[Pseudomyxoma peritonei: a case with multiple metastases. Ultrastructural study and chemical analysis of the mucoid substance].

A patient with spreading pseudomyxoma peritonei probably caused by a tumor of the appendix showing suggestive clinical features is presented. The long evolution was marked by the discovery of gelatinous substance in the stools and in the lumen of the gut, and the appearance of distant metastasis in the lungs, adrenal glands and lymph nodes. Histochemical studies showed that periodic Shiff acid and Alcian blue were fixed by the mucoid substance. The use of toluidin blue (pH 1) revealed metachromosia. Electron microscopy ascertained the presence of enterocytes and mucous cells and revealed a more obvious nuclear disorganisation than by histological microscopy. Biochemical study showed that the mucoid substance extracted from the pseudomyxoma was rich in carbohydrates and could be identified as a mucin on the basis of the composition of its monosaccarides and of the alkali-lability of its glycanprotein linkages. The mucin was weakly hydrolysed by trypsin in vitro but was deeply attacked by the same enzyme after desialylation. These results: a) confirm the possibility of observing spreading distant metastasis in the pseudomyxoma peritonei; b) underline the potential interest of electron microscopic study in case of wavering diagnosis; c) show that the mucoid substance belongs to the group of neutral sulfomucins; d) legitimate, in the future, a trial of palliative treatment by neuraminidase in such cases.

[Gastric pseudolymphomas. Apropos of 3 cases. Review of the literature].

The authors report three cases of gastric pseudolymphoma, before going on to review the literature on the subject. The pseudolymphoma has only recently been identified in relation to other primitive gastric lymphoid growths, on the basis of histological and evolutional criteria, which show it to be benign. It raises two problems. Firstly, a practical problem of diagnosis, as it often visualizes radiologically and endoscopically as a malignant tumor, and this diagnostic uncertainty persists, even after endoscopic biopsy. This necessarily implies surgical management of the case, normally leading to a clear diagnosis based on study of the gastrectomy specimen. Histologically, a growth of this nature consists of a very large lymphoid hyperplasia, often of an organoid type, strictly confined to the mucosa, which, incidentally, shows no real recent or ancient ulceration. Cytologically, this lymphoid infiltrate is benign and the neighbouring nodes are always normal. Point by point, therefore, these characteristics distinguish the pseudolymphoma from the real lymphoma and the lymphoid hyperplasia associated with ulcers. Fluorescence microscopic study of the intracytoplasmic immunoglobulin, which was carried out in the cases treated by the authors, and also in another recent case reported in the literature, shows a polyclonal type immunofluorescence. The pseudolymphoma is therefore, a benign form of lymphomatosis without ulceration. The second problem concerns the real nature of the lesion: a local immunosuppressive pathology? An atypical lymphoid presarcomatous hyperplasia? In order to support these hypotheses, fresh tissue lymphocyte typing tests covering the entire field of digestive lymph pathology should be carried out to complete the intracytoplasmic immunofluorescent study of the immunologically competent cells.

Clinico-pathological conditions associated with pancreas divisum.

Nonfusion of the pancreatic ducts (pancreas divisum) was diagnosed in 41 of 812 patients in whom ERCP provided a definite diagnosis (5.04% of cases). Pancreas divisum was significantly associated with diseases of the pancreas. Chronic pancreatitis was less frequently observed in cases of pancreas divisum whereas acute pancreatitis, especially acute recurrent pancreatitis, was statistically more frequently seen in patients in whom the anomaly was present. These results support the hypothesis that pancreas divisum favours the development of acute pancreatitis.

[Modifications of pure pancreatic secretion in patients with chronic renal failure treated by hemodialysis (author's transl)].

In 20 patients suffering from chronic renal failure treated by hemodialysis (CRFH) the pure pancreatic juice (PPJ) collected via endoscopic cannulation of the papilla was studied. PPJ was collected during 20 minutes in 1 minute samples. Secretin and pancreozymin (CCK-PZ) were given as boluses respectively at the beginning and at the 10th minute of the test. Volumes, total protein and bicarbonate concentrations, total protein and bicarbonate outputs, Lactoferrin (LF) levels were measured. The results observed in patients were compared to those of ten subjects free of any organic disease. The volume of PPJ was significantly more important in CRFH than in controls. Bicarbonate and protein concentrations were not significantly modified. Protein output was not significantly more important in CRFH at the exception of the 12th sample. Bicarbonate output was not significantly increased at the exception of the 9th and the 11th samples. LF level was more than 0.03% of total proteins in 4 CRFH.

