[A case of intrahepatic bile duct cancer responding to 5-fluorouracil, adriamycin and mitomycin C chemotherapy].

A 64-year-old woman who was diagnosed as inoperable intrahepatic bile duct cancer with massive liver metastasis was treated with 5-fluorouracil (5-FU), adriamycin (ADM) and mitomycin-C (MMC). Obstructive jaundice was reduced and performance status was soon improved by this regimen. The reduction rate of the tumor was noted to be more than 50% on CT images, so this patient was considered to have achieved a partial response. In histological examination, well differentiated tubular adenocarcinoma cells were degenerated and replaced fibrous tissue. Thus, it was considered that the reduction of the tumor was caused by these phenomena. This case suggested the activity of 5-FU, ADM and MMC combination for bile duct cancer.

[Ovicidal effects of heat and disinfectants on Syphacia muris estimated by in vitro hatching].

The ovicidal effects of heat and various chemical disinfectants on an oxyurid rat nematode Syphacia muris were investigated, using the hatching methods in artificial intestinal juice. The eggs were collected from the perianal skin of spontaneously infected rats by means of a piece of transparent adhesive tapes, and these eggs were treated with each disinfectant for two hours. It was found that 70% ethanol and 80 degrees C 30 min treatments killed almost all of the eggs. However, a small number of the eggs tested was killed by 0.02% chlorhexidine digluconate or 0.05% benzethonium chloride. Alcide, 3% saponated cresol solution, 50% isopropanol, 10 ppm sodium hypochlorite and 5 ppm iodophol had some effects against the eggs, but they didn't kill the eggs completely. A biological assay through infection of the eggs to rats might be necessary because the effects of 2% formalin on the eggs were not determined by the hatching methods.

[Necessity of adjuvant therapy after radical mastectomy based on a study of patients with negative nodes, who have recurrent breast cancer].

The status of the regional lymph nodes influences the prognosis of women with breast cancer who have received a radical mastectomy. To investigate the characteristics of patients with negative nodes who have had a recurrent breast cancer, a statistical study has been carried out involving 279 women who had received a radical mastectomy in our hospital from July 1970 to October 1986. From this study we conclude that systemic adjuvant therapy after radical mastectomy is necessary for patients with negative nodes who have had a breast cancer greater than 2.1 cm (T2) in diameter or a solid-tubular carcinoma.

[Improvements in placement of implantable drug delivery catheter for prevention and treatment of hepatic metastases from colorectal cancer].

Intra-arterial infusion chemotherapy was carried out using a subcutaneously implantable drug delivery system in 7 patients with rectal cancer, 5 patients with sigmoid colon cancer, 2 patients with transverse colon cancer, and 1 patient with ascending colon cancer, for prevention and treatment of hepatic metastases. The transport of intra-arterially infused drugs to the liver was examined during surgery using an infusion catheter, which was inserted by the fluorescence technique using Fluorescein-Na. The gastroepiploic artery was shifted to the abdominal wall to avoid leaving the infusion catheter as a foreign body after removal of the port. The fluorescein test, which is safe and can be performed in a short period by a simple procedure, is considered to be useful for intraoperative evaluation of the distribution of the drug to the liver. Since the infusion catheter remaining in the body after removal of the port may cause thrombosis and infection, measures to avoid leaving foreign bodies are considered to be necessary.

Anticancer drug distribution in lymph and blood during adjuvant chemotherapy after surgery for gastric carcinoma. A study with a combined preparation of 1-(2-tetrahydrofuryl)-5-fluorouracil and uracil.

