Intermittent claudication of the hand after creation of an arteriovenous fistula in the forearm.

Eighty-five patients were followed up at least 1 year after creation of an arteriovenous fistula in the forearm. The anastomosis was side-to-side in 33 patients, end-to-side in 33 and end-to-end in 19. Trophic lesions were not observed. Intermittent claudication of the hand was more frequent in patients with a side-to-side arteriovenous fistula (42 percent) than in those with end-to-side (21 percent) or end-to-end fistulas (16 percent). Clinical and x-ray studies indicate that two different mechanisms are responsible for cramping pains: arterial steal phenomenon and venous hypertension. Their relative importance depends on multiple hemodynamic factors that may vary with time.

Spontaneous rupture of right iliac arterial aneurysm into ureter.

Among the causes of severe hematuria, spontaneous ruptures of aneurysms into the urinary tract are rare. The diagnostic approach and treatment of this disease constitute a difficult urologic problem whose clinical, radiologic, and anatomopathologic aspects are discussed.

Contribution of angiography in bladder cancer.

Pelvic angiography was performed in 24 patients with primary or secondary tumours. 17 patients were submitted to arteriography and 7 to phlebography of the pelvis veins. An angiographic classification was established and compared to the pre-operative and histopathological results determining tumour staging. These results were compared to those obtained by bimanual palpation under narcosis and demonstrated the significant contribution of angiography in determining the extravesical spreading of bladder tumours.

Preoperative embolization of renal tumors--comparison of different methods.

Different techniques and materials for preoperative embolization of renal carcinoma have been assessed in 26 cases. The different methods used and compared were gelfoam fragments (10 cases), cyanoacrylate (4 cases), Gianturco stainless steel coil (3 cases) and the combination of gelfoam and the Gianturco device (9 cases). The method of choice appears to be the combination of gelfoam and the Gianturco device which allows excellent peripheral ischemia with complete occlusion of the main arterial trunks. Embolization is particularly useful in invading and hypervascularized renal tumors supplied by collateral blood vessels.

Selective pudendal arteriography.

Selective pudendal arteriography is indicated in selected cases of male impotence when there is a strong presumption that peripheral arterial lesions are responsible for it. These lesions are classified as: (a) traumatic; (b) obstructive in thrombo-angiitis or atheroma, and (c) dysplasic. Selective arteriography will confirm the diagnosis and indicate the nature of the obstruction, its degree and extension.

Angiographic features of xanthogranulomatous pyelonephritis.

Preoperative diagnosis of xanthogranulomatous pyelonephritis (XGP) is possible in numerous cases by renal angiography, thus permitting the differentiation of this disease from renal adenocarcinoma. Angiographic features are reviewed in patients with proved XGP. The most useful findings are: (1) presence of hypervascularised areas; (2) dilated renal capsular and perforating arteries, and (3) heterogenous aspect of the kidney during the nephrographic period.

Metabolism of human PTH by the kidney and the liver.

Immunoreactive PTH was measured by amino terminal and carboxyl terminal specific assays in the femoral artery, the right renal vein and the suprahepatic vein of ten hyperparathyroid patients. A marked arterio venous difference for amino terminal immunoreactivity was observed in the kidney and the liver. In contrast, the arterio venous difference for carboxyl terminal immunoreactivity was small in the kidney and not significantly in the liver. It is concluded that intact PTH and possibly amino terminal fragments of the hormone are metabolized by the kidney and the liver. Considering the fact that a carboxyl terminal specific antiserum is also capable of recognizing intact hormone, the finding of a small positive arterio venous difference for carboxyl terminal immunoreactivity does not permit us to exclude the possibility that the kidney and/or the liver are capable of generating carboxyl terminal fragments.

Computed tomography of the kidney.

The application of computed tomography to the diagnosis of kidney disease and the features of renal neoplasms, cysts, hydronephrosis, inflammatory parenchymal lesions and other pathological processes are described. The diagnostic accuracy of computed tomography is excellent when compared with ultrasound and selective renal arteriography. This new non-invasive technique can obviate the need for arteriography in a significant number of morphological diseases of the kidney and is especially useful in defining the extent of malignant tumours.
