He was then hospitalized for diverticulitis and left pericolic abscess. After treatment of the abscess by left segmental colectomy and reanastomosis, the cecostomy was closed. Two years later, he presented a normal bowel habit and improvement of his diverticulosis. Rectal manometric study showed that the rectal was normal both before and two years after surgery. Occurrence of diverticular disease in a normally innervated and vascularized, but almost empty, colon presenting with normal mechanical properties is consistent with Burkitt's hypothesis concerning long-term effects of a low residue diet on the gastrointestinal tract.

Interpretation of plasma luteinizing hormone assay for the collection of mature oocytes from women: definition of a luteinizing hormone surge-initiating rise.

Analysis of plasma luteinizing hormone (LH) assays (4 assays per day) permits the discernment of the onset of the physiologic effects of ovulatory release. This threshold value of LH plasma concentration (LH surge-initiating rise, LH SIR) is determined for each cycle in terms of the average baseline level of the previous day. The chronology of follicle and oocyte maturation after LH SIR is similar to that which follows chorionic gonadotropin (hCG) administration: none of the 20 patients, compared with 2 out of 10, had ovulated when laparoscopy was performed 30 to 35 hours and 36 to 38 hours after LH SIR time, respectively; 1 out of 3, 9 out of 14, and 5 out of 5 patients had an in vitro fertilized egg when oocytes were collected 30 to 32, 33 to 35, and 36 to 38 hours after LH SIR time, respectively. Thus the oocyte can be collected 34 to 35 hours after the LH SIR; at this point ovulation has not occurred and the oocyte is capable of being fertilized.

The action of anti-inflammatory drugs on the fertility of female rats with intrauterine contraceptive devices.

A silk thread (0-0) or a copper wire (0.18 mm diameter) was placed in the left uterine horn of rats, at least 3 days before fertilization. The animals were untreated or given daily injections of 3 mgh hydrocortisone or 30 mg aspirin beginning the day after insertion of the IUD; 2 weeks after coitus, the animals were killed. A longitudinal (less than or equal to 10 mm) silk thread and the copper wire were effective IUDs regardless of treatment. With the short silk thread (less than or equal to 2 mm), implantation occurred in all groups of rats but a high failure rate was observed in hydrocortisone-treated rats (5/9 with implantations). The no. of implantations/no. of corpora lutea (x 100) was much higher (20.4%) in rats treated with aspirin and hydrocortisone than in untreated animals (3.2%) (P less than 0.01). We conclude that the effectiveness of an IUD can be altered by treatment with an anti-inflammatory substance.

[Recovery of preovulatory human oocytes (author's transl)].

Rapid radio-immune assaying of LH was carried out four times daily on 52 women in the preovulatory phase of the cycle. This method enables the precise detection of the LH surge and recovery of the mature oocyte by laparoscopic aspiration of the follicular content, just before ovulation, which is between 28 and 36 hours after the start of the LH surge. The oocytes was recovered in 27 out of 41 patients with a normal cycle (66%). The rate of recovery per follicle aspirated is comparable in a natural cycle to that after treatment with Clomid (80 to 84%).

[Infection of vascular sutures: a report on 26 cases (author's transl)].

The authors describe 26 cases of infection after vascular surgery, affecting a Dacron prosthesis in 13 cases and venous grafts in the other 13 patients. The site of infection was usually the region of Scarpa's triangle (15 cases) with the presence of enterobacterium in 40% of cases. The most frequently isolated germ was staphylococcus aureus (20 cases). Infection was revealed by hemorrhage in 17 patients and was evidence of breaking down of the anastomosis. Results of surgical treatment in the two groups were evaluated by using three criteria: --persistence or not of infection, --vital prognosis, --functional prognosis. Surgical treatment methods and possible contamination sources are discussed. When the operative zone is infected, the authors recommend ablation for the prostheses and conservative therapy for venous autografts. The prosthesis was left in place in 5 patients, and only one patient recovered, while two deaths occurred. In the 10 cases where ablation of the prosthesis was performed, however, the infection was cured, and in 4 patients there was revascularization in the infected area. Apart from these cases, conservative therapy was applied in 11 cases and healing occurred in all of them.

[Benign vascular digestive tract malformations. Review and report on two cases (author's transl)].

Benign vascular digestive tube malformations were first described in 1839. Their incidence is low, they occur mainly in the small intestine and colon, and they are of current interest because of their rare nature, the difficulty in establishing a diagnosis, and the large variety of lesion observed. They tend to be diffuse, multiple, and may be found in the rectum. Treatment is difficult, therefore, and though surgery is usually indicated, endoscopic electrocoagulation, laser beams, or embolization may be of value when used alone or as complementary therapy.

[Human in vitro fertilisation. Preliminary results (author's transl)].

We here report the results of our first attempts at fertilising the human oocyte in vitro. No external hormonal stimulation was used to induce follicular growth or maturation of the oocyte. Nine oocytes were obtained laparoscopically in ten women in the pre-ovulation hours. They were incubated in a synthetic culture medium in the presence of freshly ejaculated spermatozoa. Five oocytes were fertilised normally, of which three were cultured just up to the stage of 4-6 blastomeres. The three failures of fertilisation can be put down to poor quality of the gametes. The discussion is carried on about the quality of the oocyte and of the embryo and justifies further studies to deepen our knowledge before embarking on embryo transplant.

["Suspended" popliteal artery shunts. A report on 15 cases (author's transl)].

A "suspended" popliteal artery (S.P.A.) constitutes one of the most pejorative anatomical forms for atheromatous lesions of the lower limbs, as obliteration of the superficial femoral artery is often associated with atheromatous lesions in the tibial arteries, making them inaccessible to arterial reconstruction. Operations have been conducted on 5 cases (14 patients) over a period of 3 years. The average age of the patients was 71.6 years and the average follow-up period was 13 months. In each case operation was required to avoid amputation. Venous material was used for the shunt in 10 cases, while 5 had composite (n = 3) or prosthetic (n = 2) material. Postoperative mortality was nul, but 28% died by the end of the first year. Thrombosis occurred in 3 cases, at the 2nd, 7th, and 9th month respectively (actuarial permeability rate after 1 year: 72%), and 4 patients had high amputations (leg: 3, thigh: 1) in spite of permeability of the shunt is two cases (actuarial conservation rate of the limbs after 1 year: 67.5%). An analysis of the results of these 15 cases and those rare series reported in the published literature, enables definition of the indications for revascularization of an S.P.A.

[Acute dyspnoea by acute thyroid abscess (author's transl)].

Infection is extremely rare in the pathology of the thyroid gland. One case is reported, and a review of the literature has helped to emphasize some specific points. Acute thyroiditis can be misdiagnosed in its initial phase because a fever by itself may not lead to a through examination of the neck, even if a goitre is present; the latter being the site of infection in the case reported. Dyspnoea of laryngeal origin proves to be a dangerous complication of thyroid abscesses. Treatment is always surgical, linked with appropriate antibiotic therapy. In the long run, thyroid function is unaffected.
