Pseudo second degree atrioventricular block with bradycardia. Successful treatment with quinidine.

Pseudo second degree atrioventricular block resulting from blocked His premature beats was successfully treated with quinidine. The diagnosis was proved by His bundle electrogam which showed both blocked and conducted His premature beats. The blocked His prematures produced second degree atrioventricular block by making the atrioventricular junction refractory. Quinidine abolished both conducted and blocked His extrasystoles. There has been no recurrence of arrhythmia during a one-year follow-up.

Identification of an antigen associated with malignant melanoma.

Xeno-antisera, prepared by immunization with soluble membrane material obtained from melanoma tumours and partially purified by column chromatography, were found to detect an antigenic specificity common to some tumour extracts and sera of melanoma patients. The presence of this antigen in patients' sera allows speculation for its possible use for early diagnosis of metastases and its role in the blocking of the immunological defences of the melanoma patients. Preparation of monospecific antisera by immunization with insoluble serum-antigen/antibody complexes has been successfully undertaken.

Antenatal detection of fetal structural defects with ultrasound.

A case report of a cystic hygroma detected in utero by ultrasound at 16 weeks' gestation is presented. The potential for early second trimester detection of both CNS and non-CNS structural defects by diagnostic ultrasound is discussed "High risk" groups for fetal structural defects can be identified for screening purposes.

The current status of angiography in gastrointestinal bleeding.

Endoscopy, barium studies and angiography are all valuable diagnostic procedures in gastrointestinal bleeding. They are not mutually exclusive procedures but the correct sequence of investigation varies. Correct localization can be achieved in 84% of cases of massive gastrointestinal bleeding by their combined use. Vasoconstrictive therapy is also possible in severely ill patients and other poor surgical risk candidates.

A collagenase in extracts of the invertebrate Bipalium kewense.

The localization, isolation and partial characterization of a collagenolytic enzyme from the land planarian Bipalium kewense is described. This enzyme can be obtained by direct extraction of the organism, and can be separated from non-collagenous proteolytic activity by (NH(4))(2)SO(4) precipitation and Sephadex-gel chromatography. Its mode of attack on collagen and sensitivity to a variety of inhibitors indicate that this enzyme differs from vertebrate collagenases and a previously described invertebrate collagenase.
