Ureterocystoplasty for megaureter: a technique practicable in bladder wall fibrosis.

The cause of megaureter (hydroureter) is inadequacy of the ureterovesical junction. The only possible means of its therapy consists in a surgical reconstruction of the ureterovesical junction. A technical modification of ureterocystoplasty for megaureter, having been employed since 1976, is described. It basically consists in the construction of a tunnel from the ureter within the bladder, the ureter being sutured to the wall of the bladder only loosely. In 150 children 182 ureteroneocystoplastic operations were done. As confirmed by the postoperative course, as well as by the results of the follow-up studies, the technique may be used with benefit even in case of infection, impaired renal function or bladder wall fibrosis.

Postnatal development of perisinusoidal cells in the porcine liver.

The ultrastructure of perisinusoidal (PS) cells was studied in the liver of young pigs 1-28 days after birth. In immature animals these cells have long cytoplasmic processes often surrounding almost the entire sinusoid. In their close vicinity newly formed collagen filaments are found. While in the adult pig the PS cell cytoplasm is almost totally occupied by a large lipid droplet, in the young animal it contained only a few small lipid droplets, accompanied by a well-developed granular endoplasmic reticulum and Golgi-apparatus, microtubules and filaments 5-6 nm in diameter. The latter formed bundles with patchy dark areas. Findings indicate that during early postnatal development the function of PS cells is protein synthesis, perhaps for the production of reticulum fibres rather than fat or vitamin-A storage. In addition, PS cells by containing contractile elements may regulate the width of the sinusoids.

An epidemiological study on undescended testis.

We present an epidemiological study of undescended testes. Of the cases of undescended testes two-thirds are bilateral. Undescended testes occur more often in the sons of less educated parents and mothers with lower qualified jobs. Our study led to 3 important etiological conclusions: 1) cases of undescended testes at birth and true undescended testes cases may represent somewhat different nosological entities, 2) unilateral and bilateral cases do not show any significant difference in epidemiological parameters and 3) the pituitary and chorionic gonadotrophins may have an important role in the occurrence of true undescended testes, as suggested by the season of birth.

Genetics of undescended testis.

We present our study of the genetics of undescended testes. The occurrence of undescended testes could be confirmed in 1.5 to 4.0 per cent of the fathers and in 6.2 per cent of the brothers of index patients with true undescended testes. The pattern of the familial cluster fits into the gaussian-additive-multifactorial-threshold program. The value of heritability for the first degree male relatives is 0.67 plus or minus 0.16. In siblings of index patients with undescended testes other congenital abnormalities do not occur more often. Bilateral (more severe) undescended testes are associated with a higher recurrence risk for siblings. The nosological identity of unilateral and bilateral undescended testes have shorter menses and a delay of menarche. Thus, the pituitary hypogonadism of mothers seems to be a predisposing factor for undescended testes in their sons.

Clinicopathological studies of liver cirrhosis and hepatocellular carcinoma in a general hospital.

Clinicopathological studies of 181 patients with liver cirrhosis undergoing autopsy in the pathology department of a municipal hospital between 1973 and 1976 are reported. The main etiological types were alcoholic (25.4 per cent), HBsAg positive (14.9 per cent), and cryptogenic cirrhosis (54.7 per cent). Four patients had multifactorial and secondary biliary cirrhosis and one patient had congestive cirrhosis. The morphological characteristics of the condition and the age and sex distribution of the patients were analyzed in each etiological group. Hepatocellular carcinoma occurred in 28.7 per cent of the cases, most frequently in association with HBsAg positive cirrhosis. Hepatocellular atypia was significantly more frequent in HBsAg positive than in other etiological types of cirrhosis and significantly more frequent in cases associated with hepatocellular carcinoma than in those not associated with it.

[Ultrastructure of malignant non-Hodgkin lymphomas].

Ultrastructural characteristic features of non-Hodgkin malignant lymphomas-based on the analysis of 51 cases are dealt with. It is concluded, that ultrastructural changes of lymphomas can be correlated with the light microscopic findings and tumours arising from T and B cells can be distinguished also on the ultrastructural level. Electron microscopy should be regarded as an additional method in classification and differential diagnosis of non-Hodgkin malignant lymphomas. It contributes to a more exact diagnosis and clarifying the histogenesis of these tumours.

[Pyeloplasty in infants and children].

Pyeloureteral obstructions of various pathogeneses and their complications can be visualized satisfactorily be means of intravenous urography. Of the other diagnostic methods angiography was found to be the most useful, no retrograde examination was carried out. When distension of the end of the calyx or obvious parenchymal lesion appears, plastic operation must be performed irrespective of the patient's age, the presence of infection or anomaly of the localization. Only kidneys which have lost their functions should be removed. In the operation the pyeloureteral boundary is always excised, the new transition is secured by splinting and a transrenal drain is used. Secondary nephrectomy had to be performed in a single case. One-year and three-year intravenous urographic check-ups showed in 72 of the 100 cases good, in 19 satisfactory results, in 8 cases the pyeloureteral boundary was funnel-shaped, but the dilatations of the calyx-ends remained unchanged. Pyeloplasty is no longer a high-risk operation.