[Ultrasonographic appearances in three cases of pancreas divisum (author's transl)].

Pancreas divisum is an embryological malformation resulting from imperfect fusion of the dorsal and ventral parts of the gland. In this not infrequently observed anomaly, the excretory systems of the two parts remain independent, secretion being through the principal papilla for the ventral, and the accessory papilla for the dorsal pancreas. The pathogenic role of this malformation is now well established. Ultrasonographic findings in the three cases reported, particularly the intrapancreatic pathway of the superior mesenteric vein, were sufficiently atypical for this congenital malformation to be suspected.

A cooperative study on the detection of colorectal cancer and polyps in France.

Systematic investigations testing for the presence of polyps and cancers of the large bowel were conducted in a population of 1,369 inpatients and outpatients, aged 45-70 years, in eight university departments of gastroenterology or abdominal surgery (Toulouse, Dijon, Paris, Marseille, and Strasbourg). Double-contrast barium enema and proctosigmoidoscopy examinations were carried out in all cases, whereas total coloscopy was performed only in the case of detected tumors. A questionnaire including 233 parameters (age, sex, family, and personal history and symptoms) was completed for each patient. A total of 414 lesions were detected in 252 patients, including 245 adenomatous or villous polyps, 8 transformed polyps, and 30 carcinomas; 3 of 4 lesions were located in the rectum or sigmoid. Cancer or adenomatous or villous polyps were found in 13% of the patients. The prevalence of these lesions in the population studied was increased in patients with rectorrhagia (19%) or with a personal history of surgery for colorectal cancer or polyp (23%). In the patients without rectorrhagia or a history of intestinal tumor, the incidence was 9.7%. It was significantly increased in males and patients more than 50 year old. The efficacy of proctosigmoidoscopy and double-contrast barium enema was compared in 909 patients. Sensitivity and specificity were, respectively, 35% and 99% for endoscopy, 96% and 94% for radiology.

Lactoferrin and albumin in human pancreatic juice: a valuable test for diagnosis of pancreatic diseases.

Lactoferrin and albumin concentrations in pure pancreatic juice were studied by radial immunodiffusion in 81 patients. Lactoferrin concentration (% of total protein) was specifically increased in patients suffering from chronic calcifying pancreatitis (mean +/- SE: 0.169 +/- 0.024) and no overlap was observed with patients suffering from other pancreatic diseases (0.008 +/- 0.001), organic nonpancreatic diseases (0.011 +/- 0.001) and without organic disease (0.01 +/- 0.001). Lactoferrin concentration was not found to be correlated with chronic alcohol consumption or pancreatic insufficiency. Albumin concentration (% of total protein) was increased in patients suffering from chronic calcifying pancreatitis (5.55 +/- 0.49) and other pancreatic diseases (5.45 +/- 1.02). These values were significantly higher (P less than 0.001) than the corresponding values in patients with organic nonpancreatic diseases (1.66 +/- 0.16) and patients without organic diseases (1.46 +/- 0.22). Measuring lactoferrin and albumin in pancreatic juice may aid in the diagnosis of pancreatic disease and in the differentiation of chronic calcifying pancreatitis, even in the earliest stage, from other pancreatic diseases such as pancreatic carcinoma or acute pancreatitis.

Jaundice and chronic pancreatitis.

A study was conducted on a group of 48 patients presenting with jaundice during the course of chronic pancreatitis. Two different patterns of symptoms could be opposed: (1) Short-lasting cholestatic jaundice which is accompanied by pain, but free from pruritus and fever, is linked to a medium consumption of alcohol, occurs in the course of advanced chronic pancreatitis and seems to be the consequence of a stricture of the main bile duct due to pancreatic lesions. (2) Protracted, intense jaundice which is linked to a high alcohol consumption, is encountered among less-advanced cases of chronic pancreatitis, the main bile duct being generally normal. In such a case, direct toxic effect of alcohol on the liver must be questioned.

Lipase activity in blood following endoscopic pancreatography: demonstration of its pancreatic origin and existence of ductal or acino-venous pathways in man.

The presence of a lipase activity has been observed in the blood of patients undergoing an endoscopic retrograde cholangio-pancreatography (ERCP). The identity between this serum lipase and pancreatic lipase has been demonstrated by their similarity in in chromatographic, immunological, and catalytic properties. Lipase activity in serum increased when pancreatography was accompanied by a passage of the contrast medium to the kidney demonstrating the presence of ductal or acino-venous pathways in man.
