[In loco fibrinolytic treatment of acute ischemia of the lower limbs].

Peripheral artery occlusion produced by thrombosis or embolus can be cured by intra-arterial low dose thrombolysis without severe hemorrhagic complications. 18 patients suffering of acute ischemia of lower limbs were treated by intra-arterial streptokinase infusion. Diagnostic angiography is performed by antegrade femoral puncture using a 5F straight catheter. The tip of the catheter is placed within the thrombus. After a bolus of 10 000 U, streptokinase is infused at the rate of 2,000 U/min. Fibrinolysis is usually achieved within 70 min with a total dose of 150,000 U. Arterial recanalization occurred in 14 patients, in 6 cases underlying stenosis was successfully treated by percutaneous transluminal angioplasty. No severe complications were encountered.

Ultrasonic demonstration of the amnion.

The amnion can be visualized in most pregnancies before the 12th week. It appears as a thin membrane separating the amniotic cavity, which contains the fetus, from the extra-embryonic coelom and the secondary yolk sac. In a prospective study of 32 women in the first 12 weeks of pregnancy, the amnion was demonstrated ultrasonographically in 27 (84 per cent). The embryonic origin of the amnion and possible difficulties in its demonstration by ultrasound are described.

Ultrasonic evaluation of fetal ventricular growth.

The ratio of lateral ventricle to hemispheric width was measured in 200 normal pregnancies. Statistical curve fitting was tried to predict normal value and 5 and 95 percentiles. Anatomical sections of fetal head are provided to correlate the ultrasonic pictures. A typical case of application is described.

Ultrasonic evaluation of fetal limb growth.

Biparietal diameter and femoral and humeral length were measured by ultrasonography and correlated with age in 450 normal pregnancies. The resulting data may be helpful in detecting fetal limb malformation.

Transplantation of both kidneys of an anencephalic newborn to a 23-year-old patient.

Both kidneys of an anencephalic newborn were transplanted into a 23-year-old male patient. Hemodialysis was required for 3 weeks postoperatively. Both organs increased in size but did not reach the normal volume of an adult kidney. 3 years post-transplantation, the patient is doing well with a plasma creatinine level of 1.5 mg/dl. Technical problems are discussed.
