High prevalence of HLA DR7 was well documented in idiopathic nephrotic syndrome (INS) of childhood. Our data in 43 INS, child and adult patients, did not show any significant difference from control group, specially for HLA DR7. Moreover a significant difference occurred when the data was compared in child and adult INS patients. HLA DR7 was more frequent when the nephrotic syndrome appeared before 15 years old. There was no correlation between the presence of HLA DR7 and those of allergy history or increased serum IgE level, on the opposite with previous data. The presence of DR7 would favor its onset at an earlier age. Alternatively, the difference in DR7 prevalence could be an additional argument for considering that the corticosensitivity and negative histological criteria might collect, under the same name, different glomerulopathies.

The hemolytic-uremic syndrome consist of micro-angiopathic hemolytic anemia, acute renal failure, and thrombocytopenia following a prodromal illness of gastroenteritis. The syndrome can present in dramatic fashion with severe abdominal pain and signs of peritonitis suggesting an acute surgical crisis. In some cases, laparotomy is necessary, and we report a case with colectomy.
Variations in the upper limit of normal total serum IgE have been reported: they can range from 150 to 1,000 UI/ml; but the usually accepted upper limit is between 150 and 300 UI/ml. In this study we measured the total serum IgE levels (RIST-Pharmacia) in 69 randomly selected control subjects; 38 had no previous history of atopic or parasitic disease. The normal values observed were 7-460 UI/ml for the whole of the control group (10th-90th percentiles) and 0.5-540 UI/ml for the 38 selected subjects (5th-95th percentiles). Logarithmic conversion is compulsory in order to obtain a gaussian distribution. This enables 80 p. 100 of the control subjects to be grouped within 4-450 UI/ml or 90 p. 100 of the selected subgroup within the range of 1.5-677 UI/ml. As many allergic patients have a normal serum IgE level and a number of non-allergic diseases may be associated with an increased serum IgE level, the diagnostic importance of this investigation is limited. The cost-effectiveness is very low and raises the problem of using this test in everyday practice.

In three cases of minimal change nephrotic syndrome (lipoid nephrosis), an allergen sensitivity to house dust, cat hair and grass pollen was demonstrated by clinical and biological data. An increase in proteinuria after specific allergen contact and a decrease after withdrawal of the allergens, suggested that the cause was allergenic. Such cases demonstrate the necessity of an etiological investigation of particularly the atopic features in lipoid nephrosis. Results of these investigations can lead to specific therapy in selected cases.
Serum IgE was measured by RIST test in 181 adult subjects, with log transformation. In 24 controls the geometric mean was 109 IU/ml, with an upper limit (geometric mean +/- 2 SD) of 460 IU/ml. 157 cases of glomerulonephritis (GN) classified according to light and immunofluorescence microscopy were studied. Serum IgE was significantly elevated in 39 lipoid nephrosis patients (p less than 0.001, with 21 levels greater than 460 IU/ml), 21 focal and segmental glomerulosclerosis (p less than 0.05, with 8 levels greater than 460 IU/ml) and 42 membranous GN (p less than 0.01, with 10 levels greater than 460 IU/ml). In 27 membrano-proliferative GN and 28 IgA GN, serum IgE was not different from controls with respectively 4 and 3 levels greater than 460 IU/ml. In addition, the lipoid nephrosis IgE geometric mean is 493 IU/ml, which is keeping with the high incidence of atopic troubles. The significance of raised serum IgE was discussed.
Certain types of glomerulonephritis develop typically as the result of focal infection, and may recover following effective treatment of the infective focus at cause. Apart from the classical immunity mechanism consisting of the formation of immune complexes with the bacterial antigen, a major role for environmental allergens must also be evoked, as well as the probable existence of different immunity mechanisms at the origin of certain types of GN (mainly mesangial GN with IgA deposits and lipoid nephrosis). Invasion of the pulmonary allergen through a rhinosinusal route is responsible for a rhinosinusitis demonstrating a typical clinical features and course. Progression of the rhinosinusitis runs parallel with that of the GN. The course of the GN may be improved after treatment of the sinusitis and the allergic disposition. Ten particularly significant case-reports of GN associated with rhinosinusitis are studied from various aspects.
Con A stimulated lymphocytes from some patients with primary glomerular disease release in vitro a lymphokine, the vascular permeability factor (VPF). House dust sensitization was early reported among these patients. We therefore studied the action of this specific mitogen on VPF production in 58 patients with primary glomerular disease, whose 23 chosen because cutaneous house dust sensitivity. For the whole patients, house dust and Con A induced VPF production were statistically equivalent and both higher than that of unstimulated lymphocytes. Lymphocytes from the 23 house dust sensitised patients, stimulated by the allergen, produced more VPF than lymphocytes from unsensitised patients.
Atopic factors play an important role in minimal changes idiopathic nephrotic syndrome ( MCINS ). We therefore studied human basophil degranulation test ( HBDT ) in MCINS and also in patients with segmental and focal glomerulosclerosis ( SFGSNS ) whose relation with MCINS is debated. HBDT was performed with 5 or 6 allergens chosen according to the history of patients and results of cutaneous tests. HBDT was positive in 16 out of 28 MCINS and in 14 among 18 SFGSNS subjects. The difference versus 29 Immune Complex glomerulonephritis and 11 blood donors was significant with the chi 2 test. Because the basophil count was low (less than 10/microliter) in 5 cases, 4 of which had a severe nephrotic syndrome, the HBDT was performed later. The allergen most frequently involved was house dust. This was responsible for a significant degranulation in 10 MCINS (45%) and in 11 SFGSNS (68%) patients; degranulation was obtained more frequently with 2 allergens than with a single one. Obviously such a basophil sensitisation does not prove the responsibility of these allergens.
