Is adult idiopathic nephrotic syndrome food allergy? Value of oligoantigenic diets.

To evaluate the prevalence of food hypersensitivity in idiopathic nephrotic syndrome (INS), an oligoantigenic diet was given for 10 days to 13 patients with INS who had not steadily responded to corticosteroids; 5 of these patients were corticodependent, 3 were corticoresistant and 5 had multiple relapses. A history of allergy was present in 8 cases and an increase in total serum IgE levels in 6. Sensitivity to food antigens was documented by skin tests in 5, by the radioallergosarbent test (RAST) in 5 and by the human basophil degranulation test (HBDT) in 11. At the end of the oligoantigenic diet, proteinuria was significantly reduced in the 13 patients as compared to the initial level. It decreased by more than 50% in 9 patients and disappeared completely in 5. It seems that an oligoantigenic diet is helpful in cases of INS that do not respond to corticosteroids. This argues for a role of food hypersensitivity in this disease and suggests that avoidance of specific foods on the longer term may be of benefit. This diet must be tried in INS before the initiation of immunosuppressive therapy.

Effect of protein malnutrition and maternal caffeine intake on the growth of fetal rat brain.

Pregnant dams were divided into two subgroups on day 10 of gestation. Half were fed a 20% protein diet and the other half an 8% protein diet. A second group also subdivided was pair-fed with rats of the first group. Their diet was supplemented with caffeine in amount calculated to provide daily doses of 2 mg/100 g body weight. On days 18, 20, and 22 randomly selected dams were injected with 3H-thymidine intraperitoneally and 1.5 h later their fetuses were delivered surgically in order to determine the rate of DNA synthesis along the gestation. The rest of the fetuses were delivered surgically on day 22. Pups' brains were rapidly removed and DNA, RNA, protein and 3H-thymidine uptake were studied. Average body weights of the fetuses in the caffeine-supplemented control group were smaller than those of the noncaffeine group. Effects of caffeine that were similar in both diet groups included a decrease in brain DNA content and concentration and an increase in brain protein content and concentration. However, the percent decrease and increase, respectively, was different depending on the nutritional status. DNA synthesis was not affected by malnutrition or caffeine supplementation on day 18 of gestation. Caffeine's effect on the rate of DNA synthesis was different on day 20 of gestation depending on nutritional status. Caffeine supplementation resulted in a decrease in DNA synthesis in both groups on day 22 of gestation. These data indicate that caffeine intake during pregnancy produces differential effects on fetal rat brain depending on dietary protein content.

[Effectiveness of piperacillin in the antibacterial treatment of intra-abdominal infections in children].

The effectiveness of piperacillin was investigated in 30 children operated upon for peritonitis: 13 had acute appendicitis with puriform peritoneal reaction, or a recently perforated appendix; 5 had generalized peritonitis of appendicular origin, and 13 had intraperitoneal abscess. In the 12 children who underwent right iliac appendicectomy (with post-operative drainage in 3), piperacillin was administered alone during 5 days; clinical and bacteriological cure was obtained in all cases; the mean duration of stay in hospital was 7 days. The 5 cases of generalized peritonitis required drainage; piperacillin was given alone in 4 of them and combined with an aminoglycoside and metronidazole in one who was in poor general condition. Bacteriology showed a predominance of Escherichia coli alone or associated with other organisms. Clinical and bacteriological cure was obtained in 3 patients; the mean duration of stay in hospital was 12 days. Seven of the 13 cases of intraperitoneal abscess needed drainage. Piperacillin was administered alone for 7 days on average in 10 cases and combined with an aminoglycoside and metronidazole in 2 cases. Eight patients had a favourable course, 5 developed complications. In all 3 groups piperacillin was tell tolerated. A patch of urticaria was noted in 2 cases and a transient skin rash in 2 other cases. No neutropenia was observed in these children whose treatment never exceeded 10 days.

[Prolonged preventive treatment of hereditary angioneurotic edema with anabolic androgenic steroids].

Since 1977, 22 patients with hereditary angioneurotic oedema have been treated with androgenic-anabolic steroids (danazol or stanozolol). The most frequent adverse reactions were myalgia, weight increase, changes in libido, and menstrual disorders in women. In view of these side-effects, it is suggested that patients about to undergo surgery (chiefly E.N.T. or stomatology) should be put, one week before the operation, on a 10-day high dosage treatment. Long-term treatment should be reserved to severe forms of the disease, with frequent attacks and visceral involvement, using the minimum effective dosage (which is always low) without trying to correct biochemical abnormalities. Stanozolol should be given by preference to men and danazol to women. Treatment is always more difficult in pre-menopausal women. With this method, the complications, notably hepatic disorders, are rare, and long-term treatment, when absolutely necessary, is well tolerated.

Basophil sensitization for food allergens in idiopathic nephrotic syndrome.

To estimate food sensitization, the human basophil degranulation test (HBDT), usually used for allergy diagnosis, was performed in 34 idiopathic nephrotic syndrome (INS) patients, classified as minimal change glomerular disease, with or without atopic manifestations, and in 19 blood donors; five food allergens (wheat flour, cow's milk, whole egg, beef and pork) were investigated. The HBDT was positive in 22 out of the 34 INS (64%) and in 1 out of the 19 blood donors (p less than 0.001). The test was 9 times positive for wheat flour, 9 times for pork, 6 times for cow's milk, 6 times for beef and 5 times for whole egg. Moreover in 13 cases, the basophil sensitization was present for two or three of these food allergens in the same patient. A direct causal relationship between food sensitization and INS cannot be drawn from this study: it might be demonstrated only by elemental or oligoantigenic diet prior to specific avoidances.