Previously the authors reported that the fat emulsion of 1-(2-tetrahydrofuryl)-5-fluorouracil, tegafur (FT-207), yielded significantly higher concentrations of tegafur in the lymph and plasma compared to tegafur enteric-coated granules (FT-G). However, the emulsification did not improve the metabolic conversion rate of tegafur to 5-fluorouracil (5-FU). A study was performed to assay the plasma and lymphatic concentrations of tegafur, 5-FU, and uracil in seven patients after radical surgery for gastric carcinoma who were given a combined oral preparation of FT-207 and uracil (UFT). Both lymph and plasma 5-FU levels after UFT were 20 times greater than those after FT-G, although FT-207 levels were not different. Patients given UFT showed significantly greater 5-FU and uracil concentrations in the lymph compared with the plasma. The results of this study suggest a potential use of UFT as an adjuvant postoperative chemotherapeutic agent for gastric carcinoma.

Benign osteoblastoma of the cervical vertebra.

A 31-year-old patient, with a relatively uncommon benign osteoblastoma of the sixth cervical vertebra, exhibited symptoms of spinal cord compression. Computed tomographic metrizamide myelography was very useful in the preoperative planning, particularly for locating the position and extent of the lesion. The tumor was totally removed via posterior and anterior approaches in a two-stage operation. Roentgenograms, taken 1 year and 6 months postoperatively, showed good bony union with no evidence of tumor recurrence.

[An operated case of multiple endocrine neoplasia type IIb and review of the Japanese literature].

Multiple endocrine neoplasia, type IIb (MEN IIb) is a rare syndrome characterized by the occurrence of medullary thyroid carcinoma (MTC), pheochromocytoma and mucosal neuroma. A 35-year-old male patient with MEM IIb having megacolon, marfanoid habitus and no family history of the disease underwent surgery. Because MTC was present in both lobes, total thyroidectomy and modified neck dissection were performed. Pheochromocytoma was found bilaterally and bilateral adrenalectomy with adrenal autotransplantation in the rectus abdominis muscle was carried out. Postoperative course was satisfactory except for transient hypocalcemia and mild ileus. After the slow corticosteroid weaning process, his adrenocortical function was at the lower level within a normal range. In August 1986 (24 postoperative months), he was maintained by the administration of 10mg of hydrocortisone every three days, and calcitonin and CEA levels in sera were normal. We collected 15 cases reported in Japanese literatures. MTC and mucosal neuroma were found in all cases, whereas pheochromocytoma was present in 9 cases. Bilateral and multicentric occurrences were usual, and total thyroidectomy and bilateral adrenalectomy were, warranted. We believe that autotransplantation following bilateral adrenalectomy is a worthy alternative.

Surgical treatment of symptomatic atlantoaxial subluxation in Down's syndrome.

Six cases of relatively uncommon symptomatic atlantoaxial subluxation in Down's syndrome were treated surgically. In all cases, various congenital osseous anomalies were found. Marked myelopathy was noted in four cases of ossiculum terminale and mild myelopathy in one case of anteroposterior spondyloschisis. Five cases were treated successfully with posterior arthrodesis of C1 to C2 and one from the occiput to C2. Excellent bony union and overall improvement in the patients' symptoms was achieved, with a follow-up period ranging from 11 months to three years and nine months. In cases of symptomatic atlantoaxial subluxation of Down's syndrome, where the spinal cord is at risk, early surgical stabilization is the treatment of choice.

[Tumor markers in human breast cancer].

We described tumor markers which are considered to be useful for the detection of recurrence, index of the efficacy of treatment and assessment for the prognosis of the patients with breast cancer. CEA and CA 15-3 are relatively useful markers among various serological ones. However, although accuracy of the diagnosis with CA 15-3 and CEA is generally thought to be not superior to those of imaging method, CA 15-3 and CEA seems to be clinically useful tools for making diagnosis because of its simplicity and less cost. Estrogen and progesterone receptors shows good responses to endocrine therapy. Response rate was 70 to 80% in both receptor-positive patients. Presence of ER shows higher rate of recurrence and shorter survival of the patients than those with absence of ER. Recent attention has been focused on EGF and proto-oncogene, suggesting relationship of those to prognosis.
