An unusual case of ileocaecal tuberculosis in an 80-year-old Caucasian male.

An 80-year-old Caucasian male presented with a seven-week history of diarrhoea and weight loss. Distal duodenal biopsies showed partial villous atrophy but he failed to respond to a gluten-free diet. Subsequently he developed a right iliac fossa mass associated with radiological evidence of ileocaecal ulceration. Colonoscopic biopsies from the caecum showed non-caseating granulomata and Ziehl-Neelsen (ZN) staining and culture for acid-fast bacilli (AFB) were negative. Crohn's disease was diagnosed and he was started on steroids. Although he showed an initial response, his condition then deteriorated and he died after six weeks. ZN staining of tissue at postmortem showed AFBs. Although a rare diagnosis in the UK, a high index of suspicion should be maintained for ileocaecal TB in patients with appropriate clinical features, even if classical risk factors for TB are absent.

Gas chromatographic determination of volatile congeners in spirit drinks: interlaboratory study.

An interlaboratory study of a gas chromatographic (GC) method for the determination of volatile congeners in spirit drinks was conducted; 31 laboratories from 8 countries took part in the study. The method uses GC with flame ionization detection and incorporates several quality control measures which permit the choice of chromatographic system and conditions to be selected by the user. Spirit drink samples were prepared and sent to participants as 10 blind duplicate or split-level test materials for the determination of 1,1-diethoxyethane (acetal), 2-methylbutan-1-ol (active amyl alcohol), 3-methylbutan-1-ol (isoamyl alcohol), methanol (methyl alcohol), ethyl ethanoate (ethyl acetate), butan-1-ol (n-butanol), butan-2-ol (sec-butanol), 2-methylpropan-1-ol (isobutyl alcohol), propan-1-ol (n-propanol), and ethanal (acetaldehyde). The precision of the method for 9 of the 10 analytes was well within the range predicted by the Horwitz equation. The precision of the most volatile analyte, ethanal, was just above statistically predicted levels. This method is recommended for official regulatory purposes.

Severe hyponatraemia secondary to beer potomania complicated by central pontine myelinolysis.

A case of severe hyponatraemia in a 56-year-old male alcohol misuser secondary to beer potomania is presented. In view of severe volume depletion and the patient's inability to drink, normal saline was cautiously infused. Despite initial improvement, he subsequently deteriorated neurologically. Magnetic resonance imaging demonstrated the classical lesion of central pontine myelinolysis (CPM). The rate of correction of plasma sodium was within limits normally considered safe. Beer potomania should be considered as a cause of hyponatraemia in alcohol misusers. Recognition is important as the electrolyte imbalance repairs simply with cessation of alcohol intake and institution of normal diet. Correction of chronic hyponatraemia by infusion of normal or hypertonic saline should not be attempted unless life-threatening neurological complications supervene. When the balance of risks favours correction, caution should be exercised, as CPM may occur. Although a rate of correction of plasma sodium of up to 10 mM per 24-hour period has been associated with a low risk of precipitating CPM, this case illustrates that a completely safe rate of correction probably cannot be defined.

Diabetes care: a comparison of management systems.

During 1997, the Alaska Area Diabetes Program audited a sample of 837 charts from among the 1373 individuals receiving regular diabetes care at facilities serving predominantly American Indians/Alaska Natives. Charts were evaluated for the delivery of American Diabetes Association Standards of Care. Results of the audit showed a great deal of variability among facilities. Facilities which scored highly in the audit were more likely to use an organized multidisciplinary team approach that included coordinated clinic appointments with multiple providers on the same day, maintenance of a diabetes registry, proactive preclinic preparation, flow-sheet use, intensive individual nutritional counseling, a case manager or field clinic coordinator system with standing orders, and strong self-management support.

Determination of 3-chloro-1,2-propanediol in foods and food ingredients by gas chromatography with mass spectrometric detection: collaborative study.

The results of a collaborative study are reported for the determination of 3-chloro-1,2-propanediol (3-monochloropropane-1,2-diol; 3-MCPD) in a wide range of foods and food ingredients, using gas chromatography with mass spectrometric detection and incorporating the use of a deuterated internal standard. After a pretrial study, 12 laboratories (6 United Kingdom, 1 Switzerland, 1 Japan, 2 United States, 1 The Netherlands, and 1 from the European Commission) were asked to analyze 12 test materials (as known duplicates or split-level samples) by using a prescribed procedure. The test materials consisted of duplicate samples of acid-hydrolyzed vegetable protein (containing 3-MCPD at 0.029 mg/kg), malt extract (0.055 mg/kg), wholemeal bread crumbs (0.030 mg/kg), salami (0.016 mg/kg), cheese alternative (0.043 mg/kg), and soup powder (split levels at 0.045 and 0.041 mg/kg). Repeatability ranged from 0.005 to 0.013 mg/kg and reproducibility, from 0.010 to 0.027 mg/kg, for the samples tested. Precision values were well within statistically predicted levels (HORRAT values of <1 for 5 of the 6 matrixes tested) and within method criteria prepared by a joint working group composed of the United Kingdom Ministry of Agriculture, Fisheries and Food and industry representatives. The study demonstrated the satisfactory validation of the method for quantifying 3-MCPD at levels of > or = 0.010 mg/kg. The limit of detection derived from separate in-house studies was estimated to be 0.005 mg/kg. The method was adopted First Action by AOAC INTERNATIONAL.

Dads and Daughters: grassroots advocacy.

Dads and Daughters (DADS) is a grassroots organization of fathers. Its mission is to provide tools to both strengthen father-daughter relationships and to transform the pervasive cultural messages that value girls more for how they look than for who they are. The DADS philosophy is captured in ten "Tips for Fathers with Daughters" which include: (a) listen to girls; (b) encourage their strength and savvy; (c) respect girl's uniqueness, urge them to love their bodies, and discourage dieting; (d) get physically active with girls; (e) get involved in their schools; (f) get involved in their activities; (g) help make the world better for girls; (h) take daughters to work; (i) support positive alternative media for girls; and (j) talk to other fathers. DADS urges health care providers to actively involve fathers in their daughter's preventive health and illness care.

Pelvic actinomycosis and usage of intrauterine contraceptive devices.

Pelvic inflammatory disease (PID) is one of the most commonly encountered serious infectious disease entities in gynecology. The past decade has witnessed many advances in our understanding of the pathogenesis of PID. It is now evident that such pelvic infections are largely polymicrobial in origin, with major involvement by anaerobic organisms. Salpingo-oophoritis is a part of the spectrum of PID. Included among this group of infections are tubo-ovarian abscesses, traditionally referred to as either gonococcal or non-gonococcal in origin. Within the latter group of infections the importance of anaerobic organisms has also been elucidated. Of particular interest is the reported observation of an increased frequency of salpingo-oophoritis among users of intrauterine devices (IUDs). These reports have noted the specific occurrence of serious pelvic infections due to Actinomyces species, and this will be the topic of the infectious disease conference. Our patient presented with a chronic illness characterized by lethargy, back pain, fever, and anemia; subsequently evaluation disclosed the presence of a large pelvic mass which was confirmed as a tubo-ovarian abscess at surgery. Histological evaluation demonstrated involvement by Actinomyces species. This patient's illness is discussed as a complication of chronic IUD usage with reference to specific management for this emerging problem.

Making sense of drug compliance by patients.

Success in the treatment of patients frequently relies on their taking prescribed medication. However, patients do not always comply. Some of the reasons for this are considered together with some of the means by which compliance can be improved. It is suggested that the patient requires sensitive individual attention in order to understand and use his or her medication to its best effect.
