Biamnial alpha fetoprotein concentration in twins, one with multiple malformations.

In the 18th week of pregnancy in a 22 year old patient, twins were diagnosed by ultrasound. It was found that one twin had an abnormal skull outline and an echo-free area covered by a thin membrane in the region of the abdomen. The second embryo showed no sign of malformation. Amniocentesis was performed and the AFP in both samples of amniotic fluid were in the pathological range. Our own observations with indirect immunofluorescence confirmed that one twin with defects leading to abnormally high AFP levels can cause pathological AFP levels in the amniotic sac of a healthy twin.

Local application of PGE2 by means of a portio-adapter. A new method of induction of labor.

In 206 pregnant women with unfavorable cervix, labor was induced by local application of 1.5 mg PGE2 by means of a portio-adapter. In 70.4% labor was successfully induced after a single "PG-cap". An additional 11.6% went into labor after a second or third cap. In 14.1%, a sufficient priming of the cervix was obtained and amniotomy was performed. 3.9% had to be considered failures. The overall incidence of operative deliveries was 17%. Apgar scores and cord gases were within normal ranges. Internal tocography revealed a physiologic pattern of contractions with slightly elevated maximal pressures but normal baseline values. With local application of PG by means of a portio-adapter the preparation of a gel mixture can be avoided and the exact anatomic location of the PG-containing solution allows a withdrawal in case of hyperstimulation. From the results of this study it is concluded that local application of 1.5 mg PGE2 by means of a portio-adapter is a safe and effective method of induction of labor.

[Microbiological degradation of glucosinolates in defatted rapeseed meal].

During the degradation of thioglucosides in defatted rape seed meal (RES) microorganisms were found, whose ability to degrade glucosinolates (GSL) and vinyl thio-oxazolidone (VTO) was not known so far. The isolated microorganisms are two strains of bacteria of the species Bacillus cereus and the yeast Trichosporon cutaneum. The degradation of GSL and VTO in the cultural broths by the Bacillus cereus strains was the more complete the more other Gram-negative bacteria from RES were additionally present. Drinking tests with Wistar rats showed that here is a relation between the watersoluble and mainly bitter toxin substances of rape and their influence on the increase of the animals body weight.

Threshold pressure for the pressure-dependent renin release in the autoregulating kidney of conscious dogs.

The effect of varying renal artery pressure between 160 and 40 mm Hg on renal blood flow and renin release was studied in seven conscious foxhounds under beta-adrenergic blockade receiving a normal sodium diet (4.1 mmol/kg/day). Pressure was either increased by bilateral common carotid occlusion or reduced in steps and maintained constant by a control-system using an inflatable renal artery cuff. Carotid occlusion itself had no influence on renal blood flow and renin release when renal artery pressure was kept constant and the beta-receptors in the kidney were blocked. Between 160 mm Hg and resting pressure there was no change in renal blood flow; between resting blood pressure and the lower limit of autoregulation (average 63.9 mm Hg) renal blood flow increased slightly (average 7%) indicating a high efficiency of renal blood flow autoregulation. The relationship between renal artery pressure and renin release could be approximated by two linear sections: a low sensitivity to a pressure change (average slope: -0.69 +/- 0.26 ng AI/min/mm Hg) was found above a threshold pressure (average: 89.8 +/- 3.3 mm Hg) and a high sensitivity to a pressure change (average slope: -64.4 +/- 20.8 ng AI/min/mm Hg) was observed between threshold pressure and 60 mm Hg. Bioassay procedures for norfloxacin.

A disk-diffusion bioassay procedure for the quantitation of norfloxacin in serum, tissue and urine employing Klebsiella pneumoniae ATCC 10031 was developed. When incubated at 28 degrees C, the assay has a sensitivity of 0.2 microgram/ml. Zones of inhibition on assay plates may be measured by hand (ruler, vernier caliper) or by automated image analysis. Urine samples require an initial 1:2 dilution in 0.1N HCl before assay to assure complete solution of the compound. Bioassay values in urine are comparable to those obtained by high performance liquid chromatography.

[Urinary tract infections after gynecological surgery].

In a ten year period we selected at random 235 patients, who underwent a vaginal or abdominal hysterectomy or a radical operation (Latzko-Meigs) and evaluated their postoperative morbidity with special regard to urinary tract infections. In the period of 1971 to 1979 we observed a significant increase of urinary tract infections following vaginal hysterectomy, especially after additional colporrhaphia, as well as a significant increase of febrile postoperative phases after radical hysterectomy. Also a shift towards so called "nosocomial bacteria" (Klebsiella, Aerobacter and Pseudomonas) was observed. We therefore changed our perioperative management of Latzko-Meigs operation (closed urine catheter system, perioperative antibiotic prophylaxis). The comparison of the groups before and after this change show a significant decrease of all signs of postoperative morbidity observed.

Mechanical properties of temporary ligament replacement.

The mechanical properties of possible polymer ligament replacement materials are established by static and dynamic testing of about 50 specimens. In accordance to approximate physiological conditions the examinations demonstrate only one entangled polyester-flat-ligament to be satisfying the requirements.

Chromosome abnormalities in 150 couples with multiple spontaneous abortions.

One hundred fifty consecutive couples with recurrent abortions with or without fetal deaths, malformations, or healthy infants were evaluated cytogenetically with a G-band method. Seven couples (4.7%) showed a chromosome aberration in one partner (5 translocations, 1 inversion, and 1 gonosomal mosaicism). Of the couples with a pure abortion history, 2.9% had a chromosomal abnormality. A high incidence of chromosomal rearrangements in the groups of couples with abortions plus fetal deaths or malformations was found. In contrast to other reports, a high incidence of cytogenetic abnormalities was also found in couples with abortions and healthy infants. Chromosome analysis is advocated as a primary tool in the evaluation of couples with repetitive fetal wastage.
