Peripheral lymphocyte culture in the diagnosis of drug induced liver injury.

In order to evaluate the usefulness of the peripheral lymphocyte culture method for the diagnosis of drug-induced liver injury, a study was carried out on 21 cases of drug-induced liver injury (Group A). Nine cases in Group A showed a positive lymphocyte stimulation test. Out of 9 cases which showed positive stimulation six cases were cholestatic type of liver injury, and the remaining three were of hepatitic type. The results suggest that peripheral lymphocyte culture method is a useful method for diagnosis of drug-induced liver injury.

Ten years' survey of dialysis-associated tuberculosis.

Tuberculosis associated with dialysis was studied at the Renal Unit of the Tokyo Medical and Dental University and Yokosuka Mutual Aid Hospital Kidney Center, in both of which the treatments of chronic renal failure are the same. There are 12 tuberculosis patients out of 367 patients on maintenance hemodialysis from January 1967 to December 1976, an incidence of 3.3%. This was 6-16 times greater than that in the general population of this country according to yearly statistics. The characteristics of dialysis-associated tuberculosis include a high incidence of miliary tuberculosis, especially in aged patients and difficulty in establishing the diagnosis before death. Clinical features which are helpful in the early diagnosis are intermittent high fever of unknown origin, weight loss, anorexia, abnormalities of the central nervous system, erythrocyte sedimentation rate over 100 mm/h, leukocytosis and high value of the C-reactive protein. With the increasing number of dialysis patients, an increase of dialysis-associated tuberculosis is expected and this will be one of the major problems of dialysis patients in future.

Reduction of unfavourable effects of heparin with use of gabexate mesilate in dialysis.

Gabexate mesilate (GM), a potent synthetic proteinase inhibitor has been evaluated as an alternative anticoagulant to heparin for haemodialysis. GM haemodialysis was achieved at a dose of 1,600mg/hr. GM was found to be dialysable and so rapidly degraded that dialysis with GM anticoagulation was analogous to regional heparinisation. Although heparin induced considerable lipolytic activity, GM showed no particular effect on lipids. Some patients reported mild nausea and there was evidence of slight clot formation around the top of the venous drip chamber. GM anticoagulation is easier than the conventional unreliable techniques of regional heparinisation.

Precocious puberty due to a hypothalamic hamartoma.

A case of precocious puberty due to a hypothalamic hamartoma is presented. Concentrations of plasma LH and FSH, testosterone and its derivatives were found to be elevated. Circadian rhythms of LH were also observed. After removal of the mass, plasma LH and FHS concentrations declined to nearly half the preoperative levels.

[Autosensitization of neurosurgical cases by normal brain tissue antigens (author's transl)].

The leukocyte reactions of 106 neurosurgical cases, including 63 brain tumors, 10 intracerebral hematomas, 10 cerebral infarctions, 10 subarachnoidal hemorrhages, 8 cerebral injuries and 5 chronic subdural hematomas, against the extracts of gliomas and normal brain tissues were tested by capillary migration (LMI) and adherence inhibition (LAI) assays. Both tests showed specific responses with autochthonous and allogeneic glioma extracts in glioma patients. The sensitivity of LAI was superior to that of LMI, although LAI also showed adherence enhancement in the presence of weakly sensitized leukocytes or weak antigenic stimuli. Leukocytes from glioma patients showed positive inhibition with normal brain tissues from patients with glioma and intracerebral hematoma. Positive leukocyte reactions with normal brain tissues were also confirmed in patients with intracerebral hematomas, cerebral infarctions and severe cerebral lacerations, but not in those with subarachnoidal hemorrhages, minor cerebral contusions and chronic subdural hematomas. These results suggest that the leukocytes of patients with destructive brain lesions were autosensitized by normal brain antigens. The autosensitization has some advantages in that destroyed brain tissues are eliminated, but the hyperimmune state might cause postictal brain edema and should be properly controlled by steroids.

