Lactate accumulation relative to the anaerobic and respiratory compensation thresholds.

Anaerobic thresholds of five male subjects were determined invasively (ATi), from a marked increase in plasma lactate above resting levels (delta La), and noninvasively (ATn), from a nonlinear increase in minute ventilation (VE) during incremental work (IW) leg cycling tests; work rate was increased 30 W every 2 min. Each subject also performed four constant-load work (CLW) tasks just above and just below their ATn and respiratory compensation threshold (RCT), i.e., the point expressed as O2 consumption (VO2) or work rate, at which VE increases disproportionally to CO2 output during IW. In four of the five subjects the ATn preceded the ATi during IW. Yet the ATn delineated the CLW in which marked lactate accumulation did or did not occur. During CLW just above the ATn in these same four subjects, VE/VO2 and fractional expired O2 (FEO2) peaked well before delta La plateaued. These findings suggest that exercise hyperventilation is not necessarily proportional to increases in plasma lactate.

Bone mineral content in idiopathic renal stone disease and in primary hyperparathyroidism.

Bone mineral content (BMC) was measured with the Norland Cameron apparatus in 120 renal stone formers (RSF) with idiopathic stone disease and in 41 patients with primary hyperparathyroidism. RSF were classified, according to an oral calcium load test, into three groups: no hypercalciuria (HC; 41 cases); absorptive HC (53 cases), and resorptive or renal HC (25 cases). BMC values in RSF as a group were significantly lower than normal (p less than 0.001, Mann-Whitney test) though higher than in hyperparathyroid patients. There was a trend for BMC to decrease from male RSF without HC to patients with renal or resorptive HC. No statistical difference was found between the groups, however, BMC values in absorptive HC were different from normal (p less than 0.001). Why patients with HC are demineralized is unclear since no correlation was found between BMC and basal values of serum phosphate, TRP, calculated TmP/GFR, urinary calcium or hydroxyproline. Nevertheless our results indicate that urolithiasis, and possibly its treatment, is not a benign condition for the skeleton.

Endoscopic injection of Teflon for female urinary incontinence.

56 women with urinary stress incontinence were treated by transurethral Teflon injection. 48 of these patients (86%) were completely cured or greatly improved. No abscess or other complications were observed. Hospital stay is reduced to 2-3 days. The procedure is easy to perform and does not interfere with further surgery, should it be necessary. It is particularly useful after failure of previous surgery and in obese and debilitated patients.

Differences in repeated psychiatric examinations of litigants to a lawsuit.

Repeated psychiatric examinations of 42 litigants were compared to determine reasons for differences in findings by experts hired by the defendant and by the plaintiffs. The lawsuit resulted from the collapse of a coal slag heap in Buffalo Creek, W. Va., in 1972. All psychiatric reports prepared for the trial were screened. Experts for the two sides differed systematically over the extent of recovery from psychiatric symptoms and in other areas. Changes in the plaintiffs' mental statuses over time are shown not to be the cause. Extraneous factors, such as "forensic identification," the subtle influence of adversarial proceedings on initially neutral witnesses, are shown to play a part.

Subtotal pancreatectomy in the chicken: evidence of a diabetic state and effect on the somatostatin cells of the digestive tract.

The effect of subtotal pancreatectomy on the regulation of plasma glucose was studied in the chicken. The eventual appearance of an extra-pancreatic source of insulin and the effect of pancreatectomy on somatostatin containing cells of the intestine were also investigated. Over a period of 3-5 weeks, pancreatectomy decreased body weight gain, greatly impaired glucose tolerance and hindered the regulation of plasma glucose levels after a meal. A large deficit in the insulin release induced by a meal was noticed after pancreatectomy. Therefore, in the fed chicken, pancreatectomy results in the typical signs of diabetes. No extrapancreatic source of insulin could be found either by radioimmunoassay of extracts of proventriculus, gizzard, gizzard-duodenum junction and duodenum from controls or depancreatized chickens, or by immunohistological studies in the same organs plus pancreatic ducts. No glucagon producing cells could be found by immunohistological studies in the gastrointestinal tract. The size of the somatostatin cells from the gizzard-duodenum junction was negatively related to the magnitude of the glucose-induced insulin release, which suggests an increase in somatostatin cell activity in true insulin-deficient depancreatized chickens as observed in insulin deficient diabetic mammals.

Renal transplantation in heroin addicts.

On the basis of our experience, therefore, we believe that the reformed heroin addict is a good candidate for cadaveric donor renal transplantation if he can be discouraged from returning to heroin use. The fact that the majority of the patients in our study returned to marijuana smoking after transplantation has raised the issue of its role as a possible immunosuppressant.

Whole-body autoradiographic study of the distribution of 195mPt in healthy and tumor-bearing mice treated with labeled cisplatin.

195mPt-labeled cisplatin was administered iv and ip to control mice and to mice bearing Sarcoma 180. The delivered dose was 8 mg/kg and the injected activity was 80 muCi/kg. Whole-body autoradiography was performed at various times after treatment. For all animals, concentration of radioactive material was observed in the kidneys as early as 5 minutes after treatment. Most of the delivered radioactivity was present in the urine-bladder for the first 4 hours after the injection. Autoradiography also showed concentration of 195mPt in the bile ducts. These results suggest that a lower but significant part of the delivered radioactivity was also eliminated in the bile, which may explain in part the presence of the isotope in the gut lumen. In addition, 195mPt-equivalents were observed in the kidney parenchyma, the cartilages, the teeth, the skin, and the hair. Only trace amounts of 195mPt were present in the hypophysis and the testes. The distribution of radioisotope in healthy and tumor-bearing animals was comparable. Measurable uptake occurred in the Sarcoma 180 tumor.

Blood pressure and proteinuria in adolescents.

The study was conducted in two groups of factory apprentices including boys aged 15 to 17 years. The first group consisted of a randomly selected 10% sample of 231 apprentices of the total number of 2,300 boys. The second group included 77 boys with casually found proteinuria. In both groups, the distribution of blood pressure and of the body mass and changes in these indices were followed up over a period of two years. The boys with proteinuria had a significantly higher systolic and diastolic blood pressure and a lower body mass index. Their systolic and diastolic blood pressure was rising more steeply in regression to the body mass. The casual finding of asymptomatic proteinuria in juveniles always calls for a long-term follow-up. It signalizes, inter alia, also the presence of a factor raising blood pressure probably independently of body height and body mass.

Latent ischaemic heart disease among the male employees of an industrial enterprise.

Among 3758 men aged 40-50 years--employees of an industrial enterprise, 291 cases of suspected myocardial ischaemia (7.7%) were detected by means of resting ECG. Further 311 cases (8.3%) of latent ischaemic heart disease (IHD) were detected using a standard cardiovascular questionnaire, and 47 other men were positive both on resting ECG (ischaemic changes) and on the questionnaire. Latent IHD was equally frequent among blue and white collar workers. Ischaemic ECG findings were positively associated with a positive questionnaire, presence of hypertension, and hypertension or stroke in family history, and inversely with the alcohol consumption. No relation was found between ischaemic ECG and smoking, hypercholesterolaemia, obesity, education, and physical activity at work or during leisure time. Positive questionnaire data correlated with hypercholesterolaemia; unexpectedly this correlation was closer among non-smokers and moderate smokers than among heavy smokers.
