Uvulectomy, a traditional surgical procedure in Tanzania.

Uvulectomy is carried out by traditional healers of Tanzania. The Department of Otorhinolaryngology of the Muhimbili Medical Centre, University of Dar es Salaam, has regularly treated patients who suffered complications arising from this procedure. In April 1980 the author attended the practice of a traditional healer (mganga) in Dar es Salaam and saw many uvulectomies performed. The procedure is described and its indications and complications reviewed. The study illustrates that knowledge and comprehension of traditional medicine is important for the medical profession in those countries where traditional medical procedures are still in use.

[Primary carcinoid of the middle ear. A case report with reference to light and electron microscopy findings].

A rare case of carcinoid tumour of the middle ear in a 51-year old woman is presented. The clinical features and the light and electron microscopical appearances are discussed. Carcinoid tumour should be included in the differential diagnosis when tumours of the middle ear present with adenomatous pattern. Electron microscopy will identify characteristic intracellular neurosecretory granules. Formaldehyde-induced fluorescence corresponding to the pattern of distribution of these granules confirms the diagnosis of carcinoid tumour.

Voice rehabilitation after laryngectomy with the Groningen prosthesis.

The Groningen voice prosthesis is a recently developed silicone device for surgical rehabilitation of the voice following total laryngectomy. The prosthesis is placed in the tracheo-esophageal wall as a primary procedure during laryngectomy or as a secondary procedure some time after surgery. The insertion is a one stage surgical technique. The prosthesis is self-retaining and remains in situ, on average for three months. Its replacement is a simple outpatient procedure. Successful speech was achieved in 80% of the patients rehabilitated as a primary procedure. Aspiration of liquids or food was not observed. The Groningen prosthesis has several advantages over other currently available devices.

Experiences with myocutaneous island flaps of the pectoralis major muscle for reconstruction of defects in the bucco-pharyngeal cavity.

Defects in the bucco-pharyngeal cavity after tumor resection were closed with pectoralis major myocutaneous island flaps in ten consecutive patients. The skin element consisted of an island which was transposed on a muscular pedicle containing the vascular stalk. The advantages and disadvantages of the method and the results and complications in the treated patients are discussed. When the defects mentioned require pedicled skin flaps, the pectoralis major island flap is preferred.