[Cerebrospinal fluid (CSF) cytology using "cytospin" in patients with brain tumors (author's transl)].

Cerebrospinal fluid (CSF) cytology was performed using cytocentrifugation method ("Cytospin") in 88 preoperative patients who had been known to have deeply seated brain tumors by CT. 20 cases (22.7%) among them showed positive findings and 11 cases (12.5%) probably positive findings. High incidence of positive results were obtained in germinomas (60.9%), ependymomas (50.0%), medulloblastomas (40.0%), followed by malignant lymphomas (20.0%), pontine gliomas (16.7%) and metastatic tumors (16.7%). In contrast, it was persistently negative in cerebral astrocytomas, glioblastomas, teratomas, epidermoids, meningiomas and neurinomas. This method was quickly prepared in 30 minutes and was superior in retaining detailed cytomorphology. It was concluded that the CSF cytology is a less invasive diagnostic investigation, can provide an additional information as regards the nature of some kinds of brain tumors and is still useful even in the CT era.

Spontaneous extracranial metastasis of cerebral neuroblastoma.

A case of extracranial metastasis of cerebral neuroblastoma without previous craniotomy is presented. The tumor in the left posterior frontal region penetrated the dura mater and the skull, formed a lump under the galea and metastasized to the lymph nodes in the infraparotid and superior cervical regions.

Renal vein fibrin degradation products (FDP's) in glomerulonephritis.

In order to determine the role of intrarenal vascular coagulation (IVC) in chronic glomerulonephritis, coagulant parameters in renal blood (RVB), urine FDP's and renal histology were examined in 70 patients with chronic glomerulonephritis (CGN). RVB was obtained by Seldinger's technique. A correlation was obtained between an increase of fibrinogen, FDP and soluble fibrin monomer complexes (SFMC) in RVB and the degree of intraglomerular fibrin deposition shown on immunofluorescence. An increase of FDP's, SFMC and a slight decrease of fibrinolytic activity in RVB were observed in patients with CGN with decreased renal function or with the nephrotic syndrome. Daily excretion of FDP in the urine and the extent of intraglomerular fibrin deposition were greater in nephrotics than in non-nephrotics. We conclude that measurement of coagulation parameters in RVB is a reliable and sensitive method of assessing IVC, and that IVC plays an important role in aggravation of chronic glomerulonephritis, particularly when the nephrotic syndrome is present.

Alpha-fetoprotein in intracranial malignant teratoma.

Serum alpha-fetoprotein was identified in five patients with histologically verified teratomas and in one patient with a suspected malignant teratoma. Detection of serum alpha-fetoprotein is specifically indicative of malignant teratoma, as no other known type of intracranial tumor produces it.

Glycosaminoglycan-synthetic activity of pleomorphic adenoma. Adenoid cystic carcinoma and nonneoplastic tubuloacinar cells of the salivary gland.

An analysis was carried out on glycosaminoglycan produced in pleomorphic adenoma, adenoid cystic carcinoma, sialadenitis and normal tissue of the salivary gland. After incubation of the tissue segments in a medium containing 35SO4, a radioautograph of the tissue section was made to observe the localization of 35SO4 incorporation, and 35S-labelled materials were purified from the tissues, and analyzed. High 35S-radioactivity was observed in the ductal cells of the inflammatory gland tissue and in the acinar cells of normal palatinal gland, but little radioactivity was observed in the interstitial components in these tissues, and the amount of 35SO4 incorporated in the tumor cells was also significant. Eighty to 90% of the 35S-radioactivity incorporated could be detected as 35S-glycosaminoglycans in all tissues except for the normal palatinal gland, which contained a large amount of 35S-sulfated glycoprotein. No significant difference in the synthetic activity of 35S-glycosaminoglycans and in their components were observed between nonneoplastic and neoplastic cells. These results suggest that glycosaminoglycan-producing cells in pleomorphic adenoma as well as in adenoid cystic carcinoma are derived from the tubuloacinar cells of the salivary gland.
