Complex immunological monitoring of breast cancer patients treated postoperatively by electron beam irradiation.

To monitor the electron beam therapy some immunological parameters of breast cancer patients previously undergone surgery were tested before, during and after irradiation. Immune complex levels measured by complement consumption technique were not altered by irradiation. Killer cell activity tested in so-called antibody dependent cellular cytotoxicity (ADCC) capacity assay showed a marked decrease in some cases. Based on the phagocytic capacity of the granulocytes the patients could be divided into two groups: one with declining activity and another with rising activity: The majority of the patients (22/45) were humoral leukocyte adherence inhibition (H-LAI) negative before and during irradiation. Those showed positive H-LAI indices before electron therapy had unchanged (7/45) or decreasing (9/45) tendencies during the observation period. Further study is needed to establish the clinical relevance of these in vitro assays used by us.

The wide bladder neck and posterior urethra in childhood.

Involvement of anomalies of the bladder and urethra in recurrent urinary tract infections and micturition disorders in children are examined in the light of published evidence and of the authors' own observations. The data of 300 children followed up for a wide bladder neck and posterior urethra at the Nephrology Clinic of Heim Pál Children's Hospital, Budapest, are reviewed. The dilatation in question is regarded as functional, attributable to a congenital neurogenic or myogenic defect. For the management and prevention of the recurrent ascending infections long-term urinary disinfection and meticulous toilet of the external meatus are recommended. The symptoms are expected to subside spontaneously at the age of menarche. The importance of a close nephrologic follow-up is emphasized.

Neurofibromatosis of the bladder.

Neurofibromatosis of the urinary tract is estimated to account for 20 per cent of the visceral manifestations of the generalized disease. Among its urological forms involvement of the bladder is the most common. Clinically, neurofibromatosis resembles a tumour, therefore, its early histologic diagnosis is of utmost importance. In the present case a tumour involving the posterior bladder wall was found in a 6 years old boy subjected to examination because of a terminal haematuria. First the tumour was resected, later cystectomy with implantation of the distended ureter into the sigmoid was undertaken because of progression of the tumour. Postoperative hypokalaemia responded to the usual medication. Ureteral distension was reversed by the operation. Though neurofibromatosis (NF) is histologically a benign process, it tends to recur, is invasive, and impairs urinary functions. Therefore, it may endanger life and thus requires cystectomy with surgery for urinary diversion.

Humoral leukocyte adherence inhibition (H-LAI) before and after surgery in human breast cancer.

Eighty-four breast cancer patients were tested by the H-LAI method before and after surgery. In the cancer groups positive H-LAI responses could be stated in 90--100%. In benign breast diseases 94% positivity was also detected. The H-LAI values of the cancer patients did not change significantly at their discharge from hospital and only insignificant reduction in the H-LAI indices were detected even 3--12 months after mastectomy. The benign cases showed, however, a definite declining tendency in H-LAI after surgery.

Relationship between histological structure and lactate dehydrogenase isoenzyme pattern of rat mammary tumors induced by dimethylbenz(a)-anthracene.

Mammary gland tumors induced by 7,12-dimethylbenz(a)-anthracene (DMBA) in female Wistar rats could be divided into secreting and nonsecreting ones. Histological, histochemical, cytophotometric, and electron microscopic examinations yielded indirect evidence for the ab ovo malignant nature of the nonsecreting tumors but not for the secreting ones. Nonsecreting tumors showed high total lactate dehydrogenase (LDH) activity and a definite shift of the isoenzyme pattern toward isoenzymes containing M subunits compared to those of lactating mammary glands. Secreting tumors could be characterized by low total LDH activity and a moderate increase of M/H ratio. These biochemical results support the hypothesis that nonsecreting mammary tumors induced in rats--independently of metastatization and differentiation--have to be considered carcinomas while the secreting ones benign adenomas.

Subsequent monoclonal gammopathies.

21 patients with 'subsequent' monoclonal gammopathies which developed after non-tumour or tumour states were studied. The cases were selected from 221 patients producing M protein, partly based on retrospective analysis and partly on data of case history, autopsy, histology, and immunochemistry obtained during continuous observation. Possible ways of development of immunoglobulin-producing monoclonal cell clones are also discussed.

Congenital short colon associated with imperforate anus (Zachary-Morgan syndrome).

Two cases of short colon with imperforate anus are presented. Terminal colostomy is the therapy of choice in the neonatal period. As the definitive final procedure, trimming into a tubular structure and abdomino-perineal pull-through operation is suggested. A survey is given of similar cases reported in the literature.

3-Chloro-1,3,5-pregnatriene derivatives with glucocorticoid activity.

Novel synthetic glucocorticoid analogues were tested for receptor binding and glucocorticoid activity. They were of unusual structure, insofar as they had a 3-chloro rather than a 3-oxo function. 3-Chloro analogues of fluorinated glucocorticoids formed extremely stable complexes with the rat liver glucocorticoid receptor. 3-Chloro derivative of fluocinolone acetonide also had in vivo glucocorticoid activity. It induced tyrosine aminotransferase in the liver and repressed thymidine kinase in the thymus very effectively. It is concluded that 3-chloro analogues may retain glucocorticoid activity as well as the ability to bind to the glucocorticoid receptor protein.

Fine structure of perisinusoidal cells in developing human and mouse liver.

The fine structure of hepatic perisinusoidal cells was studied in human fetuses and in mouse embryos and fetuses. The perisinusoidal cells differed in structure from the sinusoidal lining cells, and occasionally mitosis of perisinusoidal cells was observed suggesting that these cells are independent and self-proliferating at the stage of development studied. Unlike perisinusoidal cells of the normal mature liver, those of the developing liver contained only sparse and small lipid droplets. They possessed a well developed rough endoplasmic reticulum and Golgi complex, and many microtubules. Collagen fibrils appeared in close proximity to perisinusoidal cells. In perisinusoidal cells of the mouse liver development of the rough endoplasmic reticulum and Golgi complex, accumulation of microtubules and increase of collagen fibrils progressed with the time of gestation. The thick cytoplasmic processes of the perisinusoidal cells frequently encircled the sinusoids, and 5-6 nm thick filaments appeared at the cell periphery. Perisinusoidal cells and hepatocytes were frequently linked by junctional elements. These findings strongly suggest that the perisinusoidal cells are responsible for the production of type III collagen. Also they may indicate that these cells reinforce the sinusoides, influence the sinusoidal blood flow by contractile activity and participate in the maintenance of the parenchymal organisation in the developing liver.
