A survey of our first 50 patients having craniofacial surgery was done. An attempt was made to determine the impact of facial deformity on social function for both the patient and his family and how function was altered by surgical correction. This was accomplished by a uniform series of questions. All patients were at least one year postoperative. The results emphasize that facial deformity played a dominant role in patients' lives prior to surgery and that surgical intervention led to improved social functioning in a number of areas. Meaningful data was collected on three groups of patients: 11 adults, 18 adolescents, and 17 children. Changes perceived were strikingly different in the three groups. Eight-seven percent of all patients would make the decision to have surgery again, and at least 50 percent had objective evidence of improved function. However, the survey also suggests that the extreme stress produced by the treatment may create family problems for which support is necessary.

Electronic parameters related to the cytochrome P450-catalyzed reactions of eight polycyclic aromatic amines have been calculated using all valence electron semiempirical molecular orbital methods. The reactions considered lead to the presumably carcinogenic arylnitrenium ions and to the competing hydroxylation and epoxidation products. The stabilities of the arylnitrenium ions relative to the N-hydroxylamines and their sulfate esters were also calculated, together with electrophilic reactivity parameters of the ions. The resulting parameters were used to predict major metabolites of the parent compounds and also to correlate with observed mutagenic activities of the four pairs of polycyclic aromatic amines studied. The major factor in determining mutagenic potencies of parent compounds appears to be the extent of N-hydroxylation and competing ring oxidations, as well as the electrophilic properties of the arylnitrenium ions.

A 32-year-old male with the clinical syndrome of unstable angina is presented. During cardiac catheterization and coronary cineangiography, the coronary arteries were found to be free of atherosclerosis, but segmental spasm of the right coronary artery was observed. Spasm was reproduced, but to a milder degree, upon the administration of ergonovine maleate intravenously. The patient was discharged on a regimen of oral nylidrin HCL, with symptomatic improvement.

Animal immune RNA used for the induction of the human lymphoblastoid cell line LDV/7, induces the appearance of receptor sites on the surface of these cells and the synthesis of specific immunoglobulins which are liberated into the culture medium. Furthermore, RNA has been extracted from these cells, which possesses the same properties of specific cytotoxicity as the RNA used for induction. It is supposed that the immune RNA derepresses specific genes on the genome of the induced cells.

The electrocardiograms of 65 patients with the "early repolarization syndrome" (normal variant of RS-T elevation) were analyzed to delineate the features and evaluate the natural history of this electrocardiographic entity. Maximal follow-up was 26 years. The syndrome was characterized by (1) an upward concave elevation of the RS-T segment with distinct or "embryonic" J waves, slurred downstroke of R waves or distinct J points or both; (2) RS-T segment elevation commonly encountered in the precordial leads and more distinct in these leads; (3) rapid QRS transition in the precordial leads with counterclockwise rotation; and (4) persistence of these characteristics for many years although some intraindividual changes were common. Less commonly found were (5) tall R and T waves in the precordial leads; (6) "labile" or "juvenile" T wave patterns; (7) "pseudo-R" waves; and (8) "isolated T negativity syndrome." These changes commonly simulate pericarditis, myocardial ischemia, left ventricular hypertrophy and right bundle branch block.
