The role of yearly chest radiography in the early detection of lung cancer following oral cancer.

In a study of 213 patients with oral cancer, we investigated the incidence and prognosis of lung malignancies in patients offered a yearly chest radiography in the follow-up. Three conclusions can be drawn. (1) Metastatic or primary lung cancer was diagnosed in 22 (10.3%) patients. The 2-year actuarial incidence rate of lung cancer following cancer of the oral cavity is 13%. No new lung cancers were detected after 2 years follow-up. This suggests that after this period, yearly chest radiography may be superfluous for the early detection of lung cancer. (2) The survival rate of patients with a lung malignancy following cancer of the oral cavity is poor (1-year = 25%). The survival rate of patients detected by the yearly chest radiography without symptoms is higher than for patients detected after symptoms (p = 0.006). It is not clear to what extent this different survival rate is biased by lead-time and selection of patients with a favourable prognosis. A randomized study would be required to assess whether patients with oral cancer do benefit from the yearly chest radiography compared with no regular chest radiography. (3) Of 22 patients with lung cancer, 13 (59%) were detected by chest radiography without symptoms. In the first year following oral cancer, 11 patients were diagnosed with lung cancer. Only 4 of these 11 patients (36%) were detected by chest radiography in an asymptomatic stage. The detection of patients with lung cancer in an asymptomatic stage may be increased by more frequent chest radiography examinations in the 1st year following oral cancer.

Cochlear and vestibular functions of the rat after obliteration of the endolymphatic sac.

A combined morphological and physiological study on the effect of saccus obliteration on the cochlea and the vestibular labyrinth of the rat is presented. Endolymphatic hydrops was successfully induced in 49% of the animals. It was frequently associated with fistulae of the membranous wall and degenerative changes in the organ of Corti and spiral ganglion. In hydropic ears electrocochleography revealed a CAP threshold increase only in those cases where hydrops was associated with loss of hair cells and ganglion cells. In two of these animals also an increased negative SP was present. Static otolith reflexes were found to be normal in all hydropic ears despite severe lesions of the saccule in some of these. Abnormal canal reflexes measured by electronystagmography and cupulometry were established in animals with hydrops of the semicircular canals and in one animal with a fistula of the saccule.

Longitudinal flow of macromolecules in the endolymphatic space of the rat. An autoradiographical study.

Secretion and resorption of sulphated glycoproteins by the epithelial lining of the endolymphatic space of the rat were studied autoradiographically with the use of 35SO4 as a selective marker. This isotope was found to be incorporated into the sensory area of cristae and maculae and a small area of epithelial cells adjacent to the sensory epithelium. Thereafter 35S-labelled glycoproteins were secreted into cupulae and otolithic membranes. No such process could be established in the tectorial membrane. From the gradual disappearance of 35S-labelled glycoproteins from cupulae and otolithic membrane coupled with an accumulation of this substance in the lumen of the endolymphatic sac, the existence of a longitudinal flow under physiological conditions could be concluded. Resorption of glycoproteins by the saccus epithelium was found to be very slow. Accumulation of 35S-labelled glycoproteins in the endolymphatic duct after saccus obstruction indicates that, in addition to the endolymphatic sac, also the duct contributes to the mechanism underlying the longitudinal flow of macromolecules.

Otitis media in Dar es Salaam, Tanzania.

To assess the prevalence of otitis media in Tanzania a study was made of 3772 patients attending for the first time the ENT out-patient department of Muhimbili Medical Centre, University Hospital of Dar es Salaam. Otitis media in one form or another was present in 524 patients (14 per cent). The majority reported a long history of ear discharge. Hearing loss was a major complaint and was often found to be between 40-60 dB. H.L. Most patients admitted previous treatment, but otoscopy had rarely been performed. The medical profession in Tanzania cannot possibly deal with the large number of cases of chronic otitis media. With the recent introduction of otorhinolaryngology into the curriculum of the medical school of Tanzania it seems advisable to include otology in the programme for primary health workers.

Supraglottic laryngectomy: short- and long-term functional results.

