[Effectiveness of different intervention measures in children and adolescents with hypertension and lipid metabolism disorders].

141 children and adolescents (7-21 years, mean age 14.2 years) showed values of blood pressure, total cholesterol and triglycerides greater than or equal to 90 percentile and HDL-cholesterol values less than or equal to 10 during an examination of 639 children of parents with distinct coronary risk factors or early percentile infarction. By randomization they were subdivided into two groups. (A and B) and underwent intervention measures of different intensity. While group B got only a unique recommendation concerning the preventive measures, in group A a regular, non medicamentous individual treatment was performed. After one year a decrease of blood pressure could be proved in the two groups. A significant decrease of total and HDL cholesterol was found only in group A. While the blood pressure was most clearly reduced in 7-13-year-old children, the most distinct lipid changes were shown in 14-17-year-old adolescents. An influence on body-weight and triglycerides could not be established. Our results confirm the possibilities and also the limits of preventive measures in childhood and adolescence. Apart from the intended decrease of blood pressure and total cholesterol the simultaneous decrease of HDL-cholesterol refers to open questions in the conception of the primary prevention, particularly in children.

A simple spectrophotometric assay of carboxypeptidase N (kininase I) in human serum.

Kininase I (carboxypeptidase N; EC 3.4.17.3) consists of carboxypeptidase N1 (CN1) and carboxypeptidase N2 (CN2); these two enzymes can be differentiated by their activities towards hippuryl-L-arginine and hippuryl-L-lysine, respectively. A spectrophotometric assay for both carboxypeptidases in human serum is described and the biochemical behaviour of these enzymes investigated. The pH optima are found to be 8.4 for CN1 and CN2. The Michaelis-Menten constants are: CN1 4.59 +/- 0.03 mmol/l; CN2 37.26 +/- 3.49 mmol/l. CN2 can be inhibited by EDTA (76%), dimercaprolum (97%) and phenanthroline (98%). Diisopropylfluorophosphate has no influence on both enzymes. Elevated haemoglobin only interferes with CN1 measurements, and high bilirubin concentrations slightly alter the activity of both enzymes. High CN1 activities were found in sera of patients with sarcoidosis, and elevated CN2 activities were found in lung cancer.

[The effectiveness of a prevention program for cardiovascular diseases].

In 1968 the addresses of all men of the annual courses of birth 1914-1923 of an Erfurt district were registered (n = 2 023). The aim population was according to accident subdivided into an intervention and a control group. The test persons of the intervention group were repeatedly investigated and participated in a preventive programme. Up to the final investigation in 1977 the control group remained absolutely intact. After about 10 years the intervention population had a statistically significantly lower frequency of infarction and a lower proportion of hypertensive values of blood pressure. In the younger five-year age group the total mortality was tendencially lower (not significant!) than in the adequate control group. There are no differences in the mortality from heart and circulatory diseases and in the distribution of other risk factors between intervention and control groups. A positive effect of the preventive programme was to be proved theoretically as well as empirically, though the decrease of the real incidence of infarction which is to be calculated was by far lower than generally was supposed.

Elective fetal monitoring and obstetrical operative frequency.

This paper reports on cardiotocographic findings after the introduction of elective fetal monitoring. A classification into the following four groups is recommended: normal, warning, severe functional hemodynamic disorders (SFHD) and hypoxia. This fetal heart frequency (FHF) classification prefers the evaluation of the effects on fetal condition compared with a descriptive classification. SFHD in the first stage of labor can be observed in 4% of parturient women, increasing to nearly 16% in the second stage. Cesarean section rate in this group amounts to 12.5% and forceps rate exceeds 50%. Under the condition of elective fetal monitoring (94% in total) and a reserved application of cesarean section (4.3% in total) and generous forceps delivery (15% in total) a perinatal mortality rate of 13% for obstetrical material with high-risk concentration is achieved. The total acidosis rate (pH less than 7.20) decreases to 2.3%, whereas the rate of severe acidosis (pH less than 7.10) was observed in only 0.4% of the cases. With elective fetal monitoring a low cesarean section rate taking critical evaluation into consideration and using all conservative methods of treatment is obtained. By means of the generous application of the single or repeated bolus injection of 25 micrograms fenoterol and the operative shortening of the second stage of labor in the case of SFHD, the neonatal morbidity can be kept at a low level. The small number of fetal or maternal complications using the direct methods of fetal supervision and the careful operative termination of labor justifies the recommended obstetrical management.

[Continuous peridural anesthesia--results of fractionated application in comparison to catheter infusion].

