Stability and compatibility of admixtures of intravenous ciprofloxacin and selected drugs.

The stability and compatibility of ciprofloxacin with selected drugs in intravenous admixtures were studied. Ciprofloxacin 2 mg/mL in 5% dextrose was combined with each of 22 other drugs at concentrations commonly used in clinical practice. Each combination was maintained at room temperature (approximately 22 degrees C) in constant fluorescent light. Immediately after preparation and at 6 and 24 hours, each admixture was examined visually in normal fluorescent room light and the pH value was determined. For samples lacking visible precipitates or having pH changes of not more than 1 unit, ciprofloxacin concentration was assayed by using high-performance liquid chromatography. When combined with ciprofloxacin, 14 of the study drugs did not alter the concentration of ciprofloxacin, including amikacin sulfate, atracurium besylate, aztreonam, cimetidine hydrochloride, dobutamine hydrochloride, fluconazole, gentamicin sulfate, metronidazole (intravenous, ready to use), midazolam hydrochloride, norepinephrine bitartrate, pancuronium bromide, potassium chloride, tobramycin sulfate, and vecuronium bromide. There were five drugs that were determined to be incompatible with ciprofloxacin because of precipitate formation (amphotericin B, ampicillin sodium/sulbactam sodium, cefuroxime sodium, piperacillin sodium, and sodium bicarbonate). Incompatibility with ciprofloxacin based on pH changes of more than 1 unit was found with four drugs: ampicillin sodium/sulbactam sodium, ceftazidime, metronidazole hydrochloride (powder only), and ticarcillin disodium/clavulanate potassium. Intravenous ciprofloxacin 2 mg/mL admixed in 5% dextrose was stable and compatible with 14 of the 22 test drugs for up to 24 hours at room temperature. The other eight drugs should not be combined with ciprofloxacin.

Investigations into the potential degradation of polycarbonate baby bottles during sterilization with consequent release of bisphenol A.

Twenty-four brands of plastic baby feeding bottles were purchased and all were found to be made of polycarbonate. Taking a batch of one representative sample, the polymer was tested for stability and possible release of bisphenol A following domestic practice of sterilization. Sterilization was by alkaline hypochlorite, steam, or washing in an automatic dishwasher at 65 degrees C with detergent. A total of 20 cycles of sterilization and subsequent food use were performed for each of the three procedures. Bisphenol A migration was in all cases not detectable in infant feed using a very sensitive method of liquid chromatography with fluorescence detection with a 0.03 mg/kg detection limit.

Administration of radiographic contrast media in high-risk patients.

Patients with a prior history of an anaphylactoid reaction (AR) to radiographic contrast media (RCM) have an increased risk of an AR during subsequent RCM studies. Based on previous studies in high-risk patients using prednisone or diphenhydramine to reduce the incidence of AR, high-risk patients were treated with a combined prednisone and diphenhydramine protocol in an effort to develop an effective, practical approach to this problem. All patients with convincing histories of AR to RCM with an essential need for a repeat RCM study received 50 mg of prednisone orally every 6 hours for 3 doses ending 1 hour before the RCM study, and 50 mg of diphenhydramine intramuscularly 1 hour prior to the procedure. Resuscitation equipment was readily available. One hundred forty-seven repeat procedures using RCM were carried out in 142 high-risk patients. No serious AR occurred. Two patients had generalized urticaria that resolved in 1 hour. The overall reaction rate in these pretreated high-risk patients was 6.8%, suggesting that this prednisone-diphenhydramine program may be the preferred prophylactic regimen when repeat RCM studies are necessary.

Student nurses' attitudes toward nursing research.

This study has various limitations and some generalizations have been made in the interpretations of the findings. However for nursing research to be of significance to students as a means of inquiry, problem solving and investigation, research concepts need to be introduced in nursing education programs as early as possible. Emphasis could be placed on demonstrating processes, application, participation, and environment. In conclusion, encouraging participation, a more comprehensive connection between research processes and demonstrating the relevance of research to nursing is crucial to students in preparing them for their impending role as a registered nurse.

Remission induction in refractory Crohns disease using a high calorie whole diet.

Eleven patients with severe refractory Crohns disease were treated with a high calorie, whole diet. Eight patients achieved remission with a drop in DAS from 13 to 4 (t = 6.31, p less than 0.001) and reversal of subacute obstruction in all cases. Nutritional parameters including weight, triceps skinfold thickness, arm muscle area, and serum albumin increased in all patients. The diet was well tolerated with a mean treatment period of 20 days. Clinical relapse of disease occurred in all cases within 9 months (mean 6 +/- 2 months). The mechanism of action of a high calorie diet (HCD) is unclear and warrants further study but antigen or specific food exclusion does not appear to be required as judged by this study.

Clinical, functional, and radiographic assessments of the conventional and modified Boyd-Anderson surgical procedures for repair of distal biceps tendon ruptures.

The purpose of this study was to evaluate the clinical, functional, and radiographic outcomes of the conventional and modified Boyd-Anderson procedures for repair of distal biceps tendon ruptures. Thirteen of 18 men who underwent surgical repair for unilateral distal biceps tendon ruptures at one university center participated in the study. In general, follow-up outcomes were favorable with respect to return to premorbid activity levels, patient satisfaction with surgical outcome, and overall clinical results. Elbow flexion, forearm supination, and upper extremity functional concentric peak torque and range of motion results were not significantly different between the surgical and nonsurgical arms when dominance was controlled as a confounding factor. Radiographic findings revealed no clinically remarkable signs of heterotopic ossification or proximal radioulnar synostosis. Results of the study reveal that the conventional and modified Boyd-Anderson procedures are clinically, functionally, and radiographically efficacious for repair of distal biceps tendon ruptures.

Effects and treatment of the menopause.

There are 10 million postmenopausal women in this country and another 250,000 join their ranks yearly. Nurses must have a good understanding of this stage of life if they are to support their patients, and allow these women to make informed choices about their own care.

Nursing intervention in the treatment of cataracts.

Cataract remains the leading cause of blindness in the world, affecting around half of the world's 27-35 million blind people (WHO, 1991). The only effective treatment currently available is the surgical extraction of the opacified lens. This article re-examines the condition and looks at the nursing action that is taken before and after cataract surgery.

Drug therapy and peptic ulceration.

Peptic ulceration can be treated by various types of drugs including antacids, protectors, antimicrobials and acid-secretion reducers. Nurses have a role to play in administering medicines, educating patients and, in future, prescribing drugs.

A simple data-processing system for the monitoring of cross-infection in a district general hospital.

A year's trail has demonstrated the effectiveness of a simple data processing system in the early identification of cross-infection problems at their source. The system has particular advantanges for small and scattered surgical units where cross-infection monitoring is particularly difficult because patients are transferred before their treatment is complete. A simple extension of the system could audit the efficiency of preventive antiseptic measures, particularly aseptic techniques, and provide antibiotic profiles.

The really useful guide to portfolios and profiles (continuing education credit).

It is generally accepted that there is value in developing personal professional portfolios within nursing. This assumption is based on its inclusion as one of the main features of the UKCC's Standards for Education and Practice Following Registration (1): the requirement that all practitioners should maintain a personal professional profile. The fundamental purpose of the personal professional profiel is to 'provide a means for showing that nurses, midwives and health visitors have maintained and developed their professional knowledge and competence' (1). However, there is the potential to achieve much more than this rudimentary outcome. This Unit is relevant to UKCC Professional Development category Educational Development.