A supraglottic laryngectomy offers the possibility of resection of a large portion of the larynx leaving the true vocal cords. From 57 supraglottic laryngectomies performed between 1971 and 1984 in the Department of Otolaryngology and Head and Neck Surgery of the University of Nijmegen, the 5- and 10-year actuarial survival rates were 80% and 59%, respectively. The deaths after 5 years were not directly related to the original tumour. The functional results with respect to the voice were excellent. In this study particular attention has been paid to the postoperative results with regard to swallowing and aspiration. More than half of the patients (56%) had occasional complaints of aspiration, especially when taking liquids. It is concluded that supraglottic laryngectomy is an excellent alternative, especially for the treatment of extensive supraglottic tumours without involvement of the true vocal cords.

Experimental endolymphatic hydrops in the rat.

In the rat, the epithelial lining of the endolymphatic duct and sac, and the contents of the sac, showed remarkable differences in comparison with these structures in the guinea pig and man. Obliteration of the endolymphatic duct resulted in endolymphatic hydrops of varying severity in 55% of the rats, after survival times varying from one to five months. The presence of a positive periodic acid-Schiff-stain for coagulum accumulating in the endolymphatic duct after prolonged saccus obliteration suggests active involvement of the endolymphatic duct in the migration of endolymph toward the endolymphatic sac.

Malignant fibrous histiocytoma of the tongue.

Malignant fibrous histiocytoma (MFH) can occur as a rare mesenchymal neoplasm of the deep structures of the head and neck region. An unusual case of MFH of the tongue is described in a 61-year-old male. The primary tumour measured 90 X 50 X 30 mm. Treatment consisted of hemiglossectomy in continuity with a supra-omohyoidal neck dissection. The patient has been free from disease for 2 years. The tongue as a primary site of MFH has not been reported to date. The literature of oral manifestations and treatment of this neoplasm is reviewed.

Developmental aspects of glycoprotein secretion and migration in the endolymphatic space.

Secretion and migration of sulphated glycoproteins during the early postnatal development of the rat inner ear was studied autoradiographically with the use of 35SO4. This isotope was found to be selectively incorporated into the embryonal sulcus cells and subsequently expelled into the tectorial membrane. This process decreased simultaneously with the replacement of the tall sulcus cells by cuboidal cells and the detachment of the tectorial membrane from these cells. A two stage development of the tectorial membrane is postulated: formation of filaments followed by incorporation of glycoproteins. Incorporation of isotope was also observed in the sensory epithelium of the vestibular part of the labyrinth, followed by migration into the cupulae and otolithic membranes. The accumulation of labelled material in the lumen of the endolymphatic sac 6 hours after isotope administration, suggests the existence of endolymph flow towards the endolymphatic sac in the early postnatal period.

[Sleep apnea syndrome, snoring and uvulopalatopharyngoplasty].

Sleep-related closure of the upper airways is the key feature of the obstructive sleep apnoea syndrome (OSAS). Excessive daytime sleepiness and loud snoring are the major signs. It is not possible to differentiate patients with the single complaint of habitual snoring from patients with sleep apnoea by their history alone; polysomnography is a mandatory procedure. Recently, uvulopalatopharyngoplasty (UPPP) has been introduced as a surgical procedure to treat OSAS and excessive snoring. This paper presents the results of UPPP in eleven patients evaluated by means of postoperative polysomnography.

[Obstructive sleep apnea syndrome in children; an indication for tonsillectomy and adenoidectomy].

A 3-year-old girl with obstructive sleep apnea syndrome associated with tonsillar and adenoid hypertrophy is discussed. Nocturnal sleep registration, 4 months after adeno-tonsillectomy revealed complete arrest of apnea episodes and there was improvement in sleep pattern. In the early detection of obstructive sleep apnea in children a careful history and nocturnal sleep registration are essential. In children, tonsillar and adenoid hypertrophy are the main causes of obstructive sleep apnea.

Carcinoid of the larynx: a report of three cases and a review of the literature.

Carcinoid tumors are among the exceptional neoplasms of the larynx. The laryngeal carcinoid was first described in 1969 by Goldman, et al. Only 16 cases could be traced from the literature. This paper describes three additional cases. In one patient, the tumor infiltrated the laryngeal tissues and metastasized to the cervical lymph nodes breast. In the second case, the polypoid lesion could be excised completely. The third case was initially diagnosed as undifferentiated squamous cell carcinoma. The diagnosis of carcinoid was made a few years later when a metastasis was removed. The presence of intracellular membrane bound neurosecretory granules and positive argyrophilia in the Grimelius stain are characteristic. In half of the cases described in the literature, the tumor was initially misdiagnosed as undifferentiated carcinoma. The treatment of choice is surgical removal which may be curative. The literature is reviewed.
