The prevalence of tuberculous laryngitis in pulmonary tuberculosis in Tanzanians.

To assess the prevalence of tuberculous laryngitis in Tanzanians a study was made of 341 patients admitted for pulmonary tuberculosis to the chest unit of Muhimbili Medical Centre, University Hospital of Dar-es-Salaam. Laryngeal involvement, assessed by indirect laryngoscopy, was present in 27% of previously untreated patients. Hoarseness, which invariably accompanied lesions in the glottic region, was found in 53%. If a patient with pulmonary tuberculosis complains of hoarseness then concomitant tuberculous laryngitis is the first differential diagnostic consideration. Tuberculous laryngitis is usually related to advanced stages of pulmonary tuberculosis. Indirect laryngoscopy is an essential part of routine investigations for pulmonary tuberculosis.

Pulmonary complications of methotrexate therapy.

Diffuse interstitial pulmonary infiltration was first reported in 1969, occurring during intermittent methotrexate therapy and it is suggested that it represents an adverse reaction to this drug. Despite the severity of the clinical and radiological changes, the disease is usually self-limiting. An additional case is presented and the previous 28 reported cases of this syndrome are summarized. Since intermittent methotrexate therapy is becoming more widely used, the importance of recognizing this respiratory illness as a possible complicating reaction to the drug is stressed.

Endolaryngeal synovial sarcoma: case report with immunohistochemical studies.

Endolaryngeal synovial sarcoma has only been reported in the literature three times. We add to this a fourth report of a 28-year-old woman. This case distinguishes itself by being a monophasic variant and by being the first to be substantiated with extensive immunohistochemical analysis. Treatment consisted of shelling-out followed by radiotherapy, which is a variant from standard therapy. The patient is tumor-free 3 years after therapy.

Cytokeratin expression in normal and (pre)malignant head and neck epithelia: an overview.

Intermediate filament proteins (IFPs) are important markers of tissue differentiation and have been receiving increasing interest, in particular, through their applicability in the characterization of malignant tumors. Cytokeratins (Cks) are a family of IFPs that are typically specific for epithelial cells. They are expressed in certain combinations depending on the type of epithelium and the degree of differentiation. This review presents a critical analysis of the available data on Ck expression in normal and (pre) neoplastic epithelia of the head and neck region. Special attention is paid to technical and cell biologic pitfalls, which can lead to false-negative or false-positive data. It appears that only a limited fraction of the reported data contributes substantially to our knowledge of IFP expression in head and neck cancer because of the use of ill-defined, often formalin-fixed and paraffin-embedded, tissue specimens, and the application of limited panels of monoclonal antibodies. It is concluded that the use of immunocytohistochemistry is promising for the differential diagnosis of head and neck tumors and contributes to our knowledge on their biologic behavior. However, documentations of more complete Ck expression patterns of normal and (pre)malignant epithelium are required, together with their correlation to clinical parameters.

Vertical partial laryngectomy for recurrent glottic carcinoma after irradiation.

Vertical partial laryngectomy after a full course of irradiation was performed in 26 patients as salvage surgery for recurrent or residual glottic carcinoma. An excellent cure rate and conservation of function was obtained, while complications were few. A prerequisite for this type of surgery is a limited extent and infiltration of the recurrent local tumor growth. As the great majority of local recurrence appears within the first 2 years after radiotherapy, frequent follow-up during this period is stressed for early detection of a recurrence.

Intermediate filament proteins in spindle cell carcinoma of the larynx and tongue.

In an analysis of intermediate filament protein expression of spindle cell carcinoma, a variant of squamous cell carcinoma, occurring in larynx and tongue, vimentin positivity was found in sarcomatoid areas in 12 of 13 patients. Scattered expression of keratin was observed in sarcomatoid areas in tumours of 10 patients. However, large parts of sarcomatoid areas of such tumours were negative for keratin. Overlying dysplastic epithelium and squamous cell carcinoma components were positive for keratin. In a number of cases there was a strong indication of co-expression of keratin and vimentin in parts of cells that, on histological grounds, belong to sarcomatoid areas or to cells in the interface between carcinoma and spindle cell area. The use of keratin and vimentin type intermediate filament antibodies could be of great help for the correct classification of these tumours.

A longitudinal study of the validity of parental reporting in the detection of otitis media and related hearing impairment in infancy.

A total of 150 full-term and 66 pre-term infants were selected at birth and prospectively examined at three-monthly intervals from birth until the age of 27 months. Parental reports of middle ear infection and/or hearing impairment were obtained prior to otoscopic and audiometric evaluation. The relationships between parental reports and the diagnoses acute otitis media (AOM), otitis media with effusion (OME) and hearing impairment (HI), were assessed in terms of sensitivity, specificity and positive and negative predictive values, using the data obtained during the follow-up period. AOM and OME were diagnosed using otoscopy and tympanometry. Hearing was assessed by conditioned orientation reflex audiometry. HI was defined as averaged thresholds > or = 20 dB compared with age-appropriate response levels. Despite the repeated feedback which parents received on the middle ear condition and hearing of their infants, the majority fail to recognize the presence of AOM, OME and HI. The limited sensitivity of parental reporting should discourage both researchers and clinicians from using it as a diagnostic or monitoring instrument, but they should not disregard parental concern when confronted with it.
