A prospective, randomized, controlled trial was designed to study the influence of a corrugated latex drain on anastomotic integrity, wound infection, and respiratory complications after elective colonic resections above the pelvic peritoneum. Sixty patients entered the trial; 28 were drained and 32 undrained. Three patients in the undrained group died soon after surgery of causes unrelated to their colon anastomoses. There were no other differences in the incidence of postoperative complications in the two groups. When an anastomosis leaked, neither feces nor pus emerged via the drain. It is concluded that while the drains did not increase the incidence of postoperative complications, neither did they accomplish the purpose for which they were inserted. If, in addition, the economic cost of drainage is considered, it would seem that there is no advantage in inserting prophylactic drains after colonic anastomoses and their routine use needs to be reconsidered.
A superior mesenteric vein aneurysm was diagnosed by ultrasound, computed tomography, and angiography in a 55-year-old woman. It was successfully treated by partial resection and reconstruction of the confluent veins.
Two cases of abdominal stab wounds with omental protrusion treated conservatively are presented. Omental protrusion in a patient without signs of shock or peritoneal irritation is not an absolute indication for explorative laparotomy.
Twenty-three selected patients presenting with massive bleeding from peptic ulcers underwent emergency parietal cell vagotomy (PCV). Nineteen patients had a duodenal ulcer, two a prepyloric ulcer, and one a gastric ulcer. The patients were studied retrospectively with regard to postoperative mortality and morbidity, early rebleeding, and recurrent ulceration. Two patients (9%) died after operation, one of rebleeding. No others suffered rebleeding. One further patient had major respiratory complications and 14 others developed minor complications. The remaining 21 patients were followed for between 2 and 72 months. Two patients (10%) developed recurrent ulcers. The authors conclude that parietal cell vagotomy may be used as an emergency operation for bleeding peptic ulcer in selected cases with an acceptably low mortality, rebleeding rate, and incidence of recurrent ulceration.

Blue poppy seeds, semi finished products and finished baked goods with poppy seeds, were analyzed for cadmium by atomic-absorption spectrometry. The average content was 0.739 mg/kg (median 0.654 mg/kg) for poppy seeds, 0.317 mg/kg (median 0.366 mg/kg) for semi finished products and 0.107 mg/kg (median 0.088 mg/kg) for baked goods. Poppy seeds therefore must be considered as one of the most cadmium contaminated foods. For consumers, especially children, recommendations for consumption should be established.
Sixty patients with recurrent ulcers after vagotomy were treated by partial gastrectomy without revagotomy. One patient died after surgery. Seven others died of unrelated causes and one patient was lost to follow-up before 5 years, without rerecurrence having been diagnosed. Of the remaining 51 patients, one underwent reoperation for rerecurrent ulcer and four for postgastrectomy symptoms before 5 years of follow-up. Forty-six others were followed for between 5 and 19 years, in addition to the one postoperative death. Major postoperative complications occurred in four other patients. Recurrent ulceration developed in four of 51 patients. The patients were classified according to a modified Visick system with regard to recurrent ulceration and postgastrectomy symptoms: Visick I, 16 patients; Visick II, 14 patients; Visick III, 11 patients; and Visick IV, 11 patients. Gastrectomy after vagotomy can be achieved with a low mortality and recurrent ulcer rate but with a high incidence of postgastrectomy symptoms.

A 16-year-old female complained of loosening of several teeth in the lower jaw. Radiography revealed bone resorption of the tooth sockets. The osteolytic process extended throughout the alveolar crest of the mandible and caused a complete loss of teeth within 2 years. Microscopic sections showed angiomatous tissue of Gorham-Stout-syndrome. Radiotherapy with 35 Gy controlled the mandibular osteolysis. Recurrent meningitis and osteolytic defects of the frontal skull base hint at an additional manifestation of the disease at this site.
The value of peritoneal lavage (PL) in the evaluation of 82 patients with equivocal signs after abdominal trauma was studied. The closed technique using an Intracatch (R) was employed. Fifty-four patients had blunt trauma, the predictive value of a positive PL was 86% and the predictive value of a negative PL was 94%, accuracy 94%. Twenty-eight patients had stab wounds, the figures in this group were 100%, 92% and 93%. No complications were seen. There were no retrieval of lavage fluid in 10% of the cases. PL with the closed Intracath (R) technique has the same diagnostic certainty as other methods, but is much quicker to perform and has no complications.