This report presents 100 epidural analgesias using a catheter in a high risk group. Two groups are compared, one with intermittent manual injections, the other with constant infusion rate (CIR) by means of a syringe with bupivacaine solution 0,5% and a pump. The analgetic effect was stable, without fluctuations and was adequate during the first and the second stage of labour in the group with CIR. The CIR avoids plasma peaks with transient hypoxemia of the fetus. The high blood pressure in patients with severe pregnancy induced hypertension is more decreased with this technique. In comparison to deliveries without epidural analgesia the instrumental delivery rate was higher, the fetal outcome was equal. Severe complications are not seen. The indicated use of the epidural analgesia with CIR is recommended.

[Use of computer technic in cardiotocographic childbirth monitoring].

Reference is made in this paper to cardiotocographic monitoring of labour by the principle of selective alarm recording, using trend memory scope, TMS-2, and a modified cardiotocographic recorder, FTS-101.--Upper or lower limits of individually preselected thresholds of foetal heart rate as well as upper limits of amplitude, frequency, basal tonus, and Montevideo units of uterine contractions were chosen as criteria by which to signal alarm. High-economy monitoring and savings on recording paper are some of the advantages provided by the system. Automatic alarm function as well as cardiotocographic recording twelve minutes prior to, during, and any time after an alarm signal proved to be sufficient for proper documentation of findings.

[Prevention of hypertension of the city borough of Schwedt/Oder--results to date of a regional intervention program].

More than 1,100 women and about 1,400 men, detected as being hypertensives in population screenings for cardiovascular diseases and coronary risk factors on 30-59 year-old females and 30-64-year-old males 1975 and 1976, respectively, in Schwedt - participation rate in both screenings 78 per cent (6,514 females and 7,137 males, respectively) - underwent an intervention programme carried out by all the general practitioners, physicians for industrial medicine and internists, working in the out-patient clinics of the territory. But in 1979 only 500 of these women and approximately 800 of these men were under continuous control. These registration rates after both the screenings as well as after the first follow-up in women 1977, therefore, have to be considered as unsatisfying. That lack in registration is mainly up to the enormous fluctuation of patients - and inhabitants at all-, being a characteristic feature of the population of the newly built town of Schwedt. In order to overcome these effects of migration within the territory and to improve supervision in hypertensives, a new control system regarding the peculiarities of Schwedt was established. In contrary to the failing registration an efficient lowering of blood pressure levels was achieved in those patients cooperating well, revealing normotensive diastolic mean values and systolic mean values, ranging from normotension to the lower borderline pressures. In women who showed a more impressive effect of treatment than men, the effective lowering could be maintained over a 4-year-follow-up period up to now.

[Average time prognosis of hypertension in qualitatively different treatment protocols].

In 1969 within pilot studies in an Erfurt district altogether 254 patients with definitive hypertension as well as 208 patients with borderline hypertension of the male age groups from 1914 to 1923 were recognized. 135 test persons with definitive hypertension (RR--160/95 Torr) were treated antihypertensively within a clinical and ambulatory preventive programme for 8 years and compared with a control group of the same age (119 males of 45-54 years) concerning the presence of possible complications of hypertension, which was treated with unsatisfactory results. By a strict, permanently normotensive decrease of blood pressure--as pharmacotherapy above all the beta-receptor blocker propranolol alone or in the approved combination therapy with dihydralazine and/or hydrochlorothiazide was used--in connection with suitable measures for the intervention of other individual factors with effect on heart and circulation the risk of complications in definitive hypertension could be reduced. Taking into consideration the own long-term observations the necessity of a possibly early beginning medicamentous, antihypertensive permanent therapy of hypertension is without doubt, in borderline hypertension, however, it still remains actual subject of the discussion.

[Cardiotocographic diagnosis in pregnancy (author's transl)].

A diagnostic programme has been recommended for cardiotocography in pregnancy. It is designed to non-stress testing and includes the oxytocin exposure test for better assessment of both oscillations and accelerations, depending on foetal movement, without or following arousal stimulus. -- Up to 95 per cent of all findings are obtained from non-stress or negative oxytocin exposure tests which are highly dependable diagnostic indicators to sufficient oxygen supply to the foetus. A pathological oxytocin exposure result would call for differentiated selection of those frequency images which do support suspicion of hypoxia, but these account only to something between two and five per cent among all high-risk pregnancies under conditions of selective cardiotocography.

[The correlation of blood pressure, cholesterol and triglyceride concentration with the administration of hormonal contraceptives in women from 2 Erfurt large-scale plants].

Within the framework of medical check-ups of heart and circulatory system 1,296 women at the age from 15 to 49 years were examined in two Erfurt large plants. Women with hormonal contraception in the age groups 30-34, 35-39 and 45-49 showed statistically significantly higher mean values of the systolic blood pressure than women without oral contraception. In the same way the difference of the cholesterol mean value of 15.3 mg/100 ml and 11 mg/100 ml depending on the intake of ovulation inhibitors could be statistically ascertained in women at the age of 20-24 and 30-34 years. Women with hormonal contraception have statistically significantly higher triglyceride mean value in the age group 15-24 and 35-44 years than the women without oral contraception.
